In the end, everyone got what they wanted. Ernest got to live life on his own terms, and Madeline and Helen got eternity. But only Ernest was able to find happiness.

But at a price they had to pay. Lisle warned both women to take care of their bodies as they have to live with them. Neither Madeline nor Helen heeded the warning and paid for it. Unlike them, Ernest was the only one who listened to Lisle and saw the pitfalls of such an immortal life. This is why he left both Helen and Madeline once he knew they were using him.

I now realize Ernest did the right thing, and it’s scary to think about the lengths these two have to go to just to get around in public and not fall to pieces. Forever. I mean, in all this time, they didn’t truly accomplish anything.

The sad truth is, when your life is centered around yourself, why do anything great unless it enriches yourself? I'm reminded of an exchange in Mario Puzo's book THE GODFATHER when Vito Corleone is told about the wartime accomplishments of his son Michael, and he responds with, "He does these things FOR STRANGERS!" He believes that he should be doing great things for his family FIRST, and to Hell with the rest of the world.

Lisle did warn Helen and Madeline to take care of their bodies once they made that pact to gain immortality. Neither ended up listening to her and thus paid for their sins in the end. Ernest was a POS to both women in the beginning. However out of him, Helen and Madeline, he was the only one who heeded Lisle's warning to take the immortality seriously when he saw the pitfalls of such an immortal life. Ernest also realized that Helen and Madeline were just using him for their own gain and left afterwards. He got to live his life his own way through his family and friends.

This really shows that Ernest made the right decision to refuse the potion and refuse these women, he finally realized just how stupid and unrealistic living forever is. And most important of all, Ernest lived his life selflessly and left behind a legacy of love and kindness for the people he cared for. He enhanced the lives of others and left behind so many good memories with his family, friends, and grandchildren, even lived his life like it'll be the last. This is how you live life to the fullest, especially of living your life in honesty, confidence, compassion and balance. Really shows the big difference between the lives of earnest and these two women Madeline and Helen. Ernest lived his life in selflessness, while Madeline and Helen live their lives in selfishness. Ernest lived his life in the middle ground while the women lived their lives in extremes. See the difference between the two, especially of how these decisions will affect your life even after you die? To put it bluntly, Ernest lived such a good life and I bet you he'll even be given a second chance in the afterlife while Madeline and Helen will live their Eternal lives in a never ending hell.

It was also Helen's fault too. She drove Ernest away from her by having a fixation in wanting to destroy her frenemy, Madeline, by any means necessary and she ended up alone. He couldn't put up with it and left Helen. Instead of trying to get Ernest back, Helen could've ignored his marriage to Madeline and focused on making her life better. If she did that, Helen would've had the last laugh on both Ernest and Madeline. She would've not only been a successful writer, but happily married to a man who truly loves her and is vibrant in her old age. When they see Helen again in 1992, she is on her new successful novel called Forever Young, a novel about two people living the best life of their lives. Ernest lost his job as a plastic surgeon, aged terribly from the years of drinking and is now working as a reconstructive mortician. Madeline has aged worse as her plastic surgeries in her attempt to stay young had made her look ridiculous, her acting career was on the decline and is much worse off. When Ernest asked how Helen remained beautiful and vibrant after all those years, she gives him the novel for him to read so he could see what he missed. He notices a man showing his affections to her. Helen introduces Ernest to her loving husband and soon remorse sets in. In his and Madeline's quest to stay young and on top of their careers, they lost more than what they've gained. Ernest saw too late how toxic she was to him and Helen found someone much better for her.

He added value to other people's lives through all his later work as well... not just shallow plastic surgery but real help. In the original ending 27 years later Ernest is still alive with his wife in Switzerland and they grew old but are extremely content sitting on a picnic blanket having food. Madeline and Helen are there too (still looking stunning, but bored) sitting on a terrace staring at them... Madeline ridicules them for being so old but not Helen... she looks at them with longing and she realises what a mistake she made. Ernest still has the young hand and covers his wife's older hand as they sit in the park. The eternal ladies keep staring at them (figuring out it's Ernest and his wife perhaps?) as the movie fades out to black there was a quick flash of skulls superimposed on Mad and Hel...

I've been going through thousands of commercials from NBC, Scifi Channel etc to see if there maybe was a preview for the broadcast of the Original Ending but didn't find it. Maybe one day it will pop up (along with all the other deleted scenes)... but they just seem very unwilling.

Destroy each other in the end. Lisle did warn Helen and Madeline earlier to take care of their bodies after drinking the potion and they didn't listen. On top of it, they also violated their pact with her in never telling anyone about the secrets of her potion. Lisle was well aware that her potion had flaws, but Helen and Madeline's betrayal was way worse. They told Ernest about it and brought him to her in the hope of making him drink the potion and do maintenance work on them.

It looks like some sort of poem about the passing of time and value of aging/death. It is funny to them because they feel he was a sucker for passing on eternal life and youth. He got older and wiser while they never changed both physically and mentally. They are forever stuck in that phase. They will never learn their lesson or come to a real understanding of what they lost. They are still Mel and Hel bitter frenemies. It is in fact a hell of their own making.

No. The potion forever bounded their souls to their bodies. Lisle only warned Helen and Madeline after taking the potion because she had seen their behavior resembling those of her previous clients in taking it. However, what her clients did successfully is that they took care of themselves after the potion and kept the secrets to themselves. Helen and Madeline never did and their loose lips in telling Ernest about it, got them in trouble with Lisle. Because they violated their pact, she banished the two women and they were forced to do maintenance work on each other. The only way Helen and Madeline could get death is if they went back to Lisle to get another potion to kill them. Knowing her, she'll probably keep them imprisoned in her home for all time to keep the two from telling more of her secrets to immortality.
