Thank you very much. I had an interview for oncology job where I was asked how to manage a suspected neutropenic sepsis. I mentioned antibiotics while awaiting culture results and the lead consultant said it's not the way they practice. Can you throw more light on how to approach such situations?

Neutropenic sepsis with patient undergoing chemotherapy, is a medical emergency. If the patient is critically unwell, pre-alert and arrange emergency transfer to hospital, f the person is clinically stable, arrange emergency transfer to the local oncology or haematology unit, or medical assessment unit, using clinical judgement depending on local referral pathways. If the patient is in the ward/hospital setting 1: Give oxygen, titrate accordingly, 2: Take blood tests FIRST and microbiology samples, 3: IV intravenous broad-spectrum antibiotic at the maximum recommended dose. 4: Give IV fluid bolus to restore tissue perfusion, 5: Check serial lactate measurement, 6: Check urine output, monitor fluid balance hourly and monitor the person's clinical condition. This is step wise approach, if you don't follow accordingly this is patient safety concern, hence consultant said so. Hope it helps!
