This is really useful. I have multilevel (C4 to C7) disc herniation (from a severe whiplash injury a few years ago) and degeneration with mild signal changes in the cord at C4/5. I also have Hypermobile EDS. I have just lost the use of my left hand and so I'm now waiting for a referral to the spinal team (I'm in the UK so the wait may be a long one!) My concern was the impact of a fusion on adjacent vertebra as this is a recognised issue for fusion surgery in EDS - and a problem I've had with other fusion surgery (not spine). It looks like disc replacement and debridement(?) is the best option. Restoring hand function is a work in progress! 😅

The Germans left ball and socket disc replacements more than a decade ago, viscoelastic discs such as cp-esp and lp-esp are far superior to anything in the US, and they are doing many 3 and 4 levels successfully!

I am fused from either T1 / T2 to L3/L4 is it a possibility that is why i now have a herniation around c5 c6 or could it just be fron pressure transfering to the above unfused segment from lifting items that might be too heavy for the muscles in mt back shoulder and neck becsuee it came on 1 week after i lifted a very heavy child and then i had ear pains from it but relaised i was getting ear pains earlier in the year when i probably lifted things doing house decluttering and house repairs and perhaps it was progressing and got worse with lifting the child?

Because you mentioned that you've been told that your spinal discs are not causing your pain, we wanted to let you know about a new procedure being done by Dr. Mobley in our Lone Tree and Castle Rock offices: www.neurosurgeryone.com/treatment/intracept-basivertebral-nerve-ablation/ This condition is often overlooked. If you are in Colorado, Dr. Mobley can review your MRI and have a phone or telehealth consultation to tell you if this condition is affecting you and whether you are a candidate for this procedure. He also will soon be offering another minimally invasive procedure for back pain caused by muscle failure.

I'm in kinda the same boat -- I've seen 3 neurosurgeons now and they all say that I need from 2-4 laminectomies in lumbar and either 3 ACDF or 3 disk replacements in cervical spine. I'm not excited to jump into something so invasive. I just found Sonospine in Phoenix, AZ and I'll have them review my case. They look/sound very promising and their process makes a lot of sense to me, but IDK yet if I'm a candidate. They sure have a lot of positive reviews from patients!. I hope that you find success 👍

I’m seeking medical opinions for disk replacement vs fusion and was told by a neurosurgeon that because my spinal cord is being compressed and showing sign of damage I couldn’t do a artificial disk replacement. Fusion is the only way to go. But a orthopedic surgeon said I he could perform disk replacement. I would much rather as I’m only 37. Which is the best way to go?

Another surgeon suggest three disc displacements., Though he said he may have to fuse one when he got in there due to the fact there was a cyst on a vertebrae. The opinions were within the last month within the same MRI or MRIs. I’m extremely active 57 year-old weight training, w nine-year-old boy. far from normal in that department. I also have stenosis in my lower back that doctors have discussed operating since my 20s. I’ve learned to live with that kind of at a certain level cervical pain really went south two months ago. I bring him up bc you know the richest man on the planet has the BEST , team , surgeons , advisors and they told him ARD- but my point is - he still has some pain but he has full range of motion practically. I’d do the disk replacement . Maybe give it a few months to see how it is but I’d go for that if they actually think you are a candidate for the ADR. Best of luck. Unfortunately, Kaelin, the FDA has approved ADR for a maximum of two levels so you would not be a candidate. However, there are other surgical procedures that are very effective for patients who do not qualify for ADR. If you live in Colorado, I'd be happy to have a consultation with you to explore options.

@@neurosurgeryone Since when has the FDA done us any favors? I of course don't know why the FDA has only 'approved' up to two levels, but it seems that the FDA operates on bribes & corruption (same as WHO, CDC, NIAID, et. al.) so I wouldn't put much weight in their opinion. I met with neurosurgeon in Reno yesterday who offered me a 3 level cervical disk replacement with the understanding that it's not FDA approved. I asked him if Medicare will cover it and he said that they have covered all the other 3&4 level replacements that he has done. Interesting that one government agency doesn't approve of what another agency covers :-)

Which is best ACDf or ADR for long term

  • @neurosurgeryone

Hi Raja. The answer is not really black and white. ADR has shown slightly decreased rates of reoperation in the long term, compared to ACDF, and there are some recent studies that have shown slightly decreased pain long term. But the caveat is that in patients who have facet arthropathy or spondylolistesis, ACDF is superior. Plus ADR is only FDA approved for 2 levels. I hope that helps.
