Power Supply should work on 5V and it should provide 3Amp. And check the Fan with different power source. Probably your fan is damaged

Short circuit and virus problem there are not
12V Fan on a 5V output is not ideal. It will stress the Pi out. There is a lot of 5V fans, different sizes in Aliexpress.
