@BunnyBen87 I took a trip down to Yharnam, needed a cure for my hiccups. Met this real nice guy in a hat, said he could fix me up real good. Then, I'm strapped to a table and forced to sign a contract, and as he's giving me the transfusion, a werewolf comes out of the ground, bursts into flames when it tries to touch me, and then I get dragged down by these weird fetus looking guys.

I do hope when Bloodborne II comes out that we do get to play a fully matured Great One! From Software might decide that when this Great One fully mature he or she might look completely human yet bearing an assortment of supernatural abilities!

I like to imagine that the hunter is so powerful that comunicates without the needing of a mouth or smt and he told the doll he was cold using his new omniscent powers, causing the doll to say "oh, good hunter"

If you think about it, becoming a creepy slug god is the good ending. You free Gehrman from his service to the Moon Presence and manage to achieve what noone else in the game - Byrgenwerth, Healing Church, Mensis or the Vilebloods - manage by ascending to a Great One.

Jake Matrix nah that guy was alfred. He gives you Logarius' Wheel. and is for the Executioners Covenant. You need to talk to the queen in Cainhurst to the Chikage.

What happens after this cutscene? I just finished bloodborne and head to ng+ without seeing this.
