I did a character like this in a campaign. It was this unknowable arcane sigil that followed her around. It used to be an archangel but it fell. I was playing a little elf girl who had not an ounce of sense to her. She trusted everyone implicitly, including this fallen archangelic sigil and so much of the time her patron was just effectively getting her out of trouble that she put herself in, because it knew that if she died so did it. It was trying to teach her how to survive, and any advice that revolved around abandoning faith in any individual she would brush off, like "no, no they're just playing." Or "no silly, people are inherently good, we can't just leave them, they'll get hurt." When in truth they were like, brigands or cultists. Two of the main dmnpcs that started following us around we're actually just randomly generated enemies at one point who didn't want to let her down.

Old Man Henderson works on a similar concept. He’s already, diagnosably, insane; so he dismisses eldritch things as either “the ugliest damn poodle I’ve ever seen”, or as hallucinations, because he knows he gets them sometimes. Also he uses a necronomicon page to roll a jumbo-sized blunt at one point. Irrelevant, but I’ve always loved it.

In Germany, there's a P&P game called "Plüsch, Power und Plunder" (something like Plush, Power, and Trinkets). You play as stuffed animals which have the task to protect the children and each stuffed animal has unique powers. One of them is the ability to fly which is possible because the stuffed animal is too dumb to know that it can't fly.

  • @GuybrushTThreepwood

I just love how thematic it is that insight, mechanically, does absolutely nothing. It's literally just for lore and completion purposes, but in regular gameplay it's nothing but a liability. Have fun facing the cosmic horror!"

Oddly enough there's another way Bloodborne shows how this kind of stuff could be avoided with the Skeptic Man NPC. He's the only NPC with a questline that never ends with him going insane and it's not because he's dumb - he's actually one of the most lucid and intelligent NPCs in the game. Rather it's because every time you interact with him he calls you a liar while claiming HE knows the REAL truth, any advice you give him he'll do the opposite, and any time he comments on other NPCs and what he thinks of them his interpretation is always the exact opposite to what they're actually like. He literally avoids insanity by stubbornly just living by a "I reject your reality and substitute my own" mentality and interpreting everything he sees as some kind of trick that he's "too smart to fall for."

This was a rule in 6th edition of Cthulhu. If I remember correctly, if you were going to gain an insanity you rolled an INTx5 check, and if you *failed* you didn't go mad because you didn't comprehend what you saw. I scrapped it because I ran for a lot of min-maxing fiends who would absolutely make the stupidest possible characters. Fun fact: you rolled Size and Int in the same pool of results, so people can be big or smart. So I was faced with a lot of 8 foot barbarian professors and detectives who could barely tie their own shoes until I instigated that rule.

Considering what Lovecraft thought of anyone who wasn't an old money, formally educated, urban Massachusetts resident, you're being waaaaaaaay too generous with your credit. People give Lovecraft shit for his racism (even by the standards of the time), but while he was certainly that, it was just the tip of the crazy iceberg. Dude was seriously mentally damaged. Take a look at what he thought about anybody from a small town before you give him credit for overthrowing classism.

You roll an Intelligence check and if you FAIL, then you do not comprehend what you are seeing and do not slip into temporary insanity.
