AIIMS chief explains what causes Black Fungus and how you can prevent it

Diabetics suffering from COVID-19 who are being given steroids have a high chance of being affected by mucormycosis or "black fungus", All India Institute of Medical Sciences (AIIMS) Director Randeep Guleria said on Saturday.

    He's saying that diabetic patients and people who have taken high dose of steroids are at High risk, control your sugar , wear masks as its spores are present in soil and air, avoid construction sites, dirt, mud, dirty water, maintain proper hygiene. Symptoms may include pain in eyes, swelling at one side of face , skin discoloration , runny nose , bleeding from nose , chest pain, blood in cough.

    This fungus was literally not much known during the first wave. It is not much reported in other parts of the world. So what has changed in India and that too during the second wave? One thing stands out. Industrial oxygen. Industrial oxygen and medical oxygen are two different entities. Medical oxygen has to pass high stringent tests of purity. Due to extreme shortage of oxygen and numerous deaths due to it India had to hurriedly move oxygen from industrial sources to hospitals . Authorities need to investigate whether the contaminated oxygen from industrial sources is causing these infections.

    @kaustubh saxena Bro I am not spreading rumours. I have expressed a possibility and said it needs to be investigated. How can you rule it out without any investigation? The containers holding industrial oxygen can be really contaminated and the whole process of converting and transporting needs to be evaluated carefully. After all it's a question of life and death for patients.

    Covid associated mucormycosis. This is a fungal infection. The spores in this fungal infection can be found in soil, in air and to an extent in food also. But this fungus is often low virulence. It doesn’t cause infection. And if we talk about the days before Covid, these fungal infections were present but the incidents were very less. And it was a rare infection. It was seen in people who had high sugar and diabetics was uncontrollable, or in those who had a transplant and were on Immunosuppressant drugs which has made their immunity low or even in cancer patients who are on chemotherapy. In normal people these infections were not usually found. So this is not a new disease. But due to covid and the treatments given for covid, the cases are found to be increasing. In many areas there are reports of high number of cases. If we talk about AIIMS, here we have 23 cases admitted with this fungal infection related to covid. 20 out of them are still covid positive and three are negative. And there are few countries in which they have reported about more than 400-500 such cases. This disease either effects the face, and we call it Rhino-orbital-cerebral-mucormycosis. This effects the eyes, the orbit of the nose and the infection might also spread to the brain. This will lead to the person losing his vision, go blind. And it can also effect the brain. It can also effect the lungs. And it is called Pulmonary mucormycosis. Both have many symptoms. Like in Rhino-orbital-cerebral-mucormycosis who can have headache. Severe pain in the head. Nose can be blocked. There will be secretions from the nose that is green in colour. Or even blood. You can also lose sensation on your face. There will be swelling on the side of your face. There will be discoloration on your skin. And if it is in the lungs, you can have fever, pain in the chest. You will have cough. And when you cough there might be blood in it. But the most important issue is, why is this fungal infection more prevalent these days. What are the reasons for it and what are the preventive measures to be taken. One thing is that due to covid, it is a risk factor. In covid infection when we look at the blood counts, one cell line which is called lymphocytes decreases and it becomes lymphopenia. And due to this the CD4 and CD 8 cells in the body decreases, and due to this the chances for opportunistic infections or fungal infections increases. This was seen during the first wave. And if we talk about 2003 SARS disease, if we look at the data there were a lot of reporting on the fungal infection. And the reason why we see a spike this time is because of two reasons. One is because there are lot of patients who are diabetic issues and if this is not under control there are higher risks of fungal infections in such people. And the third and most important reason is the misuse of steroids. The patients who are taking steroids, have diabetics and have covid. If these three come together, then there are increased chances for fungal infection. So to prevent this: first, we have to reduce the misuse of steroids. And we have mentioned this before, patients with mild disease and who doesn’t need oxygen, and those who are in the early stage of covid, giving steroids to such patients can cause more harm and there will be absolutely no benefit. Even in the recovery trial it showed that if we give steroids to patients with mild symptoms. Then morbidity and mortality due to secondary infection is more. So, if anyone has covid and it is mild and if they only have fever and saturation is maintained. Who have cough but there are no other symptoms of low oxygen level, then steroids should not be given because in the future there are chances for fungal infections especially if that person has diabetics. Second thing is diabetics or sugar control. This is very important. This fungal infection is seen more in those whose sugar level is not under control. So it is very important that those who are diabetic or those who are taking steroids they have to check their sugar. In many cases people don’t know if they have sugar problems. And as soon as you take steroids the sugar level immediately spikes to 300 and 400. So please monitor your sugar. If you are diabetic and you have covid infection and you are on steroids than it is very important to monitor your sugar daily. Third thing is to do keep wearing mask. As I said earlier these are spores, these fungus can be found in air too. Wearing a mask is very important especially if you are going to some construction site where there are more chances of finding this fungus. So it is very important that if we are taking steroids, please be careful not to misuse it. Second thing is the dosage of steroids, there is no use in giving high dose of steroids. Data shows that mild to moderate dose of steroids are good enough. If we give very high dosage of steroids, sugar levels can go high and there will be higher chances for infections. Thirdly, the data also shows that steroids should only be taken between 5-10 days maximum in majority of covid patients. Giving steroids for longer days, makes the control of sugar more difficult and there are higher chances for fungal infection. So I would like to say that lot of these cases are being reported now. And we must work aggressively to bring down the number of cases and it is also very important that it should be suspected earlier so that in the future there will be no loss of vision and infections in brain. So whenever a patient is discharged and if they have any symptoms that I have mentioned earlier, immediately contact your doctor, keep your sugar under control and if you are taking steroids unnecessarily please stop taking it.

    proper hygiene and air purification system is important to capture the fugal spores that's in the air... Most hospitals are not making any investments on air purifiers. Covid wards must have air purifiers of HEPA 13 grade and proper hygiene.

    We didn't heard of black fungus in late April to early May. It's only from mid May that we all are hearing about Black fungus. If we were told about it from the beginning, all the covid patients would have easily prevented this fungus. Many patients have missed their initial Black fungus symptoms.

    Why don't we see mucuromycosis in first wave of covid , still the treatment is same either its first or second wave What changed is the use of industrial grade oxygen rather than medical grade It's also seen in those patients in which steroids are not used

    I fall in Same case. I had mild cough, corona Positive and doctor went for CT scan, result was corads 6. Doctor wants me admit, I agreed. Then he gave 4 remedisivir. First two does in first day. Followed by one by one next two day. I complained him about chest, back pain after drips. But he said yes that would happens. I was in pain for next two day after every drip. This was a particular drip called Lionozol. It was painful. I think I didn't needed those remedesivir. Doctor want me take total of six doses of remedesivir but I insisted no more. Then I left hospital after four days treatment. But now I am feeling more weaker than before I got admitted to hospital. I don't have any serious problem right now except little breath out, feels like I am about to cough when I start talk. And little heavy chest, if I sleep on back. So I started sleeping on the shoulders side. Peace to lost souls. Everyone stay safe and take care for your loved ones. Thanks.

    Don’t keep the title in English if it is not in English. Union Gov get GST from all linguistic states and let the union health department deliver an important message in only one language with no English subtitle too. I agree that this reaches more people but need an English version too or at least English subtitle . When south India needed Aiims hospital the union gov says it should be built by borrowing money from Japan and does not fund a union government vaccine production facility near Chennai . Does not make sense. Ok just noticed that an English version is there in Ndtv .

    A few vital points remained unanswered: role of oxygen cylinders in spread of infection; whether risk continues even after control on blood sugar and discontinuance of steroids, if yes, for how long. Also, since most patients would ba taking steroids on doctor's advice, sone even without knowing they are on steroids, it is doctors who need to be advised and cautioned on prescribing steroids.

    Risk is reduced if sugar is in control 👍 As far as duration is concerned it is subjective & depends on patient age, co-morbidities, dose & duration of steroids use. Still to satisfy you with a number 2-4 weeks(WBC count normalises).✌️

    Steroids are also given before covid and it does not did as much.as of now...... But i think more research to be needed on this rather than creating panick. ESpecially symptoms... Toothache is so common, nasal/ sinus congestion is very common in sinusitis patients which is very common disease and also headache. Some doctor says diagnose it early before it reaches to eyes. Can they tell us how if the symptoms are so common...also not all patients develop black patches untill the fungus spread.

    @@sudeep4489 People started being relaxed in November just because govt didn't warn them about second wave instead govt started doing elections in states and people become more careless. Govt had not invested anything in vaccine research in india but media were showing "bharat bnega vaccine guru". they didn't even order vaccines from foreign comoanies. Now they tell stated to buy their own vaccines but companies abroad refused states to give vaccines as they will only deal with union govt.

    We talk very advanced things about Covid resistance but become so lazy to do simple home remedies which we follow since 2019 with good results. Very simple as such, daily gargling thrice with moderately hot salt water, drinking boiled water made of ginger,turmeric ,black peper and krishna tulsi leaves, twice a day,viz; in empty stomach in the morning and 6PM in the evening. Also drinking black coffee boiled with dry ginger, a pinch of black peper and cinnamon powder. This will enable to boost the immunity power.

    Covid associated mucormycosis. This is a fungal infection. The spores in this fungal infection can be found in soil, in air and to an extent in food also. But this fungus is often low virulence. It doesn’t cause infection. And if we talk about the days before Covid, these fungal infections were present but the incidents were very less. And it was a rare infection. It was seen in people who had high sugar and diabetics was uncontrollable, or in those who had a transplant and were on Immunosuppressant drugs which has made their immunity low or even in cancer patients who are on chemotherapy. In normal people these infections were not usually found. So this is not a new disease. But due to covid and the treatments given for covid, the cases are found to be increasing. In many areas there are reports of high number of cases. If we talk about AIIMS, here we have 23 cases admitted with this fungal infection related to covid. 20 out of them are still covid positive and three are negative. And there are few countries in which they have reported about more than 400-500 such cases. This disease either effects the face, and we call it Rhino-orbital-cerebral-mucormycosis. This effects the eyes, the orbit of the nose and the infection might also spread to the brain. This will lead to the person losing his vision, go blind. And it can also effect the brain. It can also effect the lungs. And it is called Pulmonary mucormycosis. Both have many symptoms. Like in Rhino-orbital-cerebral-mucormycosis who can have headache. Severe pain in the head. Nose can be blocked. There will be secretions from the nose that is green in colour. Or even blood. You can also lose sensation on your face. There will be swelling on the side of your face. There will be discoloration on your skin. And if it is in the lungs, you can have fever, pain in the chest. You will have cough. And when you cough there might be blood in it. But the most important issue is, why is this fungal infection more prevalent these days. What are the reasons for it and what are the preventive measures to be taken. One thing is that due to covid, it is a risk factor. In covid infection when we look at the blood counts, one cell line which is called lymphocytes decreases and it becomes lymphopenia. And due to this the CD4 and CD 8 cells in the body decreases, and due to this the chances for opportunistic infections or fungal infections increases. This was seen during the first wave. And if we talk about 2003 SARS disease, if we look at the data there were a lot of reporting on the fungal infection. And the reason why we see a spike this time is because of two reasons. One is because there are lot of patients who are diabetic issues and if this is not under control there are higher risks of fungal infections in such people. And the third and most important reason is the misuse of steroids. The patients who are taking steroids, have diabetics and have covid. If these three come together, then there are increased chances for fungal infection. So to prevent this: first, we have to reduce the misuse of steroids. And we have mentioned this before, patients with mild disease and who doesn’t need oxygen, and those who are in the early stage of covid, giving steroids to such patients can cause more harm and there will be absolutely no benefit. Even in the recovery trial it showed that if we give steroids to patients with mild symptoms. Then morbidity and mortality due to secondary infection is more. So, if anyone has covid and it is mild and if they only have fever and saturation is maintained. Who have cough but there are no other symptoms of low oxygen level, then steroids should not be given because in the future there are chances for fungal infections especially if that person has diabetics. Second thing is diabetics or sugar control. This is very important. This fungal infection is seen more in those whose sugar level is not under control. So it is very important that those who are diabetic or those who are taking steroids they have to check their sugar. In many cases people don’t know if they have sugar problems. And as soon as you take steroids the sugar level immediately spikes to 300 and 400. So please monitor your sugar. If you are diabetic and you have covid infection and you are on steroids than it is very important to monitor your sugar daily. Third thing is to do keep wearing mask. As I said earlier these are spores, these fungus can be found in air too. Wearing a mask is very important especially if you are going to some construction site where there are more chances of finding this fungus. So it is very important that if we are taking steroids, please be careful not to misuse it. Second thing is the dosage of steroids, there is no use in giving high dose of steroids. Data shows that mild to moderate dose of steroids are good enough. If we give very high dosage of steroids, sugar levels can go high and there will be higher chances for infections. Thirdly, the data also shows that steroids should only be taken between 5-10 days maximum in majority of covid patients. Giving steroids for longer days, makes the control of sugar more difficult and there are higher chances for fungal infection. So I would like to say that lot of these cases are being reported now. And we must work aggressively to bring down the number of cases and it is also very important that it should be suspected earlier so that in the future there will be no loss of vision and infections in brain. So whenever a patient is discharged and if they have any symptoms that I have mentioned earlier, immediately contact your doctor, keep your sugar under control and if you are taking steroids unnecessarily please stop taking it.

    It spreads if you directly touch an infected person & articles used by him. You won't get it even you stand near him without touching anything. Basically it's contagious but not communicable👍 Still you get infection you won't develop disease bcoz for that you must be immunocompromised/uncontrolled diabetic 👍 I'm sure we all have inhaled millions of spores but haven't develop disease as we aren't immunocompromised 😀
