    Congratulations for your videos, I love the way you explain it!! I just want to say that according by UpToDate Friedman curve and Zhang/contemporany curve are still in debate in whcih one we can use but my percepction and my examns are based on Zhangs

    Hi Paul! Thanks so much for all the videos. I have used them over and over again as a med student and now, in my 1st year of residency, I go back once in a while for topics like obstetrics which I don't know much about (because it's not my field but I need to do mandatory rotations in once in a while).     Hey Paul! Thanks for the great video. Would you mind answering one question. In case of prolonged active phase of labour, you mentioned that we could consider morphine sedation to slow this down. However, given this is the active phase, wouldn't that increase the chance of baby coming out flat and needing resuscitation? Wouldn't intrapartum foetal monitoring and caesar if indicated be a better approach? Looking forward to the reply. Thanks. :)

    Yeah it's really risks vs. benefits. That is definitely a legitimate concern. Morphine is merely an option to slow labor down if it's desired, but there are certainly circumstances where it may not be desired. Whether or not you would use it will depend on the exact situation. Morphine is described here (you can also use terbutaline) as a means for remedying hypertonic contractions, which has a host of risks for mom and baby (e.g., placental abruption, uterine rupture) but there are legitimate risks for the baby as you mentioned. The goal with medical intervention is to get the woman into having normal contractions and facilitate a natural vaginal delivery. If you're really having a rough go of things, then a Caesarean would be a viable option -- but that too comes with obvious risks and isn't always doable (e.g., no OR or qualified surgeon available). So the morphine is really just an option. I don't mean that this would be appropriate in all circumstances, by any means.

    Why are you default giving her morphine if she’s having a prolonged latent phase/possible false labor? Many women decline pain medication even during the active stage. Is this 1965 and we’re still giving women pregnancy tranquilizers? It makes sense if she requests pain management, but to default to something like that doesn’t make sense to me. Am I missing something?

    arrested is more severe form of prolonged active phase. u can think about it this way. the changes are way slower to the point it changes every 3 h
