Abdominal Examination Explained - Clinical Skills Deep Dive - Medical School Revision - Dr Gill

This Abdominal Examination Explained Deep Dive looks at the details of the examination of a patient with abdominal issues, highlighting what is being checked for during special tests and examination findings
In this video, we demonstrate how to perform a clinical examination of the ABDOMINAL SYSTEM for your medical school Clinical Skills OSCE. As the gastrointestinal exam is sure a core skill when it comes to examining patients, students should assume that an abdominal assessment is a high yield station for any clinical exams or clinical assessments.
For a passing grade in your Clinical Skills OSCE, for Abdominal exam follow the approach of:
- Inspection
- Palpation
- Percussion
- Auscultation
We will also address why in the UK the auscultation is placed higher on the examination approach
HOWEVER, an abdominal examination OSCE station does not just involve pressing around the stomach, this video also demonstrates some of the specialised examination techniques required in examining patients such as shifting dullness for ascites, and checking the size of the liver which may be seen to be enlarged in cirrhosis as an example.
Diarrhoea, pain and general concerns about the abdomen are a common reason for patients to see a doctor, and in any speciality is probably the most commonly performed patient examination

Please note that there is no ABSOLUTE way to perform a clinical examination. Different institutions and even clinicians will have differing degrees of variations - the aim is the effectively identify medically relevant signs.

    As an aspiring physician and frequent volunteer patient (for med students), I've seen several physical exams, and only by watching this was I truly able to understand the purpose of every step.     Honestly no idea what causes it. The audio seems to affect a small number of people - maybe about 10%, we’ve not been able to find why the audio is low for some people, but not for the majority

    It’s a genetic - a mutation in the ATP7B gene 🧬 Recessive, so both parents would need to be carried at least for a child to present with the condition

    @@DrJamesGill I like the multiple camera angles, but the fact I can see you turn your head to a new camera and then it cuts to it is a bit distracting and unnatural. I'd prefer a direct cut to a new view instead I think. Otherwise amazing video!

    Another great video and a belated Happy New Year Dr. Gil! Out of curiosity for future videos, have you ever considered doing one on ASMR itself or is that really more of a psychological topic... I ask because, and not to be gross, a few years ago I had to have an ear cleaning done and I noticed afterwards that ASMR wasn't as affective as it used to be. Meanwhile my wife has sparkling clean ears, and says ASMR doesn't affect her at all but listening to glass shattering makes her sleepy. Just a thought, medical peculiarities have always been interesting to me, like being part of the 1% of people who think cilantro tastes/smells like soap. Have a good day!

    Hospitals need staff. We don’t have enough staff. I worked in ITU about 10 YEARS ago now. So I can stop being a GP and restart in an ITU role. Could they redeploy people - yes they are. But it needs specialist doctors to RUN the ITU dept. Consultants there normally work in pairs, 24hrs on and 24 off. You need X number of consultants for X number of patients. Oh those doctors also are getting Sick with covid. And need down time You also have to keep the rest of the hospital running. The NHS has said we run at 99% for years. We’ve used to spare 1% now... and there is no space in the system

    @@DrJamesGill are there qualified medics in the forces who could help? Probably not qualified in ITU. Just shows the need for more people training in the field. ❤️

    Those medics normally have NHS positions when not deployed. We have Lieutenant Colonel at Warwick uni, who works in ITU in Birmingham when not in uniform. So not much spare there either

    @@DrJamesGill just horrid. I'm lucky enough to live in Cornwall but the situation even here is pretty bad now. We only have one fairly small hospital in the whole county. Scary times. Off to work at school every day looking after key worker kids,no masks in the classroom. feeling it's just a matter of time before I get it now. 🤦‍♀️

    I work as a Pharmacy dispenser (when I'm not at home shielding) and metformin is one of the most prescribed medicines we dispense so that Actavis image made me smile. Does anyone else have Teva Divas? Adamimulab (Amgevita/Humira) has always been best in my Crohn's treatment though.

    Obviously Omeprazole and Lansopraole can be important in protection against gastro-damaging medicines i.e. Naproxen.

    Dr. Gill thank you for this video! Thank you for explaining why you choose to do auscultation last. In terms of a distended aorta or even a bowel obstruction would you think that doing palpation first especially deep palpation could cause a perforation? I totally understand where you are coming from in terms of looking for certain symptoms that could cloud clinical judgement but I think that hearing a aortic bruit or absence of bowel sounds in my case I detected a bowel obstruction just by auscultation because none of the physicians have auscultated the abdomen. For a couple of weeks this patient was given milk of magnesia thinking that the abdominal discomfort and distension was due to constipation (as this person was elderly and confined in bed) it turned out to be a large cancer obstructing the whole bowel and I was able to detect something unusual just by doing auscultation. In fact in this case I would argue that palpation especially deep palpation would have been contraindicated. Anyways just sharing my experience and as always very appreciative of your expertise!     So weird the last few months it's felt like I have a tennis ball in the right side of my abdomen. Not really painful. Just feel a pressure and almost a crowded feeling. Done MRI, CT scan, xrays and even a colonoscopy. They found tiny cysts on my liver and kidney (My doctor isn't concerned about them) but no explanation why I have this feeling. Ugh.

    While palpating liver should we lift our hand while taking deep breath in or out?or it's better to go through without lifting our hand ?? Is there is any specific hard or fast rules? [Also in spleen examination?]

    Lift off as the patient breathes in. You need to lift off in order to effectively move up the abdo. Same with the spleen

    Hi Dr Gill! I tuned into this video since I frequently suffer from lower abdominal and back pain, and one part of your video has alarmed me slightly. At 4:33 it appears I have this loss of the nail fold angle; they touch 'back to back'. I'm almost certain I don't have IBS and I know for sure that I don't have coeliac disease, as I recently had a blood test, but I'm not quite sure about the others. Would you recommend seeing a GP? I am about to turn 16 if this plays a factor in it. Thanks in advance! Edit: I just had a look at some google images for nail clubbing, and they all seem to have an overgrowth of skin or tissue of some sort (correct me if I'm wrong), but I don't have that. Could it possibly be due to my strange anatomy? 😅
