    I found out my Ana was positive. I went to the ER this month for pneumonia. This came out of the blue. I went to my primary doctor, and I requested to be tested for RA. In my younger years, I was diagnosed with burtitis in my shoulders. Now that I am in my glorious 50's. My hands would flare up.. I couldn't open up my hands. My hands will stay red in the palms also. I did my own research in order to ask to be a test for RA. I do have a rheumatologist appointment this month as well. I am speaking for myself. I personally want to know if I have an autoimmune disease. I am not scarce of the diagnosis. I have learned to deal with life on life terms. I am a caregiver for my father , who has sarcoidosis since 2017. I advocate for his care at the VA, and I will definitely advocate for my own health. ** Sending positivity and love from Texas to everyone.

    Hello my mom used to experienced the same where her hands would flare u. Her palm turns red and becomes hot and fingers can't grip things properly. She has SLE.

    @@misisnijimin same here! My palms also swell around the base of my thumbs and I am losing my fine motor skills. I just had positive ANA but negative ENA test for Lupus. I have heard you can still have Lupus with negative anti DS NA (may have the letters wrong). I am waiting on my Myositis panel to come back and other tests. I have had kidney issues, fatigue, major weight loss, vascular issues, numbness, rash (not butterfly rash) but small red spots all over chest and neck after hot showers and in the sun, light sensitivity, I could go on forever. Do you know if your mom had any of these symptoms?

    @@nopressurenodiamonds5566 mom has severe hair fall during flare ups. She used to have terrible and prolonged headaches, tiredness, arthritis and so on and so forth. Btw, mom's 2 other siblings have auto immune disease. Her brother had kidney transplant 15 or more yrs ago while her sister started to have severe arthritis. Common symptom they have is arthritis. Just to share I think my mom by far is one of the longest living SLE patients in Philippines, she is now 70 yrs old. She had ITP when she was 10 then progressed to SLE when she was 15. Her kidneys are still thriving even if 50 yrs of her life she was taking steroids. I can go on and on about her medical history. I hope this somehow helps someone.

    I had actually asked my rheumatologist to get another ANA just out of my own curiosity to see if the numbers went up. And she said the same thing you said. It’s not needed. “Once it’s positive it’s positive”

    😮 wish my rheumatologist spent more than 5 minutes with me or explain like this. I have MCTD, sjdrogrens, sle lupus, had endometriosis, had hysterectomy, has hashimoto had a thyroidectomy. That's at the top of my head. We retired near the beach and it takes months to 18 months for specialists..covid was bad in NYC and moved to the Jersey shore. Not enough Dr's. Sad many don't except patients. Thank you 😊 ❤

    My ANA test a few days ago showed over 1:1000, and I do not have lupus or vasculitis. My thyroid is perfect, vitamin D levels are good. I am worried about the test result, I am looking forward to seeing a doctor and hearing her opinion...

    I really like this video, educational and to the point. Reassuring as well. Thank you Doctor!! I’ve read and commented on couple of peoples “worries” ANAs can be so confusing and scary, but it is not set in stone- you can have it negative all across , for years , and still have bunch of medical issues. I would like to remind some of you that many of these symptoms could possibly have something to do with B12 and B9 deficiencies. Although normal range in USA for B12 is 200-1100 pg/ml it is known that about 10% of patients with values between 200 and 400 pg/ml experience neuropsychiatric and hematologic abnormalities, inflammation, itching and skin problems, pain “everywhere” , headache/migraine with light sensitivity, hair loss, bone/joint pain, tired/weak/fatigue, abdominal pains, etc. Please do not assume anything. Have your doctor run B12 or B9 blood test. Do not just start taking B vitamins supplements !! they can be toxic for those who don’t need them. Hope this information helps someone out there before you go too deep into the rabbit hole. Don’t give up! You are the best advocate for your own health. Good luck to you all.

    30 y.o male here. For years, I have been complaining to my doctor about these bizarre symptoms which have now worsen excessively. I have good weeks and horrible ones. Fatigue beyond belief, thinning hair (you can see my scalp and balding does not run in my family). I've been sick 5 times this year and getting random low grade fevers. I have had swollen lymph nodes in my neck and groin. My blood pressure drastically began to spike despite my daily workout routine which I cannot physically do for weeks at a time now. My ANA came back 1:160. My new PCP was concerned because it's less likely in men and said this teiter would be higher in women since they naturally have significantly higher values. Waiting for my rheumatology appt in January. I've distanced myself from Dr. Google so I can stop driving myself nuts. Thanks for your video!

    I was tested +ANA on Friday and I had no idea what it was until I did a google search, it frighten me. I found this video and it was the same information they left on voicemail but I do get scared. There has been diseases in the family but that is due to lifestyle choices, which can be changed.     Maybe it depends on the doctor but the ones I've seen have written me off. My ANA is always very high, speckled & homogeneous pattern along with shoulder joint pain, clavicle pain (swells), chest pain, red knuckles and finger joint pain (so bad even flipping a light switch hurts), extremities turn white from cold, raised rash across nose and cheeks, sores in nose and painless ones in mouth and chronic anemia. But hey, nothing wrong there because some other tests didn't come back positive, so they don't care. They only treat test results, not the patient and their symptoms.

    That is terrible of your doctors. You have a lot going on . I’m surprised they aren’t listening to your symptoms. They are very classic ones and many that seem to be associated with Lupus. Hope you get someone to listen to you.

    My recent Ana test from my rheumatologist came back as 13.1 value.However i was referred to a rheumatologist by my primary care doctor because i had a positive Ana test with my Pcp.

    @@savinkunwar3665 I'm positive with no symptoms of any autoimmune disorder. Scheduling another blood draw to further investigate why it's positive. As she says, you shouldn't sound the alarm right away.

    Hello! I'm excited this has started a conversation! I want to reiterate that deciding if an ANA result is HIGH or LOW, doesn't have anything to do with deciding the ANA result is IMPORTANT. Whether or not the ANA means there is autoimmune disease is completely dependent on 1. your symptoms 2. your other labs (liver, kidney, anemia, etc). Regardless, getting a positive ANA test result can be scary and I applaud you for doing your own research!

    You said that a percentage of the population is healthy with ana. What titres are usally false positive. i:80? i:640? Also, ive had a positive ana i:640. I do have a family history (my aunt and cousin) who do have lupus. I just had bloodwork and my kidney functions are all great. When I was in my early twenties 8 years ago, I had hair loss. About 3 years ago, i went to egypt and had these shortness of breath spells on the plane but that was it. I do get some minor pain in my arm and my leg every once in a while, but it goes away quickly. I do hear my knees crack if i bend or something. Ive been checking myself for all other signs of lupus such as the rash, fatigue, mouth ulcers, swelling, etc but nothing. I feel fine. Normal energy. Not tired or extremly sore. Are people with lupus asymptomatic? also, i know with mononucleosis, the ebv virus stays inside of you forever. I had it as a teenager. Does that contribute to a positive ana?

    I've been living for the past 26 years (after seeing numerous medicos) with the assumption that I had Fibromyalgia. Then I saw a new GP who told me after a positive ANA titre of 640 that I had Lupus SLE!!! He sent me to yet another rheumatologist who said..no you definately have Fibromyalgia. After much discussion between them the GP said you have Lupus but you mighthave a little bit of FM!! The rheumatologist then said no you definately have FM but you might have a little bit of Lupus!! Seriously????? Just live your life as well as you can and dont dwell on the negatives. To the doctor in this video thankyou for the information you are the first person I have heard say you can have a positive ANA but not have anything wrong...I wish...but at least you show it can be caused by anything.....

    Hi Dr. Ortiz, I received a positive test for "ANA" and "SCL-70" several weeks ago and the earliest I can see a rheumatologist is in 2 months. I definitely went down the internet rabbit hole (even reading random medical studies) because I was given no information from my PCP about what these test results mean.     Peaches, were you diagnosed with systemic sclerosis? I’m asking because I have not seen the rheumatologist yet but my ANA and SCL-70 were positive also.

    My I had 4 ANA tests 2 positive and 2 negative, extremely frustrating. My DSNA was high through each time. 200 is a normal value and mine was 280. This has been going on for 3 years. Nobody would do anything until I developed skin problems, major eye issues, vessel weakness. My finger nails now get purple in the cold. Im anemic, and my vitamin D levels were almost non exisistant. I finally went to all these other doctors ( eye doctor, vien specialist etc.( with all there collective data being sent to the rheumatoid doctor. She declated I have Lupus. She repeatrd all the labs from a year ago and its I'm off the charts. I'm confused because h I told how tired I was and how painful my joints are. Why not start treatments sooner? I did get a second a opinion 6 months ago, and he said it was all in my head lol. I love to show him my blood work now.

    My ana is positive, I was tested back in 2020 with a homegnous pattern, now I'm positive with homegnous and centromere. I'm also positive with the early sjogren's panel, and have crps..but that has since gone away with a bka amputation. My new reumatologist said he wants to put me in hydroxychloroquine, I also have high blood platelets, and non detectable creatinine/protein in my urine. I'm just baffled, I have neck pain, severe issues in my joints, and struggle to lift my right arm up in the morning??

    What does the ANA 1/80 spkleed test mean, and the TPO test is very high, the normal is less than 5, and my result is 1028, bearing in mind that the TSH test is within the normal limits. Please advise.

    Thanks so much for making this video! I have something called Alpha gal-it's a tick borne disease that cause you to be allergic to all red meat and pork (all mammalian meat). I had blood work done by my dermatologist because my hair is thinning. My ANA came back positive. Not sure if there's any research on whether or not people with this allergy (it's actually a fairly new disease but allergists are thinking it's an autoimmune disease too) and having a positive ANA.

    My ANA is negative but I have everything else pointing towards Lupus. Even the steroid treatment they tried has improved my joint pain. I have been able to walk this week normally. Bone scan is clear. Feeling a bit flustered.

    Hi - hope you are doing well- what were the results of your speckled ANA? A few months ago I was diagnosed with Sjögren’s with a positive SSA but ANA was negative. But now have a positive ANA with speckled pattern. But my SSA isn’t showing. Not sure what that means. Will find out next month.

    I would love to hear a doctor talk about the frequency of someone with an autoimmune disorder having a negative ANA. Some rheumatologist rely completely on the ANA as if it is the end all, beat all test. I had a doctor listen to my patient history, ask 2 questions, perform a 2 minute cursory physical exam, and then tell me that my ANA was negative, so it wasn't a rheumatology issue. He seriously told me I may just have to learn to live with my symptoms without a diagnosis. I was dumbfounded. Even I know that not every person with a rheumatological condition tests positive. Some conditions have a significant percentage of patients who are ANA negative.

    It’s a very interesting and somewhat controversial issue! And I’m sorry you had such a dissatisfying experience with your doctor. I probably need to do a whole video on the topic, as there is so much more to it than I can put in a response. The way I look at the specific ANA issue is we (the docs) always want to be as specific & precise as possible when giving someone a diagnosis, and when an ANA is negative, labeling someone as lupus may keep someone from continuing to investigate the problem and this is how misdiagnoses can happen. In my opinion, it is exactly these types of situations, where someone has continued symptoms & issues but yet their ANA (or other labs) are negative, where MORE visits with the doc, more investigating and more symptom diary-ing (is that a word?) are useful. Time and a watchful eye can show us so much about what may be going on. Thank you for bringing this up as I think it’s a great topic to explore and hang in there!

    @@ConnectedRheumatology I think the biggest problem is that no one is doing the investigating. At least they aren't investigating the whole problem. The GI doc is looking at the GI symptoms. The gyno is looking at reproductive issues. The hematologist is looking at clotting concerns. Even the PCP is really only looking at the immediate problem. No one is looking at the whole picture. My hope was that the rheumatologist would be someone used to working with systemic problems and actually take a step back and look at me as a whole human. I am working on trying to get to a different rheumatologist that my hematologist recommended. It's just getting insurance approval. That is a whole other conversation.

    yes, in my opinion, the doctors do not look at you as a whole. Instead, they are picking you apart, giving you medicine for this and for that when there is actually a medication where one pill could correct the problem. take prednisone for example; I don't have to take 6 Tylenol a day and 12 Ibuprofen to get out of bed. When I was eating the Mediterranean Diet I was getting much better but i stopped because I need to be diagnosed, so I started eating like I used to. I bend too easy, I don't have inflammation, the list goes on. I want to get better but yet I want a diagnosis. So far I have fibromyalgia and trigeminal neuralgia. I believe I have lupus although they are reluctant to diagnosis the problem. Only way that can happen is eat and man it hurts. Can't wait til I can eat right again.

    My ANA and other autoantibodies are postive and i have severe fatigue that my doctor keeps telling me is a figment of my imagination. When my ANAs are higher, the fatigue is worse, when they're lower, it's better. I'm sick of this bullshit. I need my doctors to tell me they don't know what's going on instead of making it a psychosomatic condition simply because their knowledge isn't there yet.

    I have a positive ANA test 1/1280 granular type, but no symptoms (except for a bit a eyelid eczema) … I have seen a rheumatologist who says it could be a false positive or may have been caused by the COVID-19 vaccines. We will do more tests in 3 months and see what they reveal. In the meantime I normally would get another COVID-19 shot before Christmas….. should I or not? If I do have an autoimmune disease, it would be best to get One, but if my ANA is positive because of the vaccines I would think I should not….. both my family doctor and my rheumatologist are away from work and won’t be back until january. Your advise would be much appreciated

    Hello! I would really like some insight if possible. I have almost every lupus symptom including the occasional butterfly rash. I got blood work done and my ANA is positive, my RNP is high, as well as my Anti-Centromere B Antibodies. I heard that really isn’t related to lupus but can happen and I’m trying to do some googling research but can’t find anything on it.

    Hi doc, i got ana test recently which is 1:160 speckled dfs70, my issue started with right elbow, since that time my pcp thought it’s golfer elbow as it lasted over 2 months, but then I developed a symptoms in both arms, felt join pain in my knees. Now the pain is really moderate but whole my body is crunching and feels kinda weird, overall feeling okay! Waiting for rheumatologist appointment but it can take months in my area;( I’m kinda scared of lupus or arthritis! Praying for healing!

    The wait to get an evaluation by a rheumatologist can be maddening, hang in there! As I'm sure you researched, the DFS70 pattern on ANA testing has NOT been associated with lupus and in considered a way of telling if the ANA means anything or not. While you are waiting to see the rheumatologist, try to see if your joint pain is associated with any daily habits, sleep, diet, certain exercises. Also, be mindful if the pain is more IN the joint or AROUND the joint, as the presence of tendonitis can clue in the rheumatologist to other conditions. The wait to see the rheumatologist can be frustrating, but it does give you time to hone in on what your symptoms are, what triggers them, what helps them, etc. All this information will help the rheum get to a diagnosis faster. Hope this helps!

    Hi Dr. I Love your videos just came across you today! 😉 I have a ? To ask you can a person still have Lupus with a negative ANA? I have a rash that comes and goes across bridge of my nose and cheeks hot to touch, hair falling out in globs in shower, migraines, seizures, joint pains in my hands, wrists and ankles a day Or two after the rash. Sores in my mouth, repeated infections from URI to UTI to skin infections, swelling in legs, pain in my chest, had a CVA in 2018 from a blood clot And Anemia (that requires infusions) I also would like to know about complement blood tests levels what do they mean? Can those indicate lupus as well if they are off the normal range?

    Oh my! It certainly sounds like you have had quite a bit going on - sending you lots of compassion! The issue of whether or not someone could have Lupus with a negative ANA is a bit controversial amongst rheumatologists. Older literature indicated that ~20% of lupus patients will be ANA negative. However, many argue that we have better testing and a better understanding of lupus and other autoimmune conditions and that many (if not most) of those "ANA negative lupus" patients in the old literature probably had another autoimmune condition entirely (but we just didn't know enough at the time to diagnose them correctly). The symptoms you list certainly sound like lupus and if the ANA has been negative I would 1) make sure it's been checked using immunofluorescence and 2) been repeated. In some cases, there are situations where the illness could be doing something wonky to the antibodies, so that the ANA isn't detected when someone is particularly ill. This is why it's important to repeat the test once things have calmed down and the doctor's level of suspicion is still high. There are other autoimmune conditions that could lead to the many symptoms you have and that is something your Rheumatologist (and hematologist) could help sort through. Reviewing complements is an excellent topic for another video (I'll add it to the list) but in short, they are a part of the immune system that can go into overdrive in lupus (and other autoimmune conditions) and lab tests will show them "low" when someone has lupus or is the midst of a flare. Like face rush, psoriasis, joint pains, bad fatigue, high phototoxic reactions and more.. SO, my doctor ordered an ANA-TEST and it came back POSITIVE. Titer 1:400, humogen. The labor recommended to do the ELISA. And ENA was negative..!? Years later I still have symptoms and working on finding out what's wrong with my body. Just wanted to ask few questions regarding my test results as I’m still waiting for my doctor’s appointment and i’m dying to know what’s wrong with my body. My ANA test came back positive (1:360), ANA profile shows DFS70 strongly positive, SSA Native positive and RNP/Sm (+). Based on the test results, is it possible that I might have Sjörgen / RA / SLE?     I'm glad you have an upcoming appointment as your results and symptoms deserve a closer look. I can't give you a diagnosis, but with your symptoms and test results, an autoimmune condition, like the ones you listed, should be considered. The DFS70 pattern is classically considered as NOT associated with autoimmune disease, however given that you have other antibodies (RNP/Smith) and have symptoms, it should still be considered. While you wait for your appointment, I recommend keeping a symptom diary - what makes your symptoms better/worse? Do you feel your "blackouts" coming (with heart racing, headaches, sweating, etc) or do they come out of the blue? Try to be as specific as possible while still noticing general trends (I know, a tall order, it can take time). Good luck!

    My test said ANA pattern cytosplasmic. Another result, said ANA Titer 1:80 , another said ANA screen IFA Positve, and the last said Estradiol 42H. Now I am very depressed thinking I may have a deadly disease. Especially after a chest xray in the ER that showed Mild perihilar interstitial opacities which could reflect pulmonary edema or infection. I'm about to have my first child after 15 years of trying and today I get this devastating news.

    Hi did you find out what it was? I have same exact pattern of cytoplasmic which there is hardly any info on. I have been trying to get answers because I don't see the DR for 2 weeks. The only info I have found is that cytoplasmic pattern can be Lupus, Myositis, or auto immune liver disease. I do believe I read somewhere that the lung symptoms can be related to Myositis an inflammatory muscle disease.

    I am confused. Live in the UK. back in 2000 i went blind temporarily, and for six years i had so many weird things happen. Finally i was told i had a nerurolgogical disorder so went private in 2007 to a nuerologist. He actually listened about my family history. grandfather had R.A. and parkinsons, grand mother R.A., my father had Lupus etc. so one of the first tests he did was ANA. Now this came back POSITIVE FOR LUPUS. I was kind of relieved actually as it explained a lot. I still have the letter with this diagnosis. I was then told i had to REPEAT THE TEST in SIX WEEKS, and yes you guessed it came back NEGATIVE. So back to scatching the head. OH also i found out i have photosensitivy and can not expose my legs to the sun as they turn a purple colour. my nuerologist stuck with me i had a ton of tests for MS, and finally in 2016 i was diagnosed with Primary Progressive MS. BUT HE DID SAY, one of the tests a lumbur puncture muddied the waters as something showed up which wasnt for MS or something i cant remember now, and thinks I did have Lupus or similar. Over the time I have had really weird things that dont match MS. I get terrible dry eyes, nose and mouth, occasionally i feel like i am flaring with a flu, my temp will raise from 36.4 to 37.4. Then suddenly go back to normal. I get pain in my feet so bad i could cry. over those years i have have had raised blood tests but no one takes any notice. My daughter has recently been diagnosed with Sorgrens. I am 71 now and been told through blood tests, i have ANEMIA THROUGH Cronchic disease (as folates etc are good), i have an issue with liver enzemys so being investigated, oh yes and in March i went into hospital with heart issues. I recently had a half body scan and they found granulomas in my lung. but told me it was nothing to worry about. So well i just have to get on with it. xx thank you for a good uncomplicated video.

    I had my first reumatologist brush it off with the homegnous pattern. Then I lost my right foot, wasn't feeling well and my primary care Dr said there is something autoimmune going on. Now I still have the homegnous pattern and centromere pattern (which is the new one) I was told I have sjogrens Gi Dr said it's the reason I struggle with food??

    I've have fibromyalgia, CFS, depression, anxiety, ibs-c, chronic neck/back pain, migraine/headaches, tremors, insomnia, etc. Oh and my hair is starting to shed a lot again. Was dx in 2018. Current rx are Prozac, Linzess, omeprazole, & tramadol as needed. Last wk I had lab work done-ordered by psychtraist & neurologist-- the past few weeks my pain and fatigue has been really kicking my butt. Yesterday my lab ANA ifa is positive and speckled 1:80. I emailed my psychtraist-also a PCP-- about these labs haven't got reply yet. Given my documented symptoms, would my results mean I need to follow-up with more testing with my rheuma Dr? I don't think it's lupus cause I've never had a butterfly rash.

    You certainly have a lot on your plate and I can understand why your doctors were searching for a possible unifying diagnosis that may explain all the symptoms you have. An ANA of 1:80 is considered very low and the decision to move forward with more testing will really rely on the discussion you have with your doctors and/or a rheumatologist. Even without a butterfly rash, this may be an opportunity to get a fresh perspective on your symptoms from a rheumatology stand point. I can't tell you you should or shouldn't get a rheumatology consult, but in my experience, many doctors will prefer a rheumatologist weighs in on results such as yours, which I believe is reasonable. My doctor tested me last year in aug for ANA came back positive 1:160 i decided to remove my implants last year in oct it’s been 1 year since explant my titer for ANA still came back positive but lower 1:80 I’ve been very stressed out with this positive ANA .     Hi! I absolutely loved your video! I just wanted to know if there's a link between a positive ana test (tests were positive 4 years ago and now as well) and increased IgE level (650+) ? Also pompholyx and rashes on the body, fatigue, knee pain (have a bone defect there too), recurrent UTIs, brain fog, etc are present but no malar rash has been seen.     An +ANA test and elevated IgE levels can definitely be seen together. However, how those two lab tests relate to your symptoms will need a thorough eval by a rheumatologist and possibly also an allergist/immunologist. Both specialties study and treat conditions of the immune system and there is some overlap, but they typically take care of different conditions. The key will be trying to figure out if your symptoms are being driven by the "allergy side of things" (which is my oversimplified way of looking at it) versus the "autoimmune side of things." And your lupus tests will help clarify that. Hope this helps and good luck!

    Hello, I do have a question! I have a positive ANA but a negative ANA HEP-2 titer? I’ve always been positive for the general ANA, and always negative for the ANA HEP-titer. I’ve had a crazy history of alopecoa, low grade fevers, you name it. But my symptoms might be secondary to my POTS diagnoses. My grandma had RA and my aunt on maternal side - Lupus. I’m unsure if I should keep a rheumatologist in my Arsenal or if the negative titer and otherwise clean bloodwork (with the occasional elevated ESR) means it’s something I shouldn’t revisit.

    It sounds like you are running into a common problem people find themselves in when their ANA is checked using different lab techniques. Regarding your ANA results: the gold standard testing method for detecting the presence of an ANA is Immunofluorescence (IF), which is the Hep-2 titer you refer to. The "positive general ANA" is likely a screening test the lab does using a different method, called ELISA. Regardless of the initial ELISA result, with a negative IF, this means, for all practical purposes, your ANA is negative. But then there's the issue of your symptoms and who is best to care for you. In cases like this, I always find it helpful to focus on the individual doctor as opposed to their specialty. Some rheumatologists are familiar and comfortable with POTs and if you have found someone like that, then I'd say keep them! Some Neurologists and Cardiologists (who also take care of POTs) are familiar with the basics of autoimmunity and could help identify if/when new symptoms arise and labs should be tested again, in which case, maybe the Rheumatologist isn't necessary right now. Hope this helps!

Currently had so bad flares and actually had bunch of connecting dots of autoimmune disease symptom specifically SLE. But i just dont know how to tell the dr. To refer me to rheumatologist. :(
