4 Signs That Bending Forward May HELP Your Herniated Disc

Forward bending and disc herniations aren't generally something people mix together... But why not? The world has been led to believe that bending backwards "pushes the jelly back in the donut" and reduces pressure on a sensitive sciatic nerve... but is this really true? Research shows that bending forward actually has the ability to reduce a disc protrusion, whereas bending backwards may further compromise the disc and the sciatic nerve root. Sure, bending backwards sometimes helps people... but it sometimes hurts people, too.
In this video, I share 4 signs that indicate flexion (or bending forward) may actually help you- not hurt you.
Remember, beliefs inform actions which inform results.
If you believe extension (bending backwards) is best and forward is bad... you'll push through pain into cobras or press-ups- which will make you worse. And you'll keep doing so until you change your belief system. If you open your mind to the possibility that flexion could actually be good for you... you'll create a new opportunity for healing.

@@drcharliejohnson I’m also experimenting with raising most of my feet so that the only part of foot on floor is my toes while I am sitting inclined forward - I think that may help, too. Though I’m not sure if that’s going to lead to a long term pain relief or cure for sciatica

She's having to use muscle to "sit up tall" because she hasn't shifted her ischia back-the coccyx is tucked under. You have to stick your bum out as you sit down, and then you don't have to use muscles to sit up straight-you maintain the low back curve without having to use muscles. If that causes discomfort then for sure, don't do it. You may have an issue like a forward bent sacrum, a sacral rotation, many things are possible from a Hesch Practitioner perspective. I totally agree that pain is a message that something is not right and we shouldn't persist in a an exercise or position that elicits pain. When she bends forward, you can see that her right side is superior (looks higher) than her left. That's a sign of a pelvic issue that could possibly be treated. Regardless, whatever works-do it! Thanks for the video Charlie.

I have more pain when I walk or stand. So I tried these forward bends. I think this helped. How many times per day?

I have spinal stenosis and my facet joints are bad enough that it shows up on an x-ray. I did 20 PT visits last year and they insisted on bending backwards. It didn't help. Now I'm doing RFA and after almost a week I guess the nerves haven't died yet. I still feel pain, particularly on my left side. Bending forward feels much better than backward. I do the classic "lean on the cart" when I shop. I'm not sure this pain on the left is facet joint or not. No stretch I've tried seems to help more that a few minutes. But yeah, if this your issue, run away if you hear "McKenzie Method" at the PT office. I've searched You Tube endlessly. What can I do to fix this? I bend forward a lot but there's no lasting relief.

Hey! sorry to hear. Sounds like it may be facet or stenosis as the cause for sure... but would need some more details. Many motions to explore and would need to do some more detective work here. No pressure, but I'd be happy to review your case personally to see if/how I can help. If you'd like- just visit www.drcharliejohnsonpt.com/apply - select a time to chat, then follow instructions to submit a formal Case Review. Hang in there! Dr. Charlie PT

Good video. Is it safe to say most people have extension bias? I heard a stat that people make some type of flex forward motion a thousand X day which can lead to back/butt/sciatica issue.

Many people will have a bias to extension. But recent research shows that using a directional preference (extension for many)- Mckenzie Method type of approach is not superior to other treatments in reducing symptoms.

@@drcharliejohnson how common is hamstring entrapment of sciatic nerve for someone with bi lateral mainly lower leg, ie from knee to big toes?

@@Theclassiccarnut good question- I'd have to do some prevalence research on this... we call this "Hamstring syndrome" and my guess is that bilateral hamstring entrapment is extremely rare... so much so that it wouldn't even be in the diagnostic deck of cards with symptoms as you describe.

Not sure what has changed for me but bending forward and over to pick something up now causes horrific spasms in the upper butt muscles and nerve pain just above it. I can't bend backwards either. Lying down causes freezing of the lower back. I have a bulging disc in L4-L5. I'm currently thinking this is the piriformis, it feels stuck and extremely painful.

  • @drcharliejohnson

Sorry to hear- doesn't sound like a piriformis problem- especially if moving your back causes pain in the butt/leg. Usually this is disc related (but could be other things)... and sometimes yes, both bending backwards and forwards is yucky... in which case we can test rotation/sideways motions... so many options to explore.

Many ways. Reclined back to open the hip trunk angle. Tilted left, tilted right, trunk twisted left or right. One leg dropped off the side of chair. Other leg dropped off side of chair. Cushion under one cheek vs cushion other other cheek. So many ways to tweak sitting so that you can find comfort. Time to get creative. Remember, if sitting isn’t painful, don’t change it. 👍
