  Jennifer Riordan was the one who died. When the engine failed, a pice of the engine shattered the window she was near to and she was partially sucked out of the plane. Although she would be recovered by the passenger next to her, she succumbed to her wounds

An accident is defined by the FAA as meeting one of the 3 criteria between boarding and deboarding: someone dies, someone is seriously injured (more than 48hrs hospitalization, 2nd degree or worse burns, and more), or damage occurs resulting in the aircraft no longer being airworthy. Since someone died as a result of the engine explosion, this is considered an accident in the eyes of the FAA
