Helen Hotai - Form Help Founder

The purpose of this channel is to provide video's/tutorials on 'How To' complete various types of application forms either online or in hardcopy form. The videos are not limited to forms, but also educates the public particularly in NZ and Australia to available services they may not have been aware of. I understand there are people out there, who may need a little assistance, some step by step guidance or help to know where to look or what types of questions to expect in an application and how to approach them.

I hope these 'How To' video's will aid and benefit those seeking that assistance.

    Both me & my wife have bought return tickets to Melbourne mid October 2024 for holidays & visiting relatives. We applied ETA yesterday night in Kuala Lumpur using my mobile phone and paid AUD40. Within minutes, both of us gotten an email from ETA. She got an instant approval visa under subsclass 601. Meanwhile I received a pdf attachment requiring me to click immiaccount website link. After creating an account, I came across the option to apply various subclass. There was no 601, only option for 600 and others, which I then selected 600. I filled up and attached uploads totalling 19 pages before pressing submit. To my dismay, it was shown AUD195 that I need to pay before submission is complete. I came multiple times to Australia before pandemic. I owe no money to the govt as I am not a PR or citizen. But I have an unsettled traffic summon from car rental back in 2017 in Perth. So why is my wife having instant approval but me having to fill up the 1554 e-form and subjected to pay AUD195? Both us in the 50s so low risk of us working illegally or overstaying. So why I do not have option to apply for 601 for free?

    Hi @rakeshr8154 When my 18 yr old applied to renew their Australian Passport in NZ: 1. From the time they submitted their passport application via the Australian High Commission in Wellington, NZ 2. To the date of collection at the Australian High Commission in Wellington NZ; 3. It took around 3 - 4 weeks. 4. Depending on how many Australian Passports they're processing at the time may also dictate how quick you receive your passport.     Hi @Mohammad-qh8gw 1. This tutorial was from almost 3 years so it's highly possible the process in the tutorial has changed slightly. 2. However, because you need an updated photo for your passport - I am sure they will be contacting you to either upload one or send in a hardcopy version. 3.     This is a nightmare. People. Don’t believe it!! They have made it impossible for eligible passport holders to enter australia as they used to easily so either you don’t travel to australia OR pay them $200. It’s all about money guys. It’s all about money. My friends used 4 different phones across the world and none of them were able to get visas. All American citizens!!! This is a scam! To emphasise, none of my friends were able to even pass the image section. The app is faulty. They couldn’t even get to step 2. It’s impossible to use this FAULTY app. Btw, if you are from Palestine or lebonon, they have a special way and number and portal for you to get visas easily and quickly. So you know.

If you've chosen a guarantor with an Australian Passport that lives in Australia, they need to: (a) Meet the criteria in section 11 - *Who can be a guarantor* (b) You'll need to send section 11A of your passport application for them to fill in and sign; and (c) Send your passport photo for them to endorse OR 2. Get a guarantor who lives in Italy who meets the criteria of an approved occupation or one of the groups on the list, they will need to: (a) Meet the criteria in section 11 - *Who can be a guarantor* (b) Fill in section 11B (b) endorse your passport photo (c) *see what the UK section below says - it looks like this professional person should have known you for *at least 12 months* (this person doesn't need to hold an Australian Passport) www.passports.gov.au/getting-passport-how-it-works/how-get-adult-passport#:~:text=Unless%20you%20qualify%20for%20a%20passport%20renewal%2C%20you'll%20need,the%20same%20address%20as%20you. ======== The next section was from the *Australian High Commission UK* you might find the information helpful: *Passport guarantors* If you are submitting the PC7 Adult Renewal form, a guarantor is not required. Usually this is because they have recently relocated to the United Kingdom and do not know anyone here who has known them for longer than 12 months. If you are unable to obtain a guarantor in the United Kingdom, you should obtain a guarantor from a country where you previously lived - eg. Australia. You will need to send them Section 11 of the PC8 form, together with one of your passport photos, for them to sign. Long answer: Here's what it says on the NZ Passports page: *Passport timeframes* Find out how long it will take to get your passport. This page is updated regularly to reflect current processing timeframes. A 'working day' is Monday to Friday, excluding public holidays and the extended break over Christmas and New Year. *Standard passports* Standard passports are currently being processed in up to *20 working days.* Allow extra time for delivery. We encourage you to consider this timing when planning a trip and apply for your passport with enough time to spare. *Urgent passports* Urgent passports are processed in up to *3 working days.* Allow additional time for delivery. If you need to travel sooner than 3 days, apply for an urgent passport and then call us straight away.     I'm just trying to find out whether I need to mention referee details when I apply to renew my Australian passport online. I thought I needed referee details only when I applied for a new passport for the first time. When I renewed my passport last time in 2015, I didn’t need to mention any referees’ details. Thanks.

I just logged in to *AusPassport Account* to check out the renewal process as of 07 February 2024 - here's some points worth noting on their website: (a) *RENEWING AN EXISTING PASSPORT* ADULT: You must have your recent passport (issued from 01 January 2006) and two new photos If you don't have your recent passport you need to apply for a new one (see below) If your name, gender or birth details have changed there are extra requirements (b) *GETTING A NEW PASSPORT* ADULT: You must have: Proof of citizenship: either Australian citizenship certificate or Australian birth certificate (if your birth certificate is after 20 August 1986 there are extra requirements) Multiple identification documents that between them show your name, photo, signature and address (e.g. drivers licence, bank cards, Medicare card) Someone you've known for 12 months (not a relation) to confirm your identity Documents to show your gender and place of birth if born overseas Evidence of any name or gender change Two new passport photos Previous passport details (if you've had one) 2. I've looked through the questions to *renew* an Australian Adult Passport - and I didn't come across any questions requesting referee's. 3. According to point 1(b) above - if you're *Getting a new Passport*, then someone you've known for 12 months needs to confirm your identity (referee). Where as 1(a) *Renewing an existing passport* - makes no mention of this requirement. 4. When you log in to your AusPassport Account to renew your Australian Passport - you'll be able to see for yourself the questions you are required to answer.     Hi @whowho7805 1. The short answer - I don't have a link to the hard copy version and it doesn't look like Australia Post provide hard copies either. You'll have to call the number below. 2. *This is what it says on the Australian Dept of Foreign Affairs website* (see the link below for more info) Adults (18 years and over) For a renewal, you will need a passport that meets all the following: was issued when you were aged 16 or older was issued on or after 1 January 2006 had, when it was issued, at least two years' validity (for example, issued on 1 January 2010 and expired on or after 1 January 2012) has your current name, date of birth, place of birth and gender has not been reported lost, stolen, or cancelled. If you can answer yes to all the above, you can renew your passport using our online portal. *If going online is not an option, you can call us on 131 232 to get a pre-filled form sent to you. Renewal forms are not available at Australia Post. If you are outside Australia, contact your nearest Australian diplomatic or consular mission(link is external).* If you answer no to any of the above, you will need to use our full application process. www.passports.gov.au/getting-passport-how-it-works/passport-renewal Hope that helps - all the best 🙂👍🙏

    Hi @masomahussaini3496 If you click on the link below it will take you through the 5 step guide: Step 1: Gather original documents and two passport photos Step 2: Ask someone to be your referee or guarantor Step 3: Fill in your application Step 4: Lodge your application and pay the fee Step 5: We’ll tell you when your passport is ready www.passports.gov.au/getting-passport-how-it-works/how-get-adult-passport In answer to your questions and as per the above link: 1. Photos - YES *All applications must include two passport-sized photos that comply with our photo guidelines.* 2. Application - *Application forms are available online. Our system will generate an application form that you will need to print.* (a) Passport renewal forms are available online, but are not available at Australia Post as they come pre-filled. If you can’t use the online portal, you can call 131 232 to request a pre-filled form. 3. Passport - *If you’re in Australia, and have a current passport, it will be cancelled when your passport application is assessed.* (a) *I'm not sure if you need to send in your old passport - when you go through the application it will list all the supporting documents you'll need to provide as part of the check list - if it's not on the list then you don't need to provide it. But ensure you do provide all the documents requested. 4. If you want to explore your visa options then click the link below: immi.homeaffairs.gov.au/visas/getting-a-visa/visa-finder (a) If you select *Visit and Tourism* you will be asked for the *Purpose of your visit* (b) Based on your reply to the next questions - you will be given a list of visa's you are eligible to apply for 2. According to the Application for NZ Citizenship by descent and passport (Adult 16+ years) (linked below) (a) YES - you do need to submit your current passport as part of the application process (b) YES - that means that if you need to travel internationally, you will not be able to travel outside the country without your passport, unless you have dual citizenship and hold another passport for another country. 2. Maybe contact them to let them know your travel dates and see whether you could have your passport returned before you travel? www.govt.nz/assets/Documents/Passports-citizenship-and-identity/application-for-new-zealand-citizenship-by-descent-and-passport-adult.pdf Hope that helps!     Hi @ShyamEnterprise My apologies, can you please help me to understand your question? 1. Do you mean your passport is damaged and you need to apply for a new passport? 2. If so - click on the link below that gives you a step by step guide on what you need to do if your NZ passport is damaged: www.passports.govt.nz/lost-stolen-or-damaged-passport/ 3. From what I can see on the above link, you will need to: (a) apply for a replacement passport; OR (b) declare it damaged; (i) approximate date it was damaged; (ii) the city/country it was damaged in; (iii) what happened (c) If you want to replace the damaged passport (i) it will be cancelled as part of your application for a new passport; (ii) this means you will need to apply for a new passport Hope that helps - all the best 🙂🙏

    Hi @rabirandhawa3555 Look at the 5 Step Guide on the link below to help you make a start: 1. How to get a passport when overseas www.passports.gov.au/how-get-passport-when-overseas 2. Step 4 on the 5 Step Guide says to allow at least 6 weeks to receive your passport after you lodge your application. (Maybe submit your child's passport application at least 8-10 weeks before it expires to be on the safe side - but totally up to you). Hope that helps - all the best. 🙏🙂

    Hi @madokariki2726 You're most welcome! I would say that whoever has the items to declare in their luggage, will need to indicate those items on "their" NZTD. If it's just all in your personal luggage, then declare them on your NZTD. If you have some in your luggage, your partner's luggage and your daughter's luggage, then declare who has what in their luggage. (that's just my personal opinion. See their contact details below and they'll be the best ones to answer your queries). I've included some info from the NZTD website below: Everyone entering New Zealand needs to submit a declaration. It is free and takes about 10 minutes. You can do it on the NZTD app or online form, using your phone or computer. The earliest you can submit your declaration is 24 hours before you start your trip to New Zealand. It needs to be submitted by the time you reach passport control in New Zealand. Someone you trust can help you fill it in or do it on your behalf (with your permission). A declaration needs to be completed for each traveller, including babies and children. Your digital declaration is linked to your passport and is checked when you arrive at the eGate or by a border officer. There is no QR code issued and you don't need to print anything out. There is a paper declaration form available, for travellers who cannot complete it online. *Questions about your declaration* If you have questions, or would like more information about the New Zealand Traveller Declaration, visit travellerdeclaration.govt.nz.     Hi @anhlacthien1477 For this example, the person didn't have an Australian Citizenship Certificate, just an Australian Birth Certificate - and they were able to renew their Australian Passport. Hope that helps to answer your question - all the best 🙂

    Hi @kvvill1868 This is an old KZread tutorial from 2 years ago, so maybe things have changed for older applicants. If you were able to submit your grandmother's passport renewal online without having to provide referee's or proof of citizenship, then maybe the requirements for older Australians is different? I'm sure they will contact your grandmother if they need her to provide any further information. I've just renewed my kids Australian passports - I filled the form in online, printed it off and had to submit a hard copy to the Australia High Commission here in NZ. It will tell him which passport he will need to apply for 4. It will likely say he needs to: *Renew your adult Passport* NEXT it will ask you to do/provide the following: 5. Login to RealMe or create a RealMe login if he doesn't have one; 6. Previous passport details; 7. A passport photo; 8. Payment details; and 9. Delivery details *REMEMBER*: Once you have submitted this application, the passport you are replacing cannot be used for travel. www.passports.govt.nz/

Here's what it says on the NZ Government Citizenship website: *Your children’s citizenship - If you are a citizen by descent* - Your children who are born in New Zealand are automatically New Zealand citizens. Your children’s citizenship is registered at the same time you register your baby's birth - You cannot pass citizenship to any children who are born outside New Zealand. They will need to apply for New Zealand citizenship by grant. - As a citizen by descent, you can also apply for citizenship by grant. If you are successful, any future children you have who are born outside New Zealand can register as citizens by descent. www.govt.nz/browse/passports-citizenship-and-identity/nz-citizenship/types-of-citizenship-grant-birth-and-descent/ 1. According to the above and based on the information you've provided, this is how I interpret the above: (a). If your father is a citizen by descent and you were born overseas, then (b).     Hi @agoogleuser4299 - I hear your frustration! It can be a long and arduous process! In saying that - a lot can happen in 10 years - I'm no expert but here's my thoughts: 1. *Referee* If the Australian Government suspect the passport applicant is a person of questionable character - they will want to follow up with the referee to ask them some questions about the applicant. OR they may select a random applicant and do the same thing. 2. *Contact details* People's contact details can change over a 10 year period, eg - people move, change their numbers, their email addresses, they divorce and change their names etc. If you do move and you haven't updated your contact details - then your passport will end up at an old address in another state from 10 years ago! 3. *ID* The Government want to ensure you are who you say you are. There's a lot of identity theft going on - they want to ensure that the person applying for the passport/renewal is you and not someone trying to steal your identity. 4. *2 Photos* Unfortunately, some of us age in the 10 years that we've last taken our passport photo. Therefore, they need an updated photo to reflect that maturity! They'll use one of these photos for your passport and the other one for their file or as a spare? 5. *Emergency contact details* Again, in 10 years - your emergency contact's details may have changed. They may have moved? You may have had a falling out and don't want to use that EC anymore, or they may have passed away etc ... In an emergency - the Government will be contacting the EC listed on your application to give them any updates, therefore it's important to ensure the EC's contact details are updated as they will be the one communicating these updates to your loved ones. In saying all of that - like I said - I'm no expert, those are just my thoughts. Just know that there is a process in place for a reason and personally I trust that process. I believe it's a process that's in the best interest of the applicant and for the national security of Australia. The Australian Government has a duty of care to their people and part of their duty of care is to ensure that they aren't just issuing Australian Passports left right and centre to anyone and everyone who wants one.     Hello, I'm Colombian but I am also a resident from the United States, I have a green card but I don't have an american passport because I'm not a citizen yet, can I still apply for the ETA visa? the embassy told me I could apply, but how are they going to know I'm an american resident if they don't ask for that information, just passport details. I've looked on the Australian Government Home Affairs Page to see if South America/Colombia fall within the "ETA eligible passport holders list" - if South America comes under "United States of America" then you can apply, HOWEVER, I'm doubting that it does and I couldn't find any further information on the website to aid in answering your question. On the other hand, I'm guessing that if the embassy advised you to apply, then they know what they're talking about. 2. If that's not of much comfort, I found that iVisa have some information on their website that you might find very helpful "Australia Visitor Visa for Colombian citizens: Everything you need to know". There appears to be some confusion here - some said the Traveller Declaration is no longer needed, yet some said NZ government has reintroduced the declaration. Thanks.

    Hi @tytan9139 - GREAT QUESTION! I had to look it up for you - short answer is "YES" the NZTD is being re-introduced from July 2023 but only at selected airports. Here's what it says on the website: Travellers currently need to fill in a paper Passenger Arrival Card, which is handed out onboard your flight to New Zealand. From July 2023, travellers arriving at selected international airports in New Zealand will be able to complete an online declaration. Travellers arriving into Christchurch airport or Wellington airport will be able to complete an online declaration from the week starting 10 July 2023. Travellers arriving into Queenstown airport will be able to complete an online declaration from mid to late July. Travellers arriving into Auckland airport will be able to complete an online declaration later in August 2023. The Passenger Arrival Card will continue to be accepted for a period of time after the NZTD is introduced. Customs New Zealand are working with other border agencies, including the Ministry of Business, Innovation and Employment, the Ministry for Primary Industries, and the Ministry of Health, to modernise our border.     Hi @Kevin Ucci - that is very unfortunate! I imagine it will cause some delays. The only thing I can suggest is that she contact them ASAP and let them know that she has made an error in her application - in that she accidentally checked that she has a criminal conviction. If she doesn't have a criminal conviction/record - she needs to include and explain that in her communications, whether that be over the phone or via email. Hopefully she'll hear back from then soon and they'll give her instructions on how to progress her application from there. All the best @Kevin Ucci - hope it all works out in your sister's favour. If you see under 'Step 2 - Apply for the Visa' you'll see it takes you through a step by step guide on how to apply for the ETA To apply for an ETA using the Australian ETA app, you need to have: - A mobile device camera enabled which can take your photo and an image of the Passport data page - Near-field communication (NFC) enabled on your mobile device - Location services enabled on your mobile device - A valid email address - A valid payment method ready to pay the service fee 2. You'll also notice that you are required to: scan your passport to pre-fill your name, date of birth, passport number and other details 3. If you don't have a chip - I would 'try' scanning my passport anyway knowing that it's going to fail to read anything in the hope that it will then ask me to fill in my details manually 4. If it doesn't allow you to fill in your details manually; THEN 5. It does suggest the following: *If you are ineligible or unable to apply for an ETA using the Australian ETA app, you can apply online for another visa that suits your needs through ImmiAccount.* - European passport holders may be eligible for an eVisitor (subclass 651) visa. There is no charge to make an eVisitor application. - Other passport holders may be eligible for a Visitor (subclass 600) visa. 6. Normally, you should be able to start typing your address and it 'should' hopefully show up with an option that matches your address - then you go ahead and select your address from the options presented. 2. If your address is not listed - that would be unusual 3. Your other option is to contact the passports department and seek their assistance. Here's what it says on their website: *Your Identity referee or witness must:* - be available if we call or email them to verify details about your application - this will help avoid delays. www.passports.govt.nz/what-you-need-for-your-application/identity-referee-or-witness/ 2. I've been a referee/witness a few times and I've never had to verify details for the applicants application 3. People who have been referees/witnesses for my family members have never had to verify their details. 4. They must randomly select applications for verification or maybe if an application looks dodgy they must get selected? Who knows? Hope that's of some little help.     Hi @Dipesh Namjali 1. Effective 20 October 2022, Travellers entering NZ are no longer required to apply for NZTD for a NZ Traveller Pass. 2. However they do state that you ensure you have the appropriate visa as applicable. 3. Safe travels and have a wonderful stay in Aotearoa NZ! 🙏😊

    Hi @Claudia Guzmán Pérez 1. The emergency contact person *doesn't* need to reside in NZ - they can be a family member, a friend etc ... who doesn't live in NZ. a. See time stamp: "7:00" 2.     Hi @Bottom Spazz - this is what it says on the Home Affairs website: *WITH THIS VISA YOU CAN* Travel to Australia: - as a tourist - for example, to cruise or have a holiday - to visit family or friends - as a business visitor You can study or train for up to 3 months in total on this visa. If your main reason for travelling to Australia is to study, you should apply for a student visa. *HOW LONG YOU CAN STAY* This is a temporary visa. It is valid for travel to Australia for either 12 months, or for the period your passport is valid, whichever is shorter. The visa is granted with multiple entry. You can leave and re-enter Australia as many times as you want while it is valid. *You can remain in Australia for 3 months after each entry.* If you stay in Australia for long periods or make frequent visits to Australia, we may ask you about your travel purpose next time you enter Australia. If you are found not to be a genuine visitor, your ETA may be cancelled. If you want to stay longer than 3 months at a time, you need to apply for a different visa that suits your circumstances. The visa starts on the date it is granted unless you hold another substantive visa that is in effect. *PROCESSING TIMES* In most cases, you will be notified of the result of your ETA application immediately. Your application might take longer to process if: - you don't fill it in correctly - we need more information from you - it takes us time to verify your information - Processing time for this visa is not available.     Hi Helen, when I got it right, NZ no longer requires pre departure tests or proof of vaccination...when I did my traveller declaration there wasnt even a section where i could have been able to upload vaccination proof. This feels so strange because NZ was one of the strictest countries regarding covid:D

Go ahead and put in the departure details for the Philipines to Taipei flight. 3. FYI - this is an older tutorial - so you wont need to provide proof of covid 19 vaccination too. I personally interpret this to mean list down the countries you have visited, will visit, or will transit through in the 14 days prior to your arrival in NZ 2. I would not have considered my home country that I’m departing from as a country that I’ve ‘visited’ as I’m not a visitor there. 3. However, because I think this question lacks clarity, to be on the safe side, personally I would add my home country that I’m travelling from to the list of countries I’ve visited. You go with how you interpret the question. 4. If you opted my interpretation you would go ahead and add ‘Germany’ and ‘Singapore’. If not - then just list ‘Singapore’. 5.     Hi Helen. My mum and my dad applied the ETA visa the ETA app together. Somehow that my dad's got approved straight away, and my mum's did not. So My mum has applied it again. But again it still did not approved as quickly as my dad's. Should my mum wait for few more days as it is the weekend at the moment, or to reapplied the whole thing again?

    Hi @ivan chow - here's what it says on their website: After you apply: In most cases, you will be notified of the result of your ETA application immediately. Sometimes there will be a delay (of up to 12 hours) before receiving your notification. Check your emails, including your junk mail folder, for a written notification from us. Your application might take longer to process if: * you don't fill it in correctly * we need more information from you * it takes us time to verify your information immi.homeaffairs.gov.au/visas/getting-a-visa/visa-listing/electronic-travel-authority-601#HowTo From memory I recall reading somewhere that it says dont reapply as it can cause further delays. However, my memory could be wrong. Go ahead and check mum’s spam and junk mail for an email from them in case the email from them is in there OR use dad’s confirmation email to see if you can send can send in a follow-up email regarding mum’s application. Hopefully they're just going through the process of doing the checks and mum will hear back from them soon. Hope that helps, all the best and take care. 🙏😊

The official website is suggesting applying for an ETA.. 1) How to apply for an ETA other than the ETA app? 2) Is there any way to submit an application online? 3) Can I apply for an ETA for my friend who is having a US passport without having him present physically with me?

    Hello! @Citimore Preethi 1. The Australian Govt says: "All ETA-eligible passport holders MUST apply for an ETA using the *Australian ETA app*." Eligible passport holders include the following: Andorra Austria Belgium Brunei Canada Denmark Finland France Germany Greece Hong Kong (SAR of China) Iceland Ireland Italy Japan Liechtenstein Luxembourg Malaysia Malta Monaco Norway Portugal Republic of San Marino Singapore South Korea Spain Sweden Switzerland Taiwan (excluding official or diplomatic passports) The Netherlands United Kingdom-British Citizen United Kingdom-British National (Overseas) United States of America Vatican City immi.homeaffairs.gov.au/visas/getting-a-visa/visa-listing/electronic-travel-authority-601#Eligibility 2. Unfortunately, it looks like there is no option for an online application other than through the Australian ETA App - as stated: "If you are UNABLE to use the app, you can apply online through ImmiAccount for ANOTHER visa that suits your needs." 3.     Hi @Mandy Allan 1. You’ll need to enter your flight details manually if it doesn't show up automatically when you enter the flight details. (I explain this in the updated tutorial if you need to refer to it) 2. They have recently scrapped the requirement to disclose your vaccination status, meaning you’re not required to provide a vaccination record. Hope that helps, all the best and safe travels. 🙏😊
