    Teachers are among the lower paid compared to other professionals (paid anywhere between 20,000 to 75 K MAX). They are the ones who prepare the other professions and are undervalued because the culture is so antii-intellectual.

    Hi, loving your content. I think there is a small error at the start of the video regarding when to start abx The Anthonisen criteria is based on the presence of increased dyspnea, sputum production and sputum purulence (i believe you said dyspnoea, sputum and cough)

    This is so helpful as a patient who has had cellulitis mostly in lower left leg, but sometimes both - 12 times in the last 3 years!! Oh and occasionally under my eyes! 100% of the time brought on by extreme stress (lowered immune system) it's never a cut, or any of the other questions they ask me. I've been on several of these drugs you mentioned, but I'm going to write them down so I'm given the better one. Though ideally I'd love to really solve what is happening (besides the extreme stress) Why can't my body handle the stress anymore after 55 years? (I don't expect an answer - it's just an odd circumstance)

    You mention LR having Crystalites, does this make it high in oxlates?? I find I am often really sore arms and legs after recieving a liter or LR vs Sodium Chloride?? Just curious??

    Great video. How many hours do physicians spend with paper work per day? Do you do this notes in the end of your day or while talk to pacients? Is faster to dictate? Do you spend more time with pacients or doing paperwork? Thanks🙏

    The most stringent parameter in our body is osmolarity. Glucose is osmolaric effective which means that it's not that simple. Besides, bacteria love glucose. Again, it's not that simple. SGLT inhibitors may have its indication but it must be more closely defined. I had quite a few patients on my ward because of them.

    Amazing video. Thanks for such detail lesson. Among the 2, Dapaglifazon and Empaglifazon, which one is better for initial stage renal patients with egfr around 40 and creatinine around 1.7, 1.2 being normal? Also, are SGLT2 Inhibitors better than Gliptins like Linagliptin among ckd patients?

    I’m on Mirtazipine 22.5 along with 25 mg Zoloft. This is the best medication I’ve ever taken for my depression and anxiety the downside is the weight gain. I don’t want to stop Mirtazipine because it work’s amazing but the weight gain sucks.

    I would like to know your thoughts on using the other service order, that is, with the setter following the outside hitter rather than the middle hitter; in 5-1 and also in 6-2. I came to think of these two variants as being either "natural" in the front row, or "natural" in the back row (as you have them in this video).     Ct brain won't help in the first few hours of ischemic stroke except for exclusion of hemorrhage or other pathologies the diagnosis mainly clinical and in such suspected cases with no obvoius metabolic derangment mri is more sensitive if available

    Are you not using Z-scores in your PFT results. Also we are now using GLI reference values under ATS/ERS 2019 and lower limit of normal.

    I am a pharmacist and if we had teachers like you, we would not have taken our naps during class lol. We need physicians like you but go in teaching. I tell people to take keflex 1000mg bid instead of 500 qid. Ok?

    No, no the claim that it is more sedative in lower doses is debunked years ago. Why people get less sedated with higher doses are due to the fact that they have started at a low dose and then when they get a higher dose they have developed tolerance for the sedative effect.     I have a question for you. When you have a case of hyponatremia from severe hyperglycemia (ie. dka) and as you correct the BG, sometimes the Na corrects rapidly. Do we have to worry about that (keeping in mind this is a true hyponatremia and not pseudo like many ppl say but at the same time no hypotonic state was made from the beginning given the hyperglycemia so risk of OD should be low?)?

    Hey Conan, thanks for sharing your experience on using Anki. I also was a heavy Anki user but realized that I was never able to synthesize the information to generate high order learning structures or understanding of the discrete facts in my brain. I know tons of facts about the kidneys, brain, etc, but always found it hard to put together the bigger picture using anki. Any tips for overcoming this? I'm a newly minted med school graduate. Ive been thinking about using anki as a scheduler for free recall sessions, which goes against the principle of making each card one fact, but stimulates your brain in a more meaningful way. Instead of: Define sinusitis, the card may be: walk through the work up of a patient with chronic sinusitis (which forces me to recall the presenting symptoms, the diagnostic criteria, physical exam, imaging, treatment, etc). My current idea is to use my tablet or notepad to free recall this process over a 5 min period and then compare it with a summary sheet that was generated in making the card. 100% of important details would score as an "easy", 80% would score as "good", 60% as "hard" and below that as incorrect. It's a bit arbitrary, but my idea is to foster a better connection with the information. vs just seeing bits as individual facts. Curious to hear your thoughts.

    good video!! but I still have a question, so when you say increasing the IPAP will help the pressure support and cause them to expire more C02; why does that occur ? is that just from natural processes like the more we inhale the more we expire ?
