  So what u see at the first start of the video is when medic shoots himself. The reason on why he was alive is because he gad revived because of his emisis blue and schizophrenia. If you want proof if you go back to the emisis blue movie watch the part where medic try’s to save scout. He was shot once but then revived by some black substance which can only be his schizophrenia. After he comes back he had gone insane which caused him to kill the two brothers and save scout. But scout died after punching medic and running away to his death. My theory is he had learn to control his revive and get revenge. After spy talking about him talking over everything medic revives and shoots him twice because of scouts line. After they take the suitcase because of the secrets that hide in there. Spoilers the secrets in the suitcase where emisis blue pills. Yeah so the whole thing about the story his the medic was actually dead the whole entire time. He was witnessing these stuff because he was still alive. But the whole entire time he could not move because of all the injuries in the car crash. He died because he was going through a phase of schizophrenia. Back to when the medic got out of the coffin. He actually planned all of this out and knew he was going to die. As they did not know the medic revived after spy speech saying he was going to control the place. This is the reason he shot everyone in the movie. He wanted more. But as you know medic came out the coffin and put an end to his speech. When medic went to the bathroom the person with no eyes was looking for the exact person… The medic. This shows they planned all of it because when medic went in the bathroom he saw pills and took them and turned out to be emisis blue. They should really make a part 2

  An easter egg is that at the start of the movie scout says to the medic "if someone hits you,you hit back twice as hard you hear me" and then the medic is shoot one time by the spy but then when he comes back to life a second time he shoots the spy two times btw cool easter egg and as a person who watched all emesis blu i can say that it's the best movie i have ever seen and i really recommend it

  Well, it was hinted many times that Medic has split personalities or violent alter ego. Hence when he Russian Roulette, he only killed his “peaceful” personality and when he came back, his alter ego becomes his dominant personality.
