    I live in Santa Monica,Ca. Do U know of any experienced TCAR doctors in S.M. or near where I live who accept Medicare & LA Care insurance? I have maybe 75% blockage on 1 corotid artery & 50ty% on the other carotid artery. I'm getting 1 more test to see if indeed 1 is so blocked & then if so I am guessing I will have to have a TCAR since as U said it is a 50ty cent surgery with a million dollar cost off something goes wrong & it can with the old procedure. I was wondering if there is any way to NOT have surgery & get the plaque to go away? I wish to lose weight, but have so many bone & joint issues doing much exercise even walking is terribly painful for me. I use to B a long distant runner, but now just easy walking is killing me. I know being 50ty lbs over weight has much to do with the pain too. I already don't eat meat.I eat organic mostly & try not to eat too many bad things like potato chips & ice cream. I love carbs so that is a big problem for me.. I only feel full if I eat carbs. Love potatoes,noodles & bread. Organic & good quality carbs, but they R still not good for someone like me. Pre-diabetic & low oxygen when I sleep down to 77. So I am worried. I do have vision problems as well.. Blurry vision, double vision too. Wondering if that is what U said pieces breaking off causing a sort of baby stroke for a few seconds.. or mins..? I sure hope not. I wanted to know if they remove the plaque when they do the TCAR procedure or not? I couldn't tell if they actually removed the plaque or just put in the mesh thing? Does that stay in your neck?? Is there any other way to get rid of the plaque other then TCAR or the CEA? I guess I am wanting to NOT have these surgeries because I fear they will cause a stroke or shorten my life in some way. But if that isn't something I can do via, diet, exercise, niacin & other things I want or am doing now then I guess it will B the gamble of a TCAR procedure & crossing my fingers. The tough part for me is I am old now ,with many health issues & basically all alone without a trusted support system surrounding me. I believe that is essential for less fear & better healing results.. I also want a 2nd cardiologist's opinion since I showed my heart doc. some things I saw another heart doctor do. He looked briefly at them & said the won't help or do anything. I was hoping he would say sure we can do those tests if U wish! No he seemed annoyed I challenged his knowledge offer suggestions he had not presented to me. I guess that was the God complex & the Big Ego coming out! I will B doing a few more test with him, but am searching for another heart doctor who WILL do those tests I wanted done. Here they R.,1. inflammation panel, 2. OGTT, 3. Quest Lab insulin survey. That was it. What R your thoughts on these added checks?

    Just wondering why they are not Using artificial disk implants when they do a disk removal instead of a fusion. 23 years ago I had a artificial disk implant instead of Fusion, this allowed me to maintain mobility. No screws and rods. And it was the best decision I ever made. But yet here we are in 2023. And there still seem to be doing old techniques.

    If a non LVAD patient was used for this video and/or this person has a PP (palpable pulse) the first sounds heard or more likely related to SBP not MAP

  • @kristigraham667
    @kristigraham66711 ай бұрын

    in this video first sounds were around 110. This would be a critically high MAP for an LVAD patient (no pulses). If the LVAD patient had PP - then once again this would likely correlate more to a SBP

    I went from trace atherosclerosis two years ago to 50% and 70% occlusion in just two years. Doctors are unable to explain how this could have happened. I didn't have any risk factors either. I never smoked drank or did drugs. Low BMI of 19, low cholesterol 127, normal blood sugar, normal blood pressure. They can't operate on it because they are intracranial occlusions. The extra cranial part is clear. All they could do is prescribe anti-platelet medication. They said it could delay but not prevent further strokes. I think I am screwed.

    So what are the target values for doppler MAP in a non-pulsatile patient?

  • @kristigraham667
    @kristigraham66711 ай бұрын

    use facility guidelines - however, LVAD patients MAPs are generally 70-80. No less than 60 and no more than 90. Preload dependent and afterload sensitive.
