Karai didn't trick Leo. She's a deeply disturbed character in a terrifying way. For example, here 0:28, she's attacking Leo with full intent after deceiving him and stealing his sword, trying to strike him down. You can see the power of her grip and the pressure on her sword, along with her sadistic expression, enjoying watching Leo struggle against her blade and shocked that she betrayed him after he saved her. She even mocks his rescue. But her expression after she actually stabbed him 0:39 is a mixture of shock, regret, sadness, and pity. In less than a minute, she shifts from a sadistic character mocking Leo and trying to kill him to a kind, compassionate, and gentle person, showing how quickly she can switch between these extreme emotions.

The fact that Shredder was able to stab him from the back with a fatal wound, but instead decided to make that wound happen by Karai's hands, shows you the truth of his evil, that he wanted Leo to suffer from the stab and the fact that the stabber was Karai.

Kara'is problem is that she has two opposing personalities with equal strength and control over her, and she is not in control of which one appears. She is the kind, honorable, and merciful woman that Leo saw, and at the same time, to the same extent, she is the despicable character who stabs from behind. She literally showcased both personalities here: the evil one who stabbed Leo and the good one who looked at Leo with regret and shock. I believe this truth is something that Leonardo personally couldn't understand about Karai-that she is as bad as she is good, in equal measure.

In this scene, we saw how truly complex and disturbed Karai is. She steals Leo's weapon, kicks him while he's down, jumps on him, and attempts to cut him with his own sword, all the while smiling at him provocatively, mocking him with her words. Then there were her determined looks about deciding to stab Leo; it was evident on her face. As Leo flying towards her, the moment the sword entered his body, she had made up her mind. But after she actually stabbed him, her expression turned to shock, sadness, and sorrow. She returned to her kinder side after realizing that what she wanted wasn't to kill Leo.

I genuinely think Shredder kicked Leo towards Karai's sword for two reasons. First, to test Karai's loyalty after seeing Leo save her. Second, he realized Leo has some sort of attachment to Karai, feelings and trust, and wanted him to feel the pain and agony of being stabbed by her. He understood that a stab from Karai, someone he seemingly has feelings for, would hurt more than if Shredder did it himself. Despite having the opportunity to kill Leo himself, Shredder chose to kick him towards her instead. The way Leo looks at Karai after the stab shows that he's more shocked by the fact that Karai was the one who hurt him than by the pain of the stab itself. He doesn't seem to care about the stab or how it happened, only that it was Karai who did it.

Frankly, I see only two explanations for this scene. The first is that Leonardo still had confidence in Karai’s honor, or the second possibility. And frankly, it is more logical from my point of view in the context. It is that he could not force himself to kill Karai and decided to accept his fate by dying by her hand instead, and that was clear. In his look before killing him, a look of readiness
