60 mg per day of dexamfetamine is a massive dose… 54 mg of MPH / day by osmotic release is a fairly low dose… Why would any non-injecting user of impure, nasty, street meth not stop immediately if provided with safely manufactured dexamfetamine tablets?

There is a feeling in emergency psychiatry that it is hard to jump to lithium right away in our homeless population because they are not eating and drinking and it is incredibly warm in some parts of the country. Can you comment on that? I do do lithium, but I know psychiatrist who are cautious to start it even when labs can be done.

I do not know why they recommend SSRI's for agitation in elderly when one of the side effects of this is that it causes people to be hostile. What has happened to our scientific community in this country? The older medications are far superior than the harmful destructive medications they are producing now. We use to be way more advanced than other countries.

Hey dude, Olanzapine causes people to go maniac on carbs, get diabetes, and so on. I know you're trying your best to help, but the #1 rule is that medicine should do no harm. Since when did medication have the right to hurt us in exchange for something else to get fixed? It's an endless cycle of brokenness, and endless need for pharma drugs, big business, dirty morals.

I have taken the most sporadic fatal insomnia where my brain is awake constantly and does not contain any sleepiness. I am on Zopiclone, Seroquel, Mirtazapine, Meltonin, and I add Megnesium ad Gaba. I get knocked out but the root probelm is not being addressed. I used to be the the complete opposite, needing a lot of sleep. My brain snapped the complete oppostie way in a 3 night period iin Feb of this year. I am so scared out of my witts what lies ahead of me. I have paid for an MRI brain scan but never heard any results.

I can't wait to see this go up against risperidone, olanzapine, and olanzapine. As an inpatient psychiatrist, I probably won't be seeing this or using this with any of my patients any time soon, but hopefully its more effective than our current options and costs come down in the next few years!

My 23 year old son experienced his first episode of psychosis and subsequent diagnosis of schizophrenia 18 months ago with 3 hospitalizations and is presently on 6mg daily of risperidone and 1mg of Rexulti. He is no longer hearing voices but the weight gain, lethargy, and cognitive changes are painful for me to watch. I have read a little about KarXT and felt optimistic but recently his young psychiatrist told us he attended a conference and there was lots of discussion of KarXT and the takeaway was that it only shows some benefit for first time psychosis patients never treated with other antipsychotics and only as an adjunct to other antipsychotics. Needless to say I am a little confused from that and hope he is mistaken because I was hoping that it is improved in September and we could gradually transition him to KarXT with a lower dose of risperidone

Its crazy how well Temazepam works as a hypnotic, no tolerance build up (15 years of 20 mg a night) and I can just stop using it whenever but Its irritating like when I was a kid spending hours too fall asleep. Its stage when people are put on such high doses of the potent ones, Temazepam’s flaw is it takes 30 mins about too start working and it help me read better with my dyslexia. Also people seem too think like 1 beer will kill you on them when a study for sleep was Temazepam and a shot of whisky, like the misinformation on its dangerous is a weird one, people act like its not a safe compound because people could abuse it but like before benzos we were using birbirtuits so far safer now, super difficult too over dose on and the drug deaths from benzo related withdrawal always ends up being person who takes like 900mg alprazolam for 20 days then stop. Its always so extreme with the dosing and length of dosing.

Need a good alliance, over period of time for patients to feel comfortable being honest About their use disorder! Thankyou for the talk!

Not sure about the refusal to start lamotrigine in an inpatient setting. Other faster acting meds are used for more acute symptoms. Antidepressants and meds like lamotrigine will be given as well for long term maintenance. Common sense really. There is more than one drug to give.
