    I don't disagree with you. In 20 years of doing this, I'm confident that I've never cannulated a vessel and injected lidocaine intravascularly. It is one of those things that is recommended by many sources. Perhaps we will get a formal study one day to put the practice to rest!

    I had stitches before, I once had ten in the head and the nurse was nice enough to skip the anesthetic upon request, itès less needles and doesn't itch as much later... Also another nurse walked by and she said look, no anesthetic, she seemed shocked... Trust me it's the way to go for just a stitch.

    I agree. Anesthetic hurts. If it is just 1-2 stitches or staples, I offer this option.

    I had seven of them and it didnèt hurt at all, you could tell you were being sewn up. I am not impervious to pain..@@thelacerationcourse2009

    Since local anaesthetics block ion channels to stop nerve cells from sending out pain signals, the worst case scenario if it's injected into blood vessels is bradyarythmia or the heart stopping (!!) because the ion channels of the cells in the heart get blocked as well! I just learnt this today. So it's important to aspirate and to make sure you don't use toxic doses.

    Yes!! Although the likelihood of cannulating a vessel and this actually happening is so incredible small, its recommended that it be done. I have never seen nor heard of this actually happening in twenty years...

    We do aspiration to check that we don't suck up blood with syringe, which means that the needle is in a vein. If we accidentally inject lidocaine with adrenaline in the vein, it can cause systemic toxicity of lidocaine or adrenaline, if it is too much ofcourse. Or the patiënt will experience central nervous system symptoms like seizure, coma or in serious doses death. And the lidocaine and adrenaline will have an cardiac effect, like arrhythmia

    My dad had a vasectomy in the early 90’s. He got it done and wasn’t too happy about it, waited 30 minutes and started driving home. While driving he felt some discomfort and tugging. It persisted and had to pull over……. The doctor had sown his ball sack to his upper thigh. He had to drive back and have it fixed Very memorable story

    Thank you for the demonstration. Could you use a horrizontal mattress on the more complex part of this wound to have a better approximation?

    For sure-there are always different way to approach and repair wounds. The horizontal mattress is a great stitch and has a lot of applications and it may have been useful here. Good eye and good thought!
