As I understand it, any professional with concerns can fill it in, so they will likely have had some contact with the individual, but that person may not have consented to a formal checklist being completed, or may not have been able to complete it fully. So a professional can complete it with the information they have regarding the individual, even if not a complete picture. I suppose it's a best guess with the information at hand.
