I think I just kept the passengers standing and slowly got them to the hospital

I found the cause. It's AI script bug This does not happen if you disable the default AI option in addon at the start of a new game.

@@shk1931 cool, I thought they were the passengers from hell. It was night and thunderstorm .. all of the sudden I crash in mid-air and the passengers just disappeared. I had to re-load a save several times.
