    I have learned a lot more about SOC information importance. Again, like the communication aspect of the team model. I believe we can do better with this by all involved disciplines.

    Found session to be helpful. Interested in the idea of an education plan for all clinicians. I think that actually goes back to more communication by all involved in the case!

    I like the HEAT model and it will help me to handle questions I sometimes get from my patients. I also was glad to be reminded that I can let the patient know they will receive a survey and it would be appreciated that they fill out. And to let them know what form the survey will be in- phone or paper.

    I am a PT and schedule patient revisits with the patient and or the family at the end of each week for the next week. I follow the schedule closely and call if necessary if I may be late. I am never sure when the SN will be returning but it sounds like I need to look at EMR more closely! As I have noted in previous comments, the biggest thing is communication with all disciplines!

    I was so impressed by the strength of importance of communication! I agree that all need to be in touch with the CGs because they know the patient better than any of us do!

    Very interesting. Glad to learn more about Casper. Liked idea of Care Team- I believe we need to communicate more. Evening/weekend phone calls make sense to me to touch base with patient when don't have a visiting clinician that day for sure.
