    So GPs are suffering like the rest of us with people being brought in who will work cheaper doing the same job , sorry I’m not going to cry over you no longer being able to afford that second home when for twenty years the poor haven’t been able to buy or rent a home. Let’s hope politicians get the same treatment and something might be done for all of our benefit.

    Are GPs struggling to find any job or struggling to find a job that they want? The GP shortage is not uniform across the country. If these unemployed GPs were willing to move somewhere that needs GPs, I'm sure they could find work.

  • @DrSunjayPaul
    @DrSunjayPaulАй бұрын

    That may be the case, but when you have a family and other commitments in one part of the country, it’s not exactly easy to just move. Especially as even employed (salaried) GPs are losing out on their jobs to non-doctors in some cases, so if they move they may not have job security

    I swear this is happening in the US too. I have heard that hospitals are trying to save money by having 1 physician and 5 PAs because their salaries are lower. This is kinda ok, but clearly not enough because it's so hard to get an appointment. The job market sucks massively right now. Physicians and therapists going unemployed when their demand is higher than ever is insane.

    In the US our health system is being flooded by Indian GPs who did their studies in India or through international exchanges in Eastern Europe. Unfortunately they don’t tend to be very good doctors, they’re very cookie cutter in their approach to the body and they lack understanding of American culture, lifestyle and even language. It is difficult to communicate with them and get adequate treatment, so most patients seek out American, European or even Hispanic doctors more often.

    They say it's because they don't want to do more than 3 days per week....the darlings can't mentally handle it fulltime! .but it's the patient's fault, eh? You're talking rubbish.. Not in our GP practice. You see a nurse or a pharmacist if you're lucky. No, never no way more appts than before.

    It is and always will be a pleasure and honor to have learnt and have been educated by Dr. Sunjay Paul as a medical student in 2023. His talks and illustrative and interactive lectures on the functions and complexities of what could arguably be one of, if not the most, complex and underrated systems in the body made falling in love with medicine all the more incredible. Loved the video doctor. Was hoping if you could create a video explaining how autoimmune diseases work and what are the treatments against them?

    We officially enrolled into the "knowledge" age where models use learned knowledge to provide new kind of services.. I think specialities that have heavy reliant on medical knowledge like intrinsic medicine will be the most threatened.. surgeons on the other hand are the hardest to replace because of the physicality (unless there is a robotic revolution after this AI revolution).

  • @DrJohnCosgrove
    @DrJohnCosgrove Жыл бұрын

    The largest element of your list is cost of living whilst at medical school. That is hardly unique to doctors: many people have a 3 year degree course. It might be fairer to look at loss of earnings after the first 3 years of study.

  • @mrkim0325

    I do think that most specialties will definitely be replaced by AI within the next decade. But as doctors get replaced, AI will still need research and clinical intelligence by actually doctors to make it smarter. I believe as AI slowly replaces specialist, big corporates will slowly hire doctors to help it get smarter. It’ll be a win win situation in my opinion. Companies already hiring retired doctors as medical consultant to help out big biotech projects. And pay them big money.

  • @DrSunjayPaul

    @@Aziz0938 yeah but its happening. Amazon just spent billions to buy out OneClinic, and to make their own pharmacy recently. They are planning to fill in the shortage of PCP and Pediatrics. OneClinic will be an AI operated, no physician, clinic. Nurses will just take orders from the AI. Everything done via online. Amazon pharmacy working with drone delivery with Rx. Soon going to PCP will be super inconvenient. Plus every year more and more people can’t even afford basic medical care. If PCP AI is more affordable, no one will care to really see a PCP. Specialists and Surgeons will keep their positions. But PCP will definitely fade away soon and fill the gap of the shortage of doctors we have atm. Still along way too go, but remember, AI is growing exponentially every 6 hours. By the end of the year it’s going to be very 2hrs. I work as a SWE for Amazon healthcare tech project btw.

    Hi Dr Sunjay I recently found your channel and must say I really enjoy the informed , well presented and calculated videos you make and im quite shocked you aren't more popular. but I have some questions for you, you mentioned how stressful and time consuming being a doctor is and "personal costs" which has worried me. Because I personally want to become a GP but my family and future relationship/children would be much more important to me than the job I would also like to use my doctors salary/expertise to start businesses and other ventures. I plan on worked a very "relaxed" contract for most my time as a GP because of this except for maybe the first few years when I would be trying to invest and work on projects and gain experience. I have recently watched many videos talking about the flexibility of GP doctors that they can basically ask for any amount of workload (within reason) for a contract and negotiate a salary from their as they are in such demand. And I've watched videos about Locum docs and how they can get paid very well without really needing to do an equal amount of extra work so I want to ask you if I prioritised my personal life and only worked contracts with the amount of hours im comfortable with would this really still be much of an issue? because I feel this is only an issue for doctors that try to work as much as they can because they are very passionate and that if you reduced your overtime work and worked a much lower hour contract then you wouldn't face such a problem. Also I somewhat feel that a GP is the least stressful type of doctor at least the least stressful that I am interested in doing but could you please elaborate on the stressful parts of the job and how to deal/avoid them.

  • @DrSunjayPaul
    @DrSunjayPaul Жыл бұрын

    Thank you so much for your kind comment, I really appreciate it 😁 Thanks for your questions, I think I might actually make a video on this. But here are my thoughts for now. I wouldn’t say working as a GP is the least stressful specialism as a doctor! So if you want something with less stress, I would consider public health or occupational health. However you are right in that working as a GP gives you the most flexibility compared to other medical specialties. I don’t think you would realistically be able to negotiate a Salaried GP contract with minimal additional workload - that will always be expected of you. However you are right that working as a Locum GP generally includes less additional work, so you would usually just see your patients and do the referral letters for them, but wouldn’t be expected to check additional lab results or hospital letters for any other patients. The stress of working as a GP comes from the heavy workload - lots of patients with increasing complex needs, demanding more investigations and referrals. Our work is also becoming riskier as patients have longer to wait for hospital appointments so patients end up coming back to us for advice on their condition which would ordinarily be managed by a specialist. Instead of a workday lasting from 9-5, it often ends up from 8-8. This is why lots of GPs work part time, doing 2-3 days per week, as it would be far too stressful and time consuming to do almost 12 hour shifts every day. I have found that the most flexible option is to work either in an urgent care centre, where your hours are fixed, or with a remote/tele-health provider where you hours are also usually fixed. You can do this as a Locum as well, so it gives you the flexibility to work when you like. Feel free to message me on Instagram @drsunjaypaul if you have any further questions.

    Hi there!! I'm interested in internal medicine training in the UK. I have a question, after internal medicine training can those who are on a work visa take a few months break like 3-4 months before joining our desired speciality/fellowship (like cardiology, hematology, neurology) or is it that only British citizens can take a break of a few months after internal medicine training?

  • @DrSunjayPaul

    It depends where you are from and the work visa you have. Unfortunately for the vast majority of people coming from abroad, this wouldn’t be possible.

    I am a retired hospital ophthalmologist but I have a doctor son and I watch videos to try to understand what pressures he is under and how I can help him so I would encourage you to keep making these useful and practical videos, sunjay! It’s spot on when you say doctors live in a capitalist world( with matching aspirations) but work in a socialised health system and I should point out that it would help young docs to think less of the money and more about the other rewards of doing medicine as a profession

    I have heard that banding can boost income to 30-40% of the base salary but in this video you mentioned that the boost is of 5-10%. could you please elaborate that what the actual boost is after banding plus how much can a junior doctor make through locuming?

  • @DrSunjayPaul
    @DrSunjayPaul2 жыл бұрын

    I’m not sure where you got the 30-40% figure from but as explained in the video, the exact amount of banding varies. This document by the BMA has the full breakdown if you’re interested: www.nhsemployers.org/sites/default/files/2022-03/Pay%20and%20Conditions%20Circular%20%28MD%29%201-2022.pdf As you can see, the highest banding would be up to 15% if you were working 1 in 2 weekends. As for locums, I will make a separate video on that soon!
