    B1 thiamine deficiency is common here! Of course a couple others play a part in the autonomic nervous system too, like magnesium and zinc or vitD. Some of these WON'T often show abnormal levels, since serum only contains 1% of total body stores, so we often times dismiss and say "YOUR LABS ALL CAME BACK NORMAL". THAT IS, IF WE EVEN TESTED ALL THESE

    Ive been dealing with thee symptoms sincerughly 2010 2011 when i had my pregnancy. Life had a way of making it very difficult to get treated for anything a year or 2 after this. When i was able to get seen by a dr about 5 years later when i was fainting, intense fatigue, heart beating fast and occasionally hard and lightheadedness they just diagnosed my crashing (possibly fainting episodes) as psychogenic non epileptic seizures. Basically a seizure thats caused by emotional distress. They later also diagnosed me with anxiety and asthma. The therapy for the seizures did nothing to help. I just came from a drs office explaining my symptoms and im finally gettng seen by a cardiologist to see whats actually going on.

    Thanks for the clear, concise intro. I read Dale Dubin's basic book, but am now interested in a textbook that students don't like because it is vast, too informative, and contains extra technical and clinical info. Which one is that? I have considered Mariott's. Any recommendations? Appreciate it -

    One of my motivations for this series was the fact that such a book doesn't exist, at least not a great one. Marriot is fine, but not awesome. Wang's Atlas of Electrocardiography is a decent reference for a budding cardiologist, but as the title suggests, it teaches only through examples without ever giving broad summaries of a topic. Also, there is a huge leap between Dubin and Wang that might make Wang feel inaccessible. I think the closest a book has come to being an intermediate to advanced ECG textbook was Phibbs' Advanced ECG: Boards and Beyond, but it's pretty dated at this point. It also gets into the weeds with things that may be academically interesting but of near zero clinical relevance. Phibbs' reminds me of Jules Constant's Bedside Cardiology and Sapira's Art and Science of Bedside Diagnosis - fascinating books from an earlier era that today are better as a bedtime read than as something that will teach you modern medicine.

    @@StrongMed Thanks for the reply! Maybe I'll just get Marriots (it has a workbook) and then keep my eye out. AI will soon be changing diagnostics, but it's not really reliable - at least yet.

    What would be a result if the EGD was normal and the barium study was abnormal but the p has dysphagia & odynophagia to both liquids and soilds intermittently. Would a physician recommend a manometry or would they find a motility disorder from barium study?

    I've been trying to get a diagnosis for quite some time now. It is quite difficult because doctors and such seem to be working against me on that. I think/hope it's cfs, because what is left on the list to check for is somehow worse. At any rate, where I'm at right now is that i can no longer work, and social security people are now forcing me to join an exercise program (i love working out, and would really like it to work like that. However i sometimes crash from a walk, and i doubt the extra stress helps me with that). I just wanted to stand in line to get in where the doctor ive been reccomended by someone with similar problems work at, because i know a lot of doctors seem to pretend this group of illnesses doesn't exist. I've been almost homebound since last autumn due to arthritis advancing and last winder being very wet, and i had to stop working halftime last spring. For the record I'd much rather work and not have all this quarreling and so on, however i recognise that it's not an option.

    Here in England I have recently been told to there is an extremely long waiting list for any referral for such problems There has gotta be a reason for this other than just blaming people’s poor diet like they are so quick to do I personally am being fobbed off like you wouldn’t believe & my symptoms are so bad I can no longer eat without the pain getting so bad Wasting away right now 😩

    12:31 in Chinese we call “(to be) pimped” as “(getting) electrocuted (被)電”. I and a few fellow Chinese classmates have had the pleasure of being “electrocuted until we exploded (電爆).” This doesn’t have to happen in med school though; it applies to generally getting grilled by your instructor on minute details, regardless of the setting/ subject.

    Sure, missing the obvious lung cancer diagnosis is bad, but trying to rhyme "study" with "physiology" is unforgivable.

    What if there if i dont smoke any weed, and i also gwt abdominal pain EVERY TIME i get my nausea and vomiting and wretching. Theres always lots of wretching.? My stomach pain feels like Labor pains like a contraction and i feel compelled to walk around to distract myself. But i want to emphasize i am NOT pregnant.

    Just got doagnosed with eds, i belive typical but the orthopedic only seen me for like five minutes. Its crappy because i have high tendencys to dislocate limbs. I also suffer from pain, some days are better then others. Atm i am 18 and after years of being labeled double jointed or, "growing pains" i finally got someone to listen to me. Now we just have to get genetic testibg and my heart checked.

    at the example that read normal ekg at 5:20, i am seeing pathological q waves in v1 and AVL and inverted t wave in avl, could that be related to past mi? also i am hesitant that this patient has wpw phenomenon since the the beginning of qrs has a wierd upstroke provied the closeness of p waves to qrses!!

    first example, why couldn't it be third degree block! i am seeing a complete dissociation between the sinus at a rate of 75 and the qrs at a rate of around 40. is it because the qrs is narrow? at the same time it could be a 2:1 2nd degree AV block

    I’ve had severe abdominal pain N/V x 6 weeks despite max dose of PPI. Every test is normal. Getting MRI and repeat EGD next week. GI doc sent me to the Emergency Dept today for another CT scan. I’ve lost 15 pounds and am existing on drinking Boost. No meds relieve the pain or nausea. I’m still in my 50s but ready for hospice.

    I'm sorry to hear about your illness and symptoms. I hope this video suggested some new diagnoses that are worth exploring with your doctors. While I can't offer specific, individualized medical advice here, I will say that - in general - if a person has abdominal pain and nausea/vomiting, and their EGD is unremarkable, a PPI is not going to be helpful. Once again, this is not intended as personal medical advice, but for patients I've treated who have refractory nausea and vomiting, most have some improvement with a med called aprepitant; unfortunately in the US it's very expensive ($30-$150/pill) and insurance companies will try to find excuses to not pay for it.

    Doctors are just regular people. They could have any number of chronic health conditions beyond their control. Like someone in the comments mentioned having RA. I also appreciate this message. I have chronic GI motility issues due to muscle problems. It's led to so many downstream problems (being underweight, malnourished, muscle wasting, etc.) It's very discouraging when people look at me and assume I don't put effort into my health. I can't force my muscles to work properly. No amount of exercise, diet changes, etc. have any impact on it. I am incredibly diligent about my health, meet regularly with my doctors to manage things, etc. I just got a little unlucky. On the flipside to all of this, someone could live a very unhealthy lifestyle, but it doesn't mean they don't have useful knowledge to share. Doesn't even have to be due to external factors - someone could simply not value their own life or health for philosophical reasons. Who am I to judge? Doesn't mean they aren't educated and have valuable information to share. You never know what's going on in another person's life! I go to the doctor for their medical knowledge, not to judge them.

    Very peculiar stories. It'd be hard to imagine that people would be compelled to vote for a candidate who is poisoned with mercury and had brain parasites. I wonder what the real reasons for these stories are. On another topic: I've been watching the series on mechanical ventilation. As always, great lectures! Though I couldn't find the twelfth lecture, which is supposed to cover troubleshooting vents, and how to approach the ventilated patient in acute respiratory distress. Is this lecture missing?

    Probably Swordfish or other high mercury fish. Also high in mercury but expensive enough that non-anglers don't eat so much.

    It's true that swordfish has much more mercury than tuna, but FWIW, he himself has attributed it to tuna.
