Cool prep, I think about doing this occasionally, seems like leaving the palatal is a good service to premolars that seem to be prone to fracture at the gum line on full crown preps

I appreciate your question! Just case reports so far, but it's a prep that we've been performing at the UCLA Center for Esthetic Dentistry for about 6 years with great success - Ed McLaren lead the push initially and we have followed the principles known for veneers and ceramic onlays, and combined the two. There is definitely a need to perform a Randomized Clinical Trial but I'm not aware of any underway as of January 2020. Thank you.

What would be the appropriate provisional for this prep ?

I use a Bis-Acryl provisional made from a dx wax-up and Duration cement.

Hi Dr. Stevenson. I've done many of these restorations with great success & your videos have helped a lot. I do have a question though. If I am doing a (same day) CEREC restoration and I have proper isolation under rubber dam and completed my IDS, would there be a difference in subsequent bond strength if I were to etch and bond the enamel ring before or after milling the restoration? It's a ~35min difference between the two.

No difference at all. The more you can get done while it's milling the better. I've been a Cerec guy for 20 years and just took the plunge with a PM1 - wish me luck! Best to you, Rich

Hi Dr. Stevenson, thank you for all your videos! This is my first year as a dentist and i am wondering- how is it possible to use diamond burr without a water-cooling? Isn't it dangerous for a pulp? Thank you very much for an answer.

These were done on extracted teeth and I turn the water off to allow better filming. Having said that, pupal temperature studies are old and poorly done. The make generalizations and recommendations that do not equate with clinical findings. I ALWAYS turn off the water when finishing my preps.

Oh, that would be possible as well - there are an unlimited amount of variations possible. This was one.

Normally if deep, I'd use RMGI, but this day I reached for the flowable simply to block out the defects and provide for a smoother wall. Not needed, but nice to do for the lab - they make better restorations with super smooth preps...Thank you Doctor.

nice, sir.how are you going to bond the zirconia to the tooth??? why zirconia and not emax? also on the provisional stage how are you going to prevent strong bonding of provisional material such as TELIO by ivoclar to the immidiate dentine sealing layer? best regards

I did a quad in e.max in a previous video, and since Zirconia is so confusing to docs, I thought I'd show how it can work in certain cases. Preps must be more retentive and the zirconia APC bonding protocol must be employed. After IDS, I place glycerin over the adhesive to lessen the air-inhibited layer and minimize the provisional from binding, then use Duralon cement. Thank you for the thoughtful question!

Coming in the subsequent part, however, a preview: APC - air abrade, primer (MDP) and composite cement. Chemical bond, not micro-mechanical - will it last? The work from Marcus Blatz says yes... more studies will help us for sure.
