In this video, Dr. Praveen Kammar (Consultant Surgical Oncologist, SSO Hospital, Mumbai) will tell us about the surgeries for intrahepatic cholangiocarcinoma.
There are 3 major types of treatments when it comes to bile duct cancer according to the stage of cancer -
Surgery
Chemotherapy
Radiation therapy
Bile duct cancer can be formed in various regions such as in the liver, bile duct, etc. The treatment for bile duct cancer depends on the stage and the area where the cancer is spread.
Surgery of bile duct cancer which has spread to the liver -
It is called intrahepatic cholangiocarcinoma (ICC). This surgery requires removing the affected part of the liver. Before surgery doctors check the overall health of the patient, doctors suggest biliary decompression for jaundice-affected patients. Liver resection depends on how much percentage of the liver is affected, some require right hepatectomy or left hepatectomy.
To confirm whether the remaining liver is good to perform the function of the liver doctors check -
Liver condition, Liver function, Liver related disease, Portal Hypertension.
If it is found that the liver is not in good shape then they require non-anatomical resection to be performed.

ULTRASOUND OF WHOLE ABDOMEN Suboptimal scan- poor acoustic window due to poor breath holding. LIVER is normal in size, outline and echopattern. No evidence of any abscess or focal mass lesion is seen. No evidence of I.H.B.R.D. Portal vein is normal in course & calibre. Marked periportal cuffing noted. GALL BLADDER is absent-post cholecystectomy status. Common bile duct: Calculus of size 13 X 6 mm is noted in distal CBD near ampulla causing upstream mild dilatation of CBD, CHD and central IHBRS. CBD measures 11.4 mm in caliber at porta. Mild echogenic dependent debris noted in CBD lumen. SPLEEN is normal in size, outline and echopattern. No focal SOL noted. PANCREAS shows normal outline and echopattern. No focal SOL noted. RIGHT KIDNEY is normal in size (10.4 X 3.6 cm), outline and raised cortical echogenicity with maintained CMD. PCS is compact. No calculi seen. LEFT KIDNEY measures (10.8 X 3.6 cm) in size with raised cortical echogenicity with maintained CMD. No calculi seen. Gross dilatation and ballooning of renal pelvis, (maximum AP diameter ~4.8 cm) with abrupt tapering of the PUJ. middle calyces are mildly dilated. Exophytic cystic structure of size 48 X 25 mm noted at interpolar region-?bifid pelvis ? exophytic cyst. Cortical cyst of size 21 X 14 mm noted at upper pole and few subcentimetric cysts at interpolar region. No evidence of ascites. URINARY BLADDER is empty. Gaseous abdomen with obscuring of mesentery. IMPRESSION: CHOLEDOCOLITHIASIS WITH UPSTREAM MILD DILATATION OF BILIARY TREE. MARKED INTRAHEPATIC PERIPORTAL CUFFING. > POSSIBILITY OF LEFT SIDED PUJ OBSTRUCTION WITH ?BIFID PELVIS? EXOPHYTIC CYST.
