I have a noob question if you don't mind: 1. What is your build? I too am using a Medium armour set, and I tried to revolve it around signs, but apparently, not as effectively. Great video, you make it look easy.

I had the basic red combat skills that increases damage on fast/strong attack, damage reduction on roll, etc. I had around 4 blue sign skills which were pretty much irrelevant for the fight. I had the alchemy skill that gives you higher toxicity tolerance the more recipes you have unlocked, and the medium armour skill. The Grandmaster armour skill that lets you double cast signs coupled with good stamina regen from the medium armour skill, stamina regen bonuses for having blue sign skills equipped, and Superior Tawny Owl should give you more than enough stamina regen to spam Quen as needed, which is really the only sign you will use in this fight. All of that is overkill, really. The Grandmaster armour skill alone is sufficient. You can use Igni if you want, but almost all of your damage will come from your sword.
