It's getting better everyday and can be found in every sector of industry. The thinking you currently have is like looking at the old clunky computers that would take up an entire room and say "computers aren't going to improve in 50 years..." Also gonna add that not only does this get better, faster and more accurate with more tech advancements and more data but it also gets a lot cheaper (want an example? Compare the very first cell phone when it first came out the one gordon gecko used in wallstreet I believe they said that would be about $50,000 in todays economy, compare that with what you buy right now.)

Human beings dont like the fact they went through all that schooling, and yet what they have to offer is artificially relevant, because in dismissing their services, you incite instability. There will approach a breaking point when the inefficience of this sort of meandering economically, might spark something, we have all the business in the world becoming concerned about. Why do you think the military budget has exploded to 700 billion dollars per anum? Funsies?

@@lpfan540 This is a brief reminder the cybertruck windows stopped the steel ball bearing, and the imperfection of a prodigy make him all the more likable, in context. Marketing genius like this doesnt come to us lightly, and so we must soak in all its immediate lessons as a society. How we, including myself, laughed at his supposed blunder, and how best as a billionaire, to distance oneself from the easily assumed elitist personality that comes with said territory. Masterful indeed. One day ill have to get me one of those damn trucks.

My mentor who's in private practice was literally begging for an A.I. to come and help him with the insane workload. Realistically, though, radiologists are working like dogs because there are so many images to read. Trend is that radiologists become efficient, and docs in other fields become happy and send in even more images. They NEED A.I. to survive right now lol. Everyone who say rads can be replaced are software engineers that have no idea what radiology really is. They think it's only reading images from a computer

All the private practice radiologists do all day is read images. To act like ai won't be able to give a correct read in pretty much every case would be delusional. Will it take years, decades or centuries? Who knows, but it's going to get there faster rather than slow.

The question is just same like 'Can Google search replace General Medicine physician' 😂😂

I’m a med student still and thinking about the future, I would like to take up diagnostic Radiology even though threatening from AI... I don’t think to take up interventional radiology because I FEAR FROM RADIATION EXPOSURE AND GETTING CANCERS!! You guys don’t fear it? Please let me know if it’s not a thing because you are experienced ones...

Secret Medico it’s a very protective environment. While there is a risk there’s a detector on your lead suit that lets you know how much radiation you’re exposed to and there’s many other methods

​@@Tonixxythis is the problem, they always defend their over optimism with you know nothing about radiology blah blah stuff, but in reality they have no idea how neural networks work, AI or even AI engineers don't really need to know anything about the problem they're working on, all you need is sufficient and good quality data, once you train it well enough, AI will do anything with better accuracy than humans, another argument is AI will never be 100% accurate, well human diagnosis can't be 100% accurate either, I don't know statistics but am sure AI will still do better than humans in many areas and it's power in image processing has already been just unbelievable. So i dont see why many of these professions will be reduntant in a few years.

This has always been on my mind and the reason why I have hesitated making this my career.

Although you addressed that there is an increasing demand on radiologists, do you think that the efficiency potentially created from AI would, in the long run, decrease the demand for radiologists? i.e. will hospitals need to hire fewer of them, given that fewer radiologists would be able to theoretically crank out more work?

Planning to take up radiology in residency. I wish that you made a video on how to approach radiology residency... What books to read... How to approach cases etc.

I did research in designing a program to differentiate primary liver tumors on triple phase CT, and although my PI (diagnostic radiologist) was skeptical about the usability of AI in radiology, he was soon convinced on its usefulness to radiologists as a secondary aide to let them do their work more efficiently

Nice video. A topic that’s been in mind that I’d love for you to cover is radiation dose. Why are so many exams ordered it’s like the docs don’t think twice about pt dose.

First things first, love your videos. You are a great inspiration for us medical students. Here is the question: Since reading studies is much about identifying patterns, don't you think AI will beat radiologists in the accuracy, by the machine learning process? It is happening in mathematics and computer science for decades... If it happen, I still think radiologists will be very valuable in procedures and dynamic studies, such as ultrasound. However, don't you think it can decrease the number of radiologists needed in a hospital? Because the study reading, at least the everyday ICU chest x-rays, will be done by AI, so the only task to the radiologist will be the dynamic stuff or procedure? If it happen, maybe doctors who are thinking about doing residency in radiology should care about..? Another question: do you think there is a possibility of hospitals hire poorer countries radiologists, who charge less per hour, to read studies for less money, since you acess studies any place in the world? It may sound silly, but I really was wondering about it...

Dr. Cellini, I am wondering about the brand new field of RRA, or registered radiologist assistants. It's similar to a PA for radiology except they are much more trained in certain radiology procedures. I am an economics bachelors and a Radiologic technologist and radiation therapist. But I would like to go back to school to be a mid level practitioner. I love radiology and want to what you think of the RRAs. Thank you for your time. - William

What makes you think AI won't replace ALL doctors, not just reading radiology scans, everything. For some reason people always think "their" jobs are somehow safe, and that only factory workers or cashiers jobs are at risk for automation. If it doesn't happen in 10 years, it'll just happen in 20 years, if not then, then 30 years. AI will only get more and more accurate and able to learn which things to mark as relevant and which to not, etc,etc.

They usually use that argument that you know nothing about our field you know nothing about radiology but the truth is they have no idea about how AI and neural networks work, AI doesn't need to know anything about the field of study, all it relies on is probability distribution based on inherent patterns between input and output values, it's not pretty logical approach like conventionally humans perform their tasks, but it does work like magic, it all depends on how well you're able to train the model, if you have enough data and you tune in the hyperparameters well enough, AI can just do anything, I know how powerful computer vision has been we built a model to identify microbial colonies from image samples which we humans barely are able to do and it does it with tremendous accuracy.

Nice video! Well laid dout facts there too. I'm thinking of venturing into imaging informatics. I'm told it holds the potential of being able to make "histologic" diagnoses of tumours on scans. Currently I think they're stacking up data on pulmonary nodules and correlating them to the eventual histology and clinical outcomes. I think it's really promising! But no, AI CANNOT REPLACE RADIOLOGISTS. Well done!

This is already happening. Software called ChexNet outperformed 50 doctors with regards to reading chest x-rays at Stanford University symposium. The traditional role of radiologists will change rather than being obsolete, which is the opinion of Geoffrey Hinton, the apparent godfather of AI deep learning

So the AI was marginally better at picking out pneumonia on otherwise normal appearing chest X-rays. Not impressed. Look at my expectation vs reality video and if they can accurately analyze those CXRs, I will be impressed. Also, AI can have 100% of my chest X-rays

My wife is a DR resident and this debate has always freaked me out. I just know how hard she’s worked to get to where she is, and for AI to jeopardize her future scared me. Your outlook has put me at ease. Thank you.

This video is very valuable and answers an important question for newly graduated doctors. But there's other important question that may deserve to be discussed : what do you think about the idea of working from home as a radiologist? For example can we work as a radiologist while we are enjoying in a hotel in Maldives or Bali haha?

What’s your impression on DACBR?

I think that in the future AI will be making the diagnoses of CT,MRI etc. because they will have a database of millions of CTs and MRIs.So radiologists practically won’t be needed for diagnosis. But interventional radiology ist definitely here to stay!

Is it easy for people to do interventional and diagnostic radiology?

As a Radiologic Technologist, I’m curious to know what Radiologist think Technologist could improve on. The Radiologist at my hospital always give us a hard time for what the doctor has ordered. Obviously we know most of the times the ER doctor is trying to “cover their bases” and the exams are a bit much but if we question the ER doctor then they get upset and we have to do the exams regardless. Then the Radiologist gets upset with us (techs) for doing the exams.

Oh, thats sad. I am radiologist and in my hospital no case done without discussing the case with the radiologist. Our philosophy the tech job to take the best image he can take not to make decision in cases.

The average for 2020 was 246. But another way to do it is go into diagnostic radiology and then do a IR fellowship. The average for DR was 240.

Thats certainly a significant point. Nor will radiologists and their lobbyists sit by and be replaced. AI will help, not replace, as he said. People way overstate the whole AI thing.

What are the effective questions that AI can answer to help radiologists? On what images modalities? Please give some clinical insight.

One thing I don't get is this telemedicine stuff, either. If you think you have rhinitis or an upper respiratory infection. What do you do get your Doc and triage to get your O2, bloodpressure and listen to your lungs, heart, lymph nodes and check for edema and all, over the telescreen? Makes no sense.

I think you have a point hospitals are pretty cheap (for the most part) and they would rather have faster scans and better scans to detect something than replace radiology because if this ''AI'' is broken it will cost them an arm and a leg to replace the piece or whatever the case maybe. I highly doubt it but again I do see more improvements in scanning and better detection. along with surgeons and many healthcare professions which will relay on AI's. well.. not AI's but programs to provide better information and location of some sorts.

not really because to be a radiologist youd need to attend med school so youd need to take many bio and chem courses.

Thanks for the video. So have you heard of Andrew Yang? Do you think he’s being disingenuous with his comments about AI and Radiology? I’ve been following him for a while and was curious to what Radiologists think.

I've been to ECR 2019 as a student radiographer and AI is near the peak of the hype graph so the hype will slow down rather quick. Also the biggest challenge in AI overall is getting data (why Google uses some photo tests) and for healthcare there is a move to bigdata but most programmes are trained on Chinese data what maybe isn't benifitial for the Western world where the demographics, healthstyles and pathologies may be different. That said AI can play a supporting role (like it was doing now) with CADx and support on workflow (with for example augmented intelligence from AGFA). Also the way it can be used to evaluate MS more precise and quantified, get results from the raw data (with even being trained to undo fold over artifacts on MRI scans) and being trained in the search for new biomarkers (like in the Rotterdam study) can help the field of medical analysis.

Agree with this. AI will help us become more efficient and accurate. It just cannot 100% replace us...yet :)

I personally would not mind this shift whatsoever - as I was sick with pituitary sourced ACTH dependent Cushings Disease for 16 years before I self diagnosed. Even then, after 8-12x normal late night cortisol AND two MRIs showing MULTIPLE adenomas - 1 micro, 1 macro invading sphenoid sinus. Yet my radiology report was unremarkable. I find this very terrifying, as my tumors are VERY obvious, no need to search! The medical community is largely inept - and that’s also terrifying and got to be a huge burden on the many great physicians out there

It will replace some radiologists because you won't need as many. It is inevitable but before AI or machine learning comes for you, it will come for pathologists

You seriously underestimate the increase of the power of technology over time. I think it is indisputable that ai will replace Radiologists (and pathologists and most jobs eventually). This is but a matter of time. However i do think, that we still have 1-2 generations of radiologists left, even though technology should be at a point where it is better than a human (in recognization *and* advice of further proceedures) in about 3 decades.

It would be interesting to go back and check on the comments from time to time. Honestly, I don't think it would take 3 decades for AI to take over radiology. Its primary function is just pattern recognition and database search. Both of these functions, a computer, let alone a supercomputer, can do so much better than human.

As a radiologic technologist I would like to get your thoughts and opinions on NPs and PAs encroachment to perform, read, and interpret radiologic images/exams. I think it’s ridiculous. They want to be the tech AND radiologist. It’s happening right now.. thoughts? Love your videos!!

It definitely is happening as we speak, and I think it is actually a good thing for Radiologists (not sure how it will affect technologists). They will perform tasks for rads that usually get in the way of attacking the list...which is a good thing. Also, they will never be trained to read complicated studies. There is a reason it takes 10yrs to become a radiologist 😬. They may be able to read CXRs or KUBs...but I think most radiologists will be thankful for that...

Dr. Cellini that’s a very interesting point of view. But don’t you think the risk of misdiagnosis will increase? Radiation exposure will increase? Most people who think they can perform or read exams no nothing about the importance of radiation safety and accurate assessments. Me and you don’t want to be out of a job and frankly I’m quite scared about this issue. Also thank you so much for you replying!

Yes! I do think there will be misdiagnoses and increased radiation if Board Certified Radiologists aren't reading studies and if trained Radiology technologists aren't performing the studies. I guess it depends on the training of the RRAs/PAs. I am not well-read on their training though since it's relatively new.

IR is love

Reading images, processing data, answering questions; These are all things AI is excelling at. The field of radiology is definitely going to change. Instead of saying "replace radiologists", I think "displace" may be a more accurate term. I think many current radiology positions will be made redundant, but the radiologists will largely be employed in different roles adapted to the new AI based landscape. Performing procedures will be a challenge further down the line for AI to tackle. The employment landscape for these people will be less affected for a longer period of time.

An alumni from my medical school came to us and discouraged us from pursuing Radiology because according to him they are outsourcing radiology jobs to India. Your thoughts on this??

It's against the law to let people outside US soil to make final diagnosis. Doesn't save money at all if you need people in US to make final diagnosis on images even if you outsource initial diagnosis. People thought about it at first, but it's been banned

*☼ plain-film point of care clinical radiology robot* bc PPOCCRR is a nice acronym (like poker, w/ a stutter). A plain film live-in nursing home robot, *who trains on the data on that PERSONAL pts past scans* - that could be done. probs want a human in the loop to sign off it. but you could even have the robot give the pt a preliminary verbal report. & most importantly: soulless eldercare insurance & housing facilities would go for it! *omg: someone get me the magnetron out of the microwave and some duct tape: daddy smells a **-grift-** novel pt care opportunity!*

Let me finish your sentence... ', yet'. ALL work will be replaced by automation someday. I went to a lecture at CERN where top Physicists debated 'when' this will happen, not 'if'. Most agree that in approx. 50-75 years, all humans in first world Countries will no longer work for a living.

The problem is the assumption that computer engineering remains static...it doesn't...it doesn't...it moves far faster than medical science does...we are looking at the first exascale computers coming on-line in 2021...and there's neuromorphic computing...which is an attempt to model the human brain not just functionally but structurally as well...the first neuromorphic supercomputer just went online in the UK...its relatively small because we are just learning how to build these things...remember the Cray 1 supercomputer seemed amazing back in the 1980s and had less power and could do less than a $100 smartphone...🤷‍♂️ you will probably be unaffected by the new technology...but what about the new kids just starting? What if in 30 years neuromorphic systems allow one radiologist to do the work of 100? Or a rad tech to do the work of a radiologist? As a computer geek with meh programming skills I don't claim to know the future...but I'd be at least concerned...🤷‍♂️ for my profession...I'm a psych nurse...and I'm concerned...🤔🤷‍♂️

Yeah they should replace those engineers as well. AI can code faster without errors. Also, it will be cheaper for the company since you won’t have to provide an office.

Blythe Saunders AI can write code faster, but it isn’t always understandable. Software engineers will still be needed to shadow the AI and make sure it’s code is efficient and doing what it’s supposed to do.

Blythe Saunders in the same sense radiologists will still be required with AI, because AI isn’t always 100% perfect like it’s made out to be. Right now it’s a tool not a solution.

@@blythesaunders3124 the scope of engineers/scientists is near infinite, they will be solving problems from space travel to nuclear fusion to robotics where as radiology is much more limited from as an overall discipline. Although one day in the far future all humans will be inferior it is likely radiology will go before scientists, mathematicians and engineers

I don't agree- Ai and computing speed is 2x every 3yrs - every thing comes down to money The best case scenario is # of jobs available will decrease the worst case scenario is inevitable and just a matter of time Dr Watson has already been to med school doesn't sleep doesn't call in sick doesn't have mood swings & is now past Version 2.0 This shift is not unique to Imaging but imaging is easiest to replace

Imaging is only part of a radiologists role. This is why I say we aren’t going anywhere....yet. Also, let me know when they can read an ekg correctly...

I agree with IBM Carry. Imaging studies is going to be greatly improved with the help of AI within a decade. The requirement of radiologist in imaging is going to be reduced. Though prospects are good currently with interventional radiology but that too shall be replaced by AI later.
