Real reason? Author added Sara on the light novel and not in the webnovel. The webnovel is the first draft of MT. But they needed some down time in the light novel before doing the school arc. So Sara was born.

Technically Sara was mentioned (but not named) in the Web Novel when he reflected on a bad experience where he tried and failed to sleep with an adventurer. The LN took that and fleshed it out, gave Sara a backstory and characterization. Just pointing out that it wasn't that she didn't exist in the WN, but she was just a nameless extra in it.

I think she did make an appearance later in the webnovel. If I remember correctly when Rudy was an adult they reconnected and she worked for him occasionally as a midwife or babysitter. I think she had children of her own or something like that. Although I'm not sure if it was Sarah or the other woman in Counter Arrow.

There was a Short moment when sara met after rudy was married . Theres also a alternate novel for sara somewhere as a bonus chapter (no clue about this , i only read that there was such a bonus chapter and i assume its still in japanese )

I feel like adding people like Sara, Linia and Pursena would just cheapen his romantic bonds. Edit : I just watched the last episode of Season 2 this year. I was pleasantly surprised by how understanding and caring Sylphy was about Rudy's problems. I guess Sylphy was the girl he needed most in his hour of ED packed turmoil. It wasn't Sara's fault but she and Rudy just weren't meant to be.

She did fall in love but that's only temporary. Rudeus basically went all by himself to save her, it is natural that she fell for him in the heat of moment. In fact, she was heartbroken when she found out Rudeus' thing didn't react to her advances at all

Sara did fall in love with Rudeus but the anime didn't portray is well. She was going to use the excuse of owing Rudeus her life in order to take him to bed. Only then revealing her true feelings. There's a saying that goes along the lines of 'Men use love to have sex, and women use sex to have love'. While not entirely true, I think that it applies well to Sara's feelings toward Rudy

In the end, they just weren't meant to be together, but it taught both of them a valuable lesson about communication. It's a realistic character interaction. Who knows how many missed connections we accrue throughout life due to bad communication.

Update. In a special promotions of Light nofel of Redundancy. 4 Mangaka of Mushoku tensei. Created 4 additional stories. One of them is Sara and Rudy meeting each other again. Sara is already married and satisfied. Both of them ended in a good conclusion.

She also made his situation worse, and didn't try to understand what he was going through.

Sara made the ED moment about her rather than trying to understand him. She doesn't deserve him.

It’s deeper than that bro. It was a miscommunication, and neither were at fault. She isn’t a mind reader, from her perspective this guy she had feelings for and made a move on her after they are naked and in bed couldn’t get it up, and then sank into silence and like shame and self pity because ofc for him it wasn’t about her all she thought was that it was because he wasn’t into her, and so to save face she said what she said. She didn’t know he spent the night drinking and wallowing in embarrassment and misery about it, and despite that miscommunication she still went to again talk and see him the next morning, when again from her perspective she comes across him saying everything that essentially reaffirmed the belief she had the night before…and ofc she again isn’t an omniscient mind reader, she didn’t know that was him drunk, saving face himself and so it was just another miscommunication. But even after that, she again went and sought him out despite her own perspective on everything and it was only then, after he was already gone that Elise told her it was ED and that it was too late and he was already gone, and she spent years regretting that. Its different with Sylphiette, or Roxy, or if it was Eris, those were all people who had deep bonds with Rudy, and knew him from childhood, and who both sides were willing to speak plainly what they were thinking

Another fun fact. Sara wasn't even in the web novel (to be fair, the entire Mid-Level Adventurer Arc wasn't in the web novel). Originally, there was a time skip after Eris left Rudeus, going straight to the University Arc.

When it comes to their relationship (in the anime at least), I think that they both messed up... it makes sense that Sara wasn't mature enough since she was something like 15 at the time. Rudeus was hurt and needed more time to learn to know her and feel comfortable with her, and she didn't see that and acted hurtful to him because she was also hurt by him not getting it up. But you have to consider that she didn't know about his ED either. Rudeus could have had a talk with her to explain his situation and clear up the missunderstanding and they could have worked thing up, but he insulted her and disrespected her in a drunken stuper while with Sol ending the possibility of conversation right then and there... so they both acted immature and they both fucked up, and I kind of like that. It's nice to see teens acting like teens and having this kind of stupid drama that could get solved if they just talked to each other. It feels real and beleavable in a way.

Especially with not only what Rudeus does for Princess Ariel, but the banter and sexual tension of their relationship AND him being a Greyrat, makes me wonder why she wasn’t added to the harem, even if in an unofficial capacity. The reason is actually more simple imo, Mushoku Tensei already popular as web novel in syosetu, and Sara is added character for Light novel serialization, I think author or editor just want to makeit safe, so not adding more wife in Light novel.
