I think from what's I've heard in videos it is 100% vanilla, only models and custom texture and data packs but I'm not certain about how it would actually work The only clue I've found is in one of Grian's latest videos where we see some distortion when he turns his head very quickly but I'm not sure what that actually implies... Maybe it means it's some kind of rendering / shadering and not actually a model change ?

Not quite. They change the models of mobs, players, and/or items, then find a way to put them in the game, likely using optifine.

you can have server mods without requiring them on the client, that propagates the shown model size or something..

Someone threw a splash potion at him. The splash potion changed his character model. This is the second time he got hit with that Splash potion during this season.
