I'm a GP. Finding your videos very helpful for my own education - sometimes I know about or have heard of the things you cover but by watching you explain things so clearly that's helping me improve my own communication. With 10 min appointments we have limited time for thorough explanation of things, so I've texted links from a few of your videos to my patients. For example, I'll often cover nasal spray technique very briefly but then also text a link to your video on that. You're helping me empower my patients to manage their conditions better themselves, understand what all their diffderent medications are for and learn more about what the alternative options are. Keep up the great content.

Thanks for this, it's very helpful and informative. I was addicted to Otrivine nasal spray for over forty years and the effects of it were exactly as you described, with far too much rebound congestion. One day I just stopped using it, despite all the problems experienced for a few weeks afterwards. I didn't use it for several years (in case you're wondering I'm in my early 70s now) and while the nasal problems I originally took it for didn't clear up, I no longer had the symptoms associated with the spray. I do very, very occasionally use it - if I have a very blocked nose - but only ever one spray a day for no more than two days as I dare not get addicted to it again. These days, I use Piriton (chlorphenamine maleate) and Calpol saline nasal spray. But in the summer months, even with those, I get really bad hayfever with a very runny nose. I have tried Beconase but it gives me a horribly dry throat (even using it properly!) and I worry about the immune system suppressing qualities (during these Covid times) of using steroids of any kind, longterm. I wonder if the non-steroid spray you mentioned, Rinatec, would do the same. If not, I might try that. I also wonder if your technique of not sniffing up nasal spray also applies to saline? I'd have thought that getting it as high up as possible, then blowing ones nose, was beneficial.

One of the things I don't like about steroid sprays is that even though they're not supposed to have side effects, no matter how thoroughly I try to keep it from going down my throat, I definitely get side effects and the primary one is that they make me feel horribly depressed and irritable. (I'm talking fluticasone and triamcinolone, which are the two widely available ones in america.) I try really hard to spray to the outside and keep my face down and everything for ten minutes - the moment I get back upright, glurp down my throat. I don't tend to get runny noses or congestion with sinus impacts, the stuff in my nose just swells up especially when I'm lying down and I can't breathe through my nose at night. I suppose it's time to find an ENT and see what I can do...

Think about finding out what allergy you have - if you have house dust mite allergy look at this video - kzread.info/dash/bejne/ZomHuJutoZyZYtY.html there is a free book with that one as well. why not try hypertonic sterimar? it doesn't last long but might help?

Jst discovered your great channel as Ive had sinusitis for last few months ( have nasal polyps as seen by ENT consultant) & tried the decongestant sprays which are the most effective for my 1 blocked nasal but like you said only for few days. I’m on prescription Nasonex spray so I’m glad it’s a low steroid spray but it’s slow going. Have mixed up my own saline spray,,,sea salt with clean water…to try flush out the mucus but do get the old man drip from my nose too…old man at mid 50s…liked & subbed 👀🙄💁🏻‍♂️

Thank you for such an informative video. I know the nasal spray Afrin (I don’t know what the generic name is or what it’s called in the UK) almost destroyed my nasal passages. I had blocked passages since February of this year up until 2 weeks ago after quitting it in March. That spray is so damaging, I’m not sure why it’s still on the market. I warn people to stay far away from it.

Will flucticasone propionate shrink the nasal polyps I have. I can breath in but it gets plugged when I breath out of the nose especially at night. You think if I commit to this spray it will help enough so I won't need surgery? I have been to an ENT and they have confirmed I have polyps also deviated septum. Also can I have polyps removed without septum surgery?

So i have really bad allergies and always have a hard time breathing, so you would recommend simething w/ low bioavailability. Do you have any recommendations as for whay brands to get? Thank you for the info!!!

Thanks for the videos, they are really interesting! Unfortunately 2 years ago my nose started being chronically congested and steroid sprays only really help during the day, but at evening/night it gets congested either way to the point where many nights it feels like suffocating so that going to sleep is impossible. Decongestants are the only thing that help me there, and I'm trying to minimize the usage by using it only at night and on a different nostril each time until my OP in 8 months. :x I wish I was able to avoid them, but before I started using them, quality of life was terrible. (I've already addressed dust mites, and the issue persists even while traveling in different hotels, and it also happens outside houses)

I have a question. I'm planning on using Nasonex (my turbonates are swollen). My question is how long should I use it for ? I hope to get your input regarding this.

ok so i use nasal snuff and ive lost some ability to smell it. i know you are probably going to tell me to stop using it, the issue is its a major part of the quality of my life other wise i would. there was one nihgt i kinda went overboard and through that night i flushed my nose between 5-7 times. i basically was manic and wasnt thinking of the consequesnces. i could smell things out side of my nose as if it werent impaired but as soon as the snuff hits my nose i cant smell the scent of the snuff. but can smell outside of my nose fine. even the snuff from the can its pretty noticeable outside of the nose like the scent from the can but when it goes in my nose i cant smell it at all. do you have any ideas about why this might be. ive talked to my ent i live in america and as soon as i told them i used nasal snuff and eplained the problem they basically dismissed me. i believe nasal snuff is more prevalant over there. if you have had a patient with this same problem, were you able to treat it and fully restore their sense of smell, if so how did you do it, if not what spray or med combo might help so i can talk to my doctor about it

Vindicated! I used to tell my mother back in the 70s that all the anti-histamines didn’t touch my severely blocked nose. I don’t think they had these sprays back then 😢

What about Clarinaze - mometasone furoate? What's the bioavailability of that?

A great spray I have found that helps me breathe better at night is called Xlear which contains Xylitol and cleans the nasal passages well. It's completely safe and I have found it works well. I also use mouth tape and have learned to breathe thru my nose at night that has really helped me with sleep apnea issues.

Hi Vik. Where does mometasone sit within this?

I have something that has caused me great misery. As soon as I lay down one or the other nasal seales up. I've tried Flonase, azelastine cortisone, netti pots, mucinex decongestant and Claritin. I'm in usa. Wakes me up with a headache. I've had a septum surgery, as well as a turbinate reduction 10 years ago.

Sorry you are going through so much, I am not sure what I can suggest other than see someone about your nose. You might have empty nose syndrome? or your turbinates have grown back? get seen by someone.

What about Nasacort - triamcinolone acetonide?

I have nasal polyp problem for 4 years i use xynosene spray it is safe or not. But my nose one side is totally off in every day this is big problem and also breathing issue . Please help me what type of medicine can use or surgery better option.

Oh dear, I've been using these sprays for 40 years every night, I can't cope with not been able to breathe through my nose. I can't break the cycle, I've tried.

I use allergy spray every night, the whole year. I have dust and pollen allergies. The spray is called Mometason (Mometahexal). Should i stop using it? I have sleep apnea too

I read decongestion sprays did not cause rebound effect when used with steroid sprays. Steroid sprays however can cause changes to brain volume which may be the cause of neuropsychiatric effects associated with their use.

What is your opinion on Saline Nasal spray?

hi vik. i had septumplasty 9 days ago. using a nasal spray with hyaluron and dexpanthenol at the moment. to which group does this nasal spray belong and would you recommend any other ?

I'm in Argentina. An ENT just prescribed me a fluticasone spray. Is fluticasone a low bio-availability steroid? Thanks

What about salt water solutions ?

But can those low bioavailability steroid nasal sprays be used during pregnancy and lactation. The obg is unsure.

There are no studies on pregnant ladies / babies and therefore everyone will be too scared to say yes. I know that Obs consultants do give massive doses of steroids in pregnancy to mature foetal lungs so it will have an effect on the child. I'm sorry - no one knows.

Hey Vik, thanks very much for your videos. They really are amazing. I have been using Otrivin to open up my nose early morning, first thing when i wake up 😅 now I am going to stop using that. Do you have any suggestions about how should i drain my nose in the morning?

otrivine is pretty hard to break away from - see this video - kzread.info/dash/bejne/npNrtriBoduvkZs.html I would slowly transition over to the nasal steroids and be prepared for a tough journey..

I guess loss of neuronal input from the nose? there are tests for this I believe.

My Dr suggested finding a spray for hay fever with minimal steroid content - she doesn’t like beconase for example.

@@VikVeerENTSurgeon I have the same issue and recently had a consultation with you Dr Veer. I have had a full allergy test profile and everything came back negative. My diagnosis from the allergy consultant is therefore non-allergic rhinitis and he has advised I have a psychological assessment and some form of counselling / CBT to examine if this might help my condition.

You forgot another spray, Capsaicin Spray.

Tried it, one side of my nostril was working, then i used it and it completely blocked it, my nose doesnt like it all.
