When to Have Rhinoplasty with Septoplasty | Dr. Angela Sturm

drangelasturm.com/ -- If you have medically documented nasal breathing problems, and also want to change the shape or size of your nose, you might consider combining septoplasty and rhinoplasty in what is sometimes called a “septorhinoplasty.” You’ll have to find a facial plastic surgeon who is also trained as an ENT (ear, nose and throat) surgeon. Here’s a guide.
FULL ARTICLE: drangelasturm.com/septoplasty...
What is the difference between rhinoplasty and septoplasty?
While both rhinoplasty and septoplasty refer to surgeries on the nose, rhinoplasty is usually an elective cosmetic surgery focused on improving the nose’s outer appearance, while septoplasty is a medically necessary surgery done to correct a nasal breathing problem.
Rhinoplasty involves manipulating bone, cartilage, and nasal tissue to refine, reshape, and/or reduce the nose. It can be a life-changing procedure for those who are self-conscious about a nose they feel is too large, prominent, or misshapen. As a facial plastic surgeon, I can tell you that many people do not “like” their own nose. As the central feature of the face, the nose has a dramatic effect on overall facial proportion. I routinely do rhinoplasty on women and men of all ages, gender identities, and ethnic backgrounds.
Septoplasty involves reorienting and/or straightening the nasal septum, which is the strip of cartilage that divides the nostrils. This is a surgery done inside the nose, and most people want to know if it will also cosmetically enhance the look of their nose. Unfortunately, the answer is no. While a septoplasty will fix a deviated septum or obstructive breathing problems, it usually will not change the outer appearance of the nose by much.
Can I get rhinoplasty with septoplasty?
Yes, and it is becoming increasingly common. A septorhinoplasty combines both internal surgical realignment of the septum and external cosmetic nose reshaping in one procedure. If you have a documented structural nasal problem like a deviated septum, cleft palate, chronic sinusitis, enlarged turbinates, or narrow internal nasal valves, you will likely need what we call “functional nasal surgery” to allow you to breathe more easily through your nose, this can include septoplasty, sinus surgery, partial reduction of inferior turbinates or repair of the internal nasal valves. Septoplasty is often covered by insurance if it is deemed medically necessary and more conservative treatments have failed. Though it is always recommended to get preauthorization from your insurance company.
Rhinoplasty, by contrast, is usually not covered by insurance unless it is necessary to address a nasal injury, fracture, or genetic defect. Most rhinoplasties are done simply to make the nose look more attractive and proportionate, so insurance companies view it as an uncovered elective surgery.
But if you’re considering both procedures, it really is best to have them done together, by one physician. Your surgeon can often use cartilage harvested during a septoplasty to reshape your nose during the rhinoplasty. If you get a septoplasty and then get a rhinoplasty, a surgeon may be limited in what they can do aesthetically because of what has already been done structurally.
What is the average cost of septoplasty and rhinoplasty?
The cost of septoplasty and rhinoplasty will vary based on whether the procedure is covered by insurance, what part of the country you live in, the extent of the surgical work to be done, and the expertise level of the practitioner. That said, recent national averages for septoplasty are anywhere from $6,000 - $8,000, though note that septoplasty is usually covered by insurance if it is medically necessary. Recent national averages for rhinoplasty are also in the $6,000 - $10,000 range. Unless it is a functional rhinoplasty, the patient will need to pay out-out-pocket.

@@sanchez5963 I got it 2 weeks ago and its only painful for about 3-4 days after the surgery but thats just because they accidentally cut a few spots when doing the surgery. But even 2 weeks after I still have blood dripping from my nose and going down my throat and it was very painful and messy to have the splints removed but only 10% of the people who get this surgery have a splint so if you don't get one then you are one lucky son of a gun but in the end its all very worth it, after about a week after the surgery I felt I could breathe better and wasn't getting congested as bad as before but I still have to tape gauze to my face because of the bleeding but besides that I think its pretty worth it.

I just had the same 5 days ago Septorhinoplasty due to a Deviated Septum. I am happy that I was able to breathe immediately after the surgery! Furthermore, I have a new nose as my nose is hump free and straight. Just going through the post surgical symptoms which is nasal dripping/feeling flu like but I have decongestants. Overall happy and recommended but requires patience as it has a long recovery.

@@ariamacias128 I will be honest. I hurt SOOOO bad after and now the doctor says it takes a YEAR for all the swelling to go down. No one said that before surgery. That being said, I guess I’m glad I did it. Get a jar of Vicks to rub under your nose. It helps with the pain and dryness. Get a humidifier and get a vaporizer that takes healing oils. Hi Angela , I have no smell and minimal air movement in my right nostril caused by a significant caudal deviation to the right which essentially obstructs the right nasal vestibule. Do you think septorhinoplasty will give a good functional outcome? And is there a risk of losing the sense of smell altogether with this surgery?

Right lamellar middle concha. 7. Constant symptoms blockade, poor sleep, headache. 8. Last 15days again sinuisits ( pansinusitis ). MSSA isolated. What's your opinion? For persistent CRS problem 1. Middle conchoplasty with FESS ( how likely is the success rate according to your experience over the years ) 2. *Do you prefer to reduce inferior turbinates along with FESS* ? ENS chances?

I have severe sleep apnea, tonsils & uvala removed, now my airway dentist says if your having septo done you might as well have the rhino, does this increase the risk of a failed septo / turbinate reduction or for that matter increase the chances of success because the ent plastic surgion can shape my nose favorable to function better?

It depends on if the part that is crooked is the bone or cartilage or both. You can straighten a nose without a septoplasty unless the septum is very crooked and is what is holding the nose to one side or the other.
