When The Immune System Attacks The Brain - Antineuronal Antibodies Discovered in a Case of Mania

This video is about a clinical case that provides further compelling evidence for the role of autoimmune mechanisms in the pathogenesis of psychiatric disorders and the need for vigilance for autoimmunity in psychiatric practice.
This case was published in ANZJP.
The poor response to psychotropics, followed by a good response to immunosuppressants, the presence of antineuronal antibodies indicates immune dysfunction and is consistent with synaptic encephalitis. Dr Sanil Rege is a Consultant Psychiatrist and co-founder of Psych Scene and Vita Healthcare.
Informed consent was obtained to present and share case details.

Strange he never said the words "neuropsychiatric lupus" which seems like the most likely conclusion, especially with the anti-dsdna. Atleast the treatment was the same.

It's a good point. Strictly according to the diagnostic criteria lupus could not be diagnosed. This is an issue as neuropsychiatric symptoms of lupus can occur before systemic manifestations. This was also the reason why the rheumatologist and Immunologist did not get involved in Rx as they deemed it psychiatric. This is an issue as many patients with neuropsychiatric manifestations may not get optimum treatment. The treatment was initiated based on the clinical presentation with steroid as an empirical trial.

Please help.me understand npsle I can't seem to control my temper. When I spoke to my doctors they referred to to psychiatrists but then the psychiatrists prescribed me some anti-depressants. I am not happy with the diagnosis

It's difficult to clarify this on comment section. NPSLE can affect moods - so both Autoimmune conditions and psychiatric conditions can coexist. Diagnoses in psychiatry are always provisional.
