Great video! Your observations with regards to the SPLIT Trial are on point, its basically a trial showing that with healthy patients (i.e. not having deranged renal function to begin with) N/S and Balanced solutions are equivalent - a finding of very little use in the setting where I work and, I suspect, most ICU's/Emergency Depts around the world! With respect to DKA treatment in particular, I think your observations are profound. Current DKA protocols, that Im familiar with, all advocate initial high volume (often 2-3L) resus with NaCl 0.9%! Considering NaCl 0.9% has a pH of around 5.5 and, given the high rates of AKI associated with the condition, such a recommendations must surely be having a deleterious effect on patient outcomes across the world?
