Unraveling the Mysteries of Cannabinoid Hyperemesis Syndrome (CHS) at CannMed 2019
Moderator: Kyle Boyar
Panelists: Michael Dor, Alice Moon, & Tom Folan
Cannabinoid Hyperemesis Syndrome (CHS) is a condition characterized by persistent vomiting induced by cannabinoids that has recently gained lots of media attention. What triggers its onset is a mystery however is seems to have a preference for those with high cannabis consumption. Additionally, not much is known about the pathophysiology of this condition and few treatments are available based on our current understanding. In this panel we hear perspectives from patients with CHS as well as the physicians who treat about its potential causes, triggers, and treatments.

After smoking for 20 years every single day I was diagnosed with CHS . I’m now 3 weeks clean. I got tired of ending up in the ER with abdominal pain and nausea. It was so bad I couldn’t take it anymore.

I smoked tons of weed back in the 70's and the 80's with zero side effects. I started using Wedding Cake and Platinum Kush Indica last fall for my insomnia. The nausea started pretty much immediately. My question is, what is different about commercially grown weed these days making so many people sick compared to the Mexican, Colombian, and Jamaican we had back in the day? I am currently recovering from CHS. I am treating myself with amino acids, probiotics, and electrolytes, but it's a slow process. Today's weed is obviously very toxic for some reason.

They believe it’s the cannabinoids and if you compare cannabis from 30 years ago to cannabis today it’s night and day everything skyrocketed you can find strains with upwards 30% THC and you can find concentrates over 90% and it’s not just THC that’s believed to be causing this. All Canabinoids are and there’s 113 of them known. Here’s 8 of the most commons THC (tetrahydrocannabinol) THCA (tetrahydrocannabinolic acid) CBD (cannabidiol) CBDA (cannabidiolic acid) CBN (cannabinol) CBG (cannabigerol) CBC (cannabichromene) CBL (cannabicyclol) And the 105 remaining are also a lot more prevalent than they were !

It's not toxic it's more potent. I smoked high grade weed and concentrate for at least 10 years before I started to experience chs. Personally I believe that it is something that builds up in a person after a long period of use. Midis probably don't have the potency to trigger it but if you smoke them long enough and switch to high grade I could see chs happening since the midis layed down the ground work.

Your comment is very interesting. I smoked pretty much all the time for 10 years then switched to dry herb vaping for about 6 years (heavy daily use) and then I switched to a more moderate schedule taking weeks and even months off at a time. I was buying off the streets for all this time and never had any issues. I got accepted for medicinal cannabis the other day (UK) and got my first prescription. On day 3 I was using the strongest one (23% THC) and then the next day after a cup of peppermint tea I got the worst abdominal pain of my life and had to leave work early. I couldn't say it was CHS for sure but there was definitely something "weedy" about my symptoms! It's a shame you can't get much mid strength stuff on prescription.

Back in end of 2019 to the beginning of 2020 I had large episodes of CHS and ultimately found myself riding my bike every day for between 15 and 30 miles, even on days that I was already exhausted from work. Learning now that cardio has a similar effect to the shower makes so much sense. I recall when I had a relapse in mid 2020 I tried to smoke again and immediately got sick again and I hopped on my bike to take a ride because I thought “being alone in nature let me ignore the pain” but now I understand that most likely I wasn’t ignoring the pain but treating it. Thanks so much for your insight guys, this helps a lot 👍

It happens while sleeping because body goes into fasting and burns the fat cells and then the thc is released. This is why many of us wake up vomiting, because the stomach isn't emptying the bile. The signal from the thc is actually telling stomach not to empty the bile. Then as the bile replaces self after vomiting we just continue to vomit it up. It is very brutal. Also, as more people do Keto (which is a great imo) more people will experience CHS, because of the fasting mode. This is a very serious syndrome, and needs to be address honestly, without demonizing cannabis, at the same time.

This chs has absolutely ruined my life. Thankfully I found out it was chs instead of svs (siclic vomiting syndrome) so unlike those with svs i can just stop using cannabis and fix it where as they can't! This stuff is so so painful and my God the place it puts me in mentally is almost as bad if not worse than the vomiting episodes. Its all so bad!

My CHS was always on orvaround my menstrual cycle. I thought it was pesticides or mold... I smoked homegrown organic and then after 10 months i got all the symptoms very suddenly one night. I used it to treat my bipolar depression and anxiety.... honestly, I'm not sure what's worst... a manic crash or cyclical vomiting for days... suicidal depression is aweful, but so is puking yourself to death. I'm really, really trying everything I can think of to reach a healthy state of being both mentally and physically.

See, everyone was acting like I was crazy anytime I mentioned this. Hell, even I felt crazy because it helps SO many other people and I *just* so happen to be on of the very *few* this happens too. The shitty part is I started smoking weed *because* of my stomach issues, as it was the ONLY thing tht helped. I've tried SO many medications, changed my diet, have been seeking help from doctors for the last 10 years because I had nausea and vomiting already and..lets just say whenever I ate, it would go right through me. ( I have IBS as well). So I was rather heartbroken that after only 2 years of me smoking it, it started having this effect on me…I have been chronically ill with stomach issues for about 10 years now, have had SEVERAL tests, procedures and have taken a chemical cocktail of medications for it, seeing doctor after doctor, specialist after specialist with no answers or relief, just “you have ibs”. So I tried weed and it helped instantly and was a godsend….then after 2 years, started the intense nausea, vomiting and pain...and then it started giving me seizures too which I don’t understand at all. It confuses me because weed is used to treat all sorts of issues but no one ever talks about the negative side effects it can have. When I do, I’m met with “oh, you just haven’t found the right strain yet” which is untrue; I have smoked Many a strain ( I smokes ALL of the weeds, hell I could smoke my dad and my boyfriend under the table at age 23) or “you’re just hating. Weed is ONLY safe and has never and CAN NEVER hurt *anyone* “...and it makes me sad...I loved the stuff...it worked really well for me in the beginning and most times was the only reason I ate and also helped with my mental illnesses like Borderline Personality Disorder and my eating disorder ( it helped me gain healthy weight because I would eat good foods and was VERY active and exercised a lot)...I’ve had many a ER visit because of the disorder...I’ve been sober 4 years in june and have hated EVERY second of it, especially since it has become legal in my state. People smoke it anywhere and EVERYWHERE and when I come into contact with it, I have a seizure...it breaks my heart that the very thing that saved my life now puts my life in danger. I do hope one day they can figure something out because I miss the relief I got from it, physically and psychologically…and people will say “just quit” but don’t realize that a lot of us aren’t just using it for recreational purposes; we’re treating something be it mental or physical illness and more often than not, it’s the *only* thing that helps with out problem.

Have you ever done a stool elastase test? May be this leading to meals “going right through you” and IBS symptoms.. I had similar issues (and relief, for a bit.. with cannabis..) but turns out my pancreas is insufficient in generating enzymes (EPI) can’t remember what he E stands for but Exocrine Pancreatic Insufficiency.. have to take a medication before every meal and it’s pricy without insurance but has seemed to be helping w the horrible IBS symptoms. I understand it sucks to let the cannabis go but with CHS episodes I never want to feel that way again. Good luck just mentioning a possible alternative diagnosis that is also overlooked sometimes…

Smoked for roughly 7-8 years now. Past couple of years I've been experiencing symptoms exactly like this with my worst vomiting episode being 2019 if I remember correctly with milder episodes of being sick since. Was put on omeprazole which seemed to help for a while, however it has of late stopped helping as much and just as recent as yesterday I threw up and had to call in sick to work. Currently being tested for a stomach virus but all prior investigations by doctors have so far not lead to a proper diagnosis. Will definitely be bringing this up with my doctor.... I use cannabis to help with my depression an anxiety and I also admit I like it at the end of a stressful day after work or uni (like the lady in this video im an evening smoker), so trying to accept i have to stop using it and find an alternative is going to be hard. I have tried anti depressants in the past and they cause nausea, so to have this cause it too after years of it being successful is hugely demoralising. I will definitely be taking a step in the right direction from here on hopefully.

My stepson is experiencing this right now! He is in total denial. He is experiencing SEVERE nausea and is vomiting while moaning and screaming. He is diagnosed with Schizophrenia, with hallucinations, delusions etc. He honestly believes pot helps with the nausea, but the more he smokes the worse it is. He refuses to believe smoking is contributing to this, instead blames "really fast people running by him and throwing things into his mouth". He believes cbd is a way to cope too. He also copes with long hot showers (up to an hour at a time) He's over 21, has both a Dr, and counselor, but he is not on board with a treatment plan. Instead choosing to self medicate with more pot and more denial. His entire day revolves around getting high, staying high, finding more ways to make the high more intense.

It’s very real and many have denial because the symptoms he’s trying to mask are reasons people smoke marijuana. It would require work on oneself, diligence and a level of acceptance most smoking marijuana try to avoid. It will get worse, but only when it does, will he be motivated to address the issue. One stop in the ER with a Haloperidol RX shot and education about CHS makes most deniers the insight to come to terms with this very real issue. I hope he gets well and can grow from the experience. Once you have it - you always have it.

But after hearing about CHS I'm now deciding to smoke my last joint tonight and I'll take a break for a long time. Just to clear out any access THC. I hope this is a good strategy in prevention even though we don't know for certain. I just love weed too much for my mental health issues. It has supported me greatly. And I want to keep it that way! Shes a bit "it happens like this to me so your wrong". Theres other things at play here and anxiety can certainly be one. Such as when she said smoking a joint around her will make her have CHS. With that said CHS is totally real. Any form of THC Can do it, any type of frequency, any level of THC content. The symptoms of THC were documented on cave walls, and its been around for quite some time. Its more prevalent now due to legalization because people are willing to admit to it without fear of jail or rehab etc. I too get it/have had it and its what made me quit. Its really quite terrible feeling. When I get it, its only for hours at a time maybe 6-8 hours. When it first started long ago, i would puke every morning and every night for about a year, but I kept using. Some pre signs of it is acid reflux, stomach pains, diahhrea. If your using and notice these symptoms stop right away otherwise it will progress to the vomiting/nausea stage and its absolutely horrible. I tried everything, elimination diets, had upper/lower endoscopy, used omeprazole was diagnosed with GERD/IBS etc. Once I quit, everything goes away. It acts like a food sensitives sometimes, or will make you believe your sensitive to milk, bread, coffee, etc. Just stop using it. I wanted to share this info. There is a Cannabis Hypermesis group on FB and people share this info. Some get relief with bennedryl for some reason some dont. But if you THINK you have this, just stop.

thanks for your work and interest on this topic to the expositors, and thanks to CannMed to make this possible. Cant understand how it is possible that some doctors dont even know the difference between THC and CBD. How can they diagnose or solve situations like this without the propper knowledge? Here in Chile a lot of doctors practically consider the endocannabinoid System as a "urban myth" how is that possible?. Thats dangerous and a really bad thing cientifically speaking.

I came down with gastrisis and ulcers this week, doc says he thinks CHS is the cause. I told him he was crazy. But after doing some research, I actually do experience most of stage 1 symptoms. No vomiting episodes yet, thankfully. I quit smoking just in case. Since we caught it before stage 2 I think if I were to smoke again later on(as long as it isn't habitually) then I wouldn't pose much risk of relapsing into CHS. I had no idea that this was a thing. We need to spread awareness on this.

i smoked for about two decades and most of that daily, all day, too. thankfully i didn't develop this, but learning about it made me stop and i haven't started again so far. for me as a daily smoker i would go through 1-2 g a day. i would say i was a heavy user by normal standards, and a moderate user compared to some other allday everyday type of smokers. there's definitely people i know that smoke up to 3-4 gs a day. for me as a heavy user that sounds like an awful lot. i was already spending a ton of money on it because i lack room to grow my own. i stopped via substituting slowly with cbd until i felt more or less clear in the head, and after that it was pretty easy on my head to quit, but i had physical withdrawal symptoms like sweating and great trouble sleeping for at least a week. i can only recommend taking liberal breaks even if you don't have chs, as these will probably help prevent you developing the syndrom at all. if i smoke again, it's gonna be once or twice a week.

just my theory, i think it's because todays weed is more amped up and complex in cannabinoids, it's like roided weed. it overwhelms our bodies. also i think people smoke way more overall today. no one had concentrates in the 70s. i remember when i was a teenager we'd share one or two joints and would be baked as fuck. now everyone smokes high grade joints by themselves of strains that are way more potent, building up crazy tolerances. the culture of consumption changed. i sure know for the last year i smoked before i stopped, i didn't even get high anymore. the same weed tho would put my friend who smokes occasionally to sleep. smoking joints was like smoking cigs in the end for me, almost no effect anymore because i fried my system.

My thought Is that MJ activates the immune system and when you smoke the body thinks it’s being attacked so the ‘high’ is the response system kicking into gear. When you smoke so much and that response system has no more capability to fight off things like allergens coming into your body, then the coping mechanism is the full rejection via vomiting of any sort of outside matter entering your body. That’s why things like caffiene and other items create havoc on your body when you smoke too much.

because this strain(see what i did there) of medical science is fairly new and has been only discovered fairly recently. basically we are just know figuring out how important the endocannabinoid system is, how it works and interacts with our body, and how the different phytocannabinoids interact with it and change it over long term use. hence most of the data so far is very statistical and gives us no rootcause yet. this is the case with every new medical discovery. 20 years ago we didn't really know a lot about the endocannabinoid system in our body and what it does. this is all work in progress.

It’s great that works for you but coco especially dark chocolate since that’s closer to pure coco is actually documented as a trigger food for CHS! Everyone’s body is different but it seems the consensus among CHS sufferers is that coco is a common trigger food! Tread carefully readers!

Explain why then I’ve smoked for at least 10 years and was sick for at least six of those years and as soon as I reached four months of total abstinence all my symptoms disappeared and I am now able to live a normal life my friends smoke in my presence I do not even feel a crumb of need when I see a cannabis joint. Cannabis is the reason

Black Walnut Wormwood Tincture- don't take if you're pregnant (anti fungal, parasite, bacterial), one capsule a day - start eating again and fix your stomachs biome imbalance from eating sugar and carb loaded munchies which lead to candidiasis overgrowth, causing daily chronic nausea, cyclical vomiting, and wasting from undernourishment. Quit smoking contaminated plants that still have chemicals on them, and quit smoking plants infected with HLVd(hops latent viroid), a plant disease which will make the trichromes production drop on the cannabis plant, thus reducing the plants immunity to pathogens .. Smoke a sick plant for long enough and you will get sick, cannabis should be handled like a food product.

I have CHS I’m not dead, I just take a Tolerance break and keep myself hydrated and eat clean food and stay away from sugar, greasy foods, salty foods and after 2 months and a half you should be okay, it’s a side effect not a overdose. 🙄 smoke marijuana a gram a week maybe 2 don’t over do it.
