When in dark form, spam both skills while they are in cooldown. It will leave you with 1hp and will make you turn back to normal

Another thing to note is that just make sure to use it as a Last Resort. It can only be used Once per level, and while it does a Lot of Damage and makes you Invincible while using it also leaves you Vulnerable after the fact. But it's a Nice One-Shot trick as long as you don't play Imba Mode on Real Mirage with Pandemonium 2 for whatever reason.
