Doctor amazing work as always, my mom has this condition her scans are showing that there is no compression so the suggestion is that the trigeminal nerve itself is irritated, what treatment should she use as she had already been using tegretol cr 400 abd also nerve block and still the pain comes on from time to time . Thank you for writing. You have raised a very valid point. Compression is demonstrable mainly if the right kind of MRI is done. Now I don't know in your particular case but getting the right sequences in the MRI at a place which is used to doing such sequences is of the utmost importance. Thanks doctor for uploading these insightful videos.I have watched all of them.My mother is currently going through this type of pain for the past few months and our local doctor is suspecting it to be trigeminal neuralgia.After going through all your videos it looks like MRI is important to diagnose this disease.My mother however has a metal rod inserted in her leg which makes MRI potentially dangerous.Is it possible to identify if TGN is being caused by MS/tumor by CT scan or any other technique apart from MRI?Also, in case it is caused by MS, is MVD still effective?I would really appreciate your respone.Thanks

success depends on the experience of the surgeon and the centre having high volume of MVds. Trigeminal Neuralgia being a rare condition, MVD is not commonly performed on a daily/ weekly basis in most places. I would suggest you go to someone who is well-experienced as that increases chance of success and reduces risks/ failures. Lasers are not used, but we use a combination of microscope and endoscope. MVD is generally a less- blood surgery, not blood-less. second surgery is possible after careful considerations of many factors.

Sir, I am from Bangaldesh. I have been suffering from facial pain for last 1.5 years. The pain is burning, radiates to my TMJ also. There is aggravating factors. Sometimes there is constant background pain & intensity increases & lasts for 2-3 hours. Again sometimes it is gone totally & again come back. I have done MRI also. It is true that surgery is brain related. Trigeminal Neuralgia treatment with surgery is indicated if medicines fail to relieve pain, have side effects, high doses are needed etc. I am glad that it is working for you...it does work initially, for most people. Surgery is never suggested as initial treatment!!!!?

@@brainandspinesurgery can you tell why some people having compression and contact with artery don't get symptoms while some get as this neurovascular contact is common among normal healthy people too as some studies showed that and one more question why pain returns after two years in some percentage of patient after mvd and then this increases ...and is resurgery needed in mvd as many people are saying this thing..Hope you answer
