Total Knee Surgery - "Do You Need an MUA?" (Manipulation Under Anesthesia)

If you've had Total Knee Replacement (TKR) surgery, and you're struggling to regain full range of motion in your knee, there's a chance you'll need to have an MUA (manipulation under anesthesia).
For an MUA procedure, your surgeon will put you back under anesthesia (you're completely unconscious) and he/she will forcefully bend and straighten your knee, tearing the excess scar tissue.
This is typically followed by aggressive PT and stretching for the next 7-10 days, so you can maintain the increased range of motion.
In this video, I'll explain how you'll know if you need an MUA, and I'll explain a few other related topics that will help you make this decision.

    I had a total knee done Oct. 21. Went to see the surgeon after about 4 weeks and he said that we need to get more motion, which is what I had thought. Had the MUA done and just WOW what a difference. No pain walking and my knee is bending great. Had PT for 6 straight days and I was at 124 degrees. Finished with the PT now, but I am still doing my exercises every other day. Highly recommend the MUA.

    @@yelenawilliams9953 Not for me. My knee hurt before the MUA, but after it was no pain and I could walk easier. I did have to go to PT for seven straight days cause if you don't the knee will want to stiffen.

    I am a 59.9 year old male that had TKR 6 weeks and 3 days ago. I had a MUA today. I progressed in PT every week through week 5 maxing out at 106. My last PT I was at 101 so I regressed. My surgeon moved quickly to get my MUA scheduled and completed. Right after MUA I was at 130, however Ortho doc said it will likely settle around 120 after the next 2 weeks of vigorous PT. I will comment back here in 2 weeks.

    Update on my MUA: One week post MUA and I am at 117. Definite improvement in ROM, Pain and Swelling. Will give a 2 week report in 7 days.

    I'm 4 months out and still only 110 degrees and 1.5 straight. I had an MUA at 7 weeks. Yes I'm tight every morning. I'm just a failure. I had no idea you could have 2 MUA's

    I'm 6 weeks out and can only bend to 60 degrees. It took my quad muscle around 3 weeks to wake up. My strength has improved tremendously, but extreme pain when trying to bend over 65 degrees.

    Hi, Christina. You're a little behind schedule with your knee bend... are you doing some PT? And have you talked to your surgeon about a possible MUA?

    Hi, I'm in my 12 week and I'm bending my knee around 80 degrees. I was stuck at 60 degrees in my 10th week but now it looks like there is a progress. But my surgeon advised me to schedule a MUA next week. Could you advise me? Should I go or should I wait for another stuck in degrees?

    I'm May 24th 2022 im coming up on July 24th 2022 I have a 100 degree angle but I'm definitely stuck. I still have swelling alot of pain. Alot of pain behind my leg I'm in tears at therapy I'm always still always in pain. I been telling my doctor I want a MUP he says too soon but my pain is unbearable. Any advise

    I'd keep pushing for the MUA, especially if your knee ROM isn't improving. If you're stuck, I don't see any reason to wait for the MUA. If your surgeon won't help, you might consider getting a second opinion, although it's much better if your surgeon will help. Good luck!

    Hello sir I talked my doctor into the MUA I will be having it on August 2nd. Can you give me any advise to stay lose and not to get stiff again. I really would appreciate any advise you give me . I'm still going to therapy I'm now at 114 if they push it but when I bend it I'm stuck at 110. I am trying to not have a 2nd MUA please send me some advise I will let you know how my MUA turns out .     Just stay aggressive with your own stretching and with stretching with your PT! If you're still taking pain medications, that can be helpful to take them before your PT visits, just so you can tolerate the aggressive stretching. Also, can you get into your PT the afternoon of your MUA? That's a great time to have them stretch, just so you have a baseline for full knee flexion, and then you can try to maintain that number in the coming days (hopefully around 130 or so).

    Hello sir another question. Can I get A Lidocaine shot for the pain I'm feeling in my knee while they bending it. It's making me throw up the pain is bad

    No, they don't usually offer injections for the pain, just oral pain meds. How are you doing now?

    I am one of those people that could touch my heals to my but. But started having problems with my right knee. After having pain for about 5 years I went to have it checked out. It was shot. Got a TKR and was at 120 the day of surgery. PT worked it very hard 3 times a week but as it swelled I started loosing motion. I have been loosing ground as time goes on. I'm at 9 weeks and have been at 80 for 3 weeks. I am waiting for a call back for the date of my MUA after the surgeon and PT both said I need one. I'm told I'll have PT 5 days a week for 2 weeks. Hope it works. Anything special I should do after the MUA?

    It sounds like you're on the right track with having the MUA and then having PT 5 days/week for 2 weeks. That's exactly what I recommend. The focus will be on aggressive ROM/stretching every day. Do your best to keep your swelling down, and take your pain medications are prescribed in order to tolerate the aggressive stretching. If you have questions about medications, make sure you talk to your doctor. Good luck!

    Today and surgery 1 day after my MUA the pain was so bad I threw up. Soon as the therapist got to a certain point I felt popping . It has me at the point I'm scared to go to therapy what can I do. The pain was so bad I threw up I felt hot then cold and my stomach felt like I was cramping. Any advise to help Me

    I had a MUA on Monday I had a Osteocondral defect replacement it is different then total knee replacement I don’t find many information about my surgery and it is sad 😞 but on Monday the doctor get me to 145 degree the same day I went to PT and she bent my knee and I reach 118 degrees it was very painful, next day she bed my Kenyans I reach 124 I was in 😭 it was super painful after that day I wasn’t able to do anything my leg hurts so bad, 3 day my leg was very swollen and my quadriceps muscles were very tight I was very sore, I could hardly walk or bear weight on my leg just bend my knee and a reach 93 degrees, and today I am still super sore and I only reach 80 degrees what can I do my quadriceps is very sore and tense the inflammation has gone down a bit but I feel very frustrated, I am scare I don’t want to lose my range of motion but it hurts so much. What can I do to reach again 124 degrees.

    That sounds awful! My best advice is to do everything you can to keep the swelling and inflammation down so that you can bend your knee as far as possible. Lots of ice and elevation, cold water immersion if possible. Do you have access to a pool right now or aquatic therapy? That might really help you as well. Good luck!

    Hello this is my 2nd message, so I got to 100 but I noticed this my 2nd or 3rd week I'm still at 100. My doctor wants me to see pain management, and think my pain is in my mind. I know it's not my quad is tight we been trying to get it loose in therapy but can't. Everytime my therapist goes to bend I can feel the sharp pain and it's so tight it hurts I say stop he stop. I know I need a MUA, I had a MUA for my reconstruction on the same knee. The doctor tells me it's not the same animal but to me it is when the scar tissue don't want to give. I really need some advise . He ordered a bike for me but I can go around but the knee don't want to bend. My therapist says he believes I can get 126 maybe 130. I refuse to lay down at therapy and yell at the top of my lungs or pass out because of the pain when he goto bend it to try and get the 126 or 130 whichever number I get. How do I get him to do a MUA.

    You're in a tough spot here... I'm a big fan of the MUA if your knee simply won't move, no matter how hard you push on it, and I'm not sure why some surgeons are so resistant to do it. My only advice is to keep advocating for yourself. Also, will your PT advocate for you? That might help too...

    I had a new knee in July and in a few weeks I am having a MUA . I have never been able to bend my new knee but now I am worried because my surgeon said there's a possibility that he could snap a Bone .I hope that doesn't happen

    That's a good question, and it probably varies from surgeon to surgeon. From my experience, I've seen clients get a second MUA about 30 days after the first one.

    It depends on the surgeon, but I haven't come across too many who will do one that far after surgery.

    Didn’t have a knee replacement. Instead I managed to rip the quadricep tendon from my knee cap. Underwent emergency surgery to repair. Two months after the surgery and I can only achieve 80 degrees of bending. Will a MUA work for me? Would a MUA damage the quad tendon repair?
