my crown hurts a lot I got a root canal and then I got a crown that was a year ago it's been hurting I'm thinking I have a cavity under the crown

Hey, what about the dirty cord. Is it staying around his teeth for good
Wasn't there a decay left between the teeth or was that hard structure, only discolored?

No there’s no decay at all. The dark line around the gum line is the staining left from the hemostatic agent I used when I pack the cord! Great comment
Can a dentist see on an X-ray if you have a cavity under your crown
Shadow was seen on X-ray under my crown, so have to get it replaced… why can’t you drill through existing crown then fill? Why a whole new crown? Thanks!

It depends on the severity of the cavity. Some dentist would drill through it but warn you that you may have to replace it 2-3 years down the line. Over time, the crown can dislodge. That’s what happened to me- 2 months later.
Which cord packer do you use? Any tips on packing cord, I always struggle with it

Great question Gyoti! So honestly I don’t think the brand of cord packer matters as much as the technique; I personally switch from a variety of different cord packers depending on the cassette I have. First make sure that you have cleared the interproximal space between the teeth. If you have not prepared the tooth deep enough than the cord will not be able to get in between the teeth. Second, make sure you are using the appropriate sized cord. Some tissue types are really tight and you can not fit a large cord while other tissue types like the one in this video allow you to pack a very large cord. If the tissue is too tight, go one size smaller. Have your assistant transfer the cord to you while you tuck in the first part. This is probably the most important step because if you can tuck it under the gums the rest will be easy. If there is a lot of saliva or bleeding make sure to dry the area to allow you to visualize the area more clearly and the cord will pack much easier. After, you want to “tuck & push” meaning try not to lift the cord packer out of the socket; rather push it forward and down bit by it and you’ll be surprised how quickly the cord goes around the tooth. This is very hard to describe in text so I’ll do my best to include more cord packing videos in the future!

Yes prior to the impression. The cord was dipped in astringident which can leave some black residue along the gumline that will go away in less than 12 hours! Great comment!

Nice work, but it would have been nice to see the pre-op radiograph to see the extent of decay under the crown before the procedure.

You wouldn’t have seen it since this is a zirconia crown and the decay was discovered upon root canal therapy at the endodontist. The BWX would have revealed an opaque crown. I will try to include BWX or PAs upon case presentations. Working on one now. Best, Sami
