    Thanks for the at-home exercises. Tibial rotation problems are irritating- it's a difficult spot for me to get to and work on without professional help, and it leads to other problems if not properly handled. And I thought I knew how to walk all these years.

    Under Mulligan theory, there should be an immediate and also lasting improvement. That being said, it's all about the correct home exercise program to lock in the improvements. Any rapidly responding condition should improve within 4-6 visits unless there are further complicating factors.

    Hello sir! I had my ACL reconstructed 3 years back. I have returned to sports and sometimes really couldn't tell which leg has gone through a surgery. . But last week things took a turn, after a week of excessive activity something unusual happened. I was stretching in the evening and while changing my pivot from the left leg to right leg (operated) I experienced a giving away sensation followed by a pop in or around my Proximal tibiofibular joint. This happened two-three times in a duration of 30 mins. The sensation was short-lived but very intense that I couldn't bear my weight on that leg. I went to see my Ortho thinking I may have raptured my ACL again but that was not the case as my knee seems stable as he did some stability tests. And there was no pain or swelling except some stiffness in the ankle and increased amount of grating sound. Today while I am writing to you it has been a week since the first incident, I am able to jump, hop, do squats but still feel something odd with my fibula head. Can you please, if you can help me understand what actually happened to my leg that day and what should I do now?

    Hi, sorry to hear that happened to you. While that is a much better history than what I normally get online, I still can’t give any kind of diagnosis without a full medical history plus thorough examination. You need to see a qualified physio.

    Hi Erson, great video . My question is same as Brad D. I would appreciate your time if you could explain more about external rotation of tibia result in over pronation or guild me to the right link. I've hear about it somewhere but all article which I read,they talk about over pronation of the food causes internal rotation."Pronation of the subtalar joint in a closed kinetic chain is eversion in calcaneal with adduction , plantarflexion of the talus and internal rotation of the tibia (Chai 2007)."Many thanks

    Hi, no citation, just observation. In genu valgus in single limb squat, if the knee is going medially, the tibia is relatively rotating externally as the femur rotates internally. This valgus collapse then also causes forefoot pronation and relative eversion.

    Great video! I have a question for you regarding tibial rotation. My right foot is rotated externally to quite an extent and so is my belly button-leading me to believe my hips are rotated toward the right. I think it might be tibial rotation issues, but should I address that first or the hips? It has been plaguing me for about 4-5 years and is preventing me from my goals.

    Hi, those things don't really have to be corrected and cannot necessarily be corrected with simple exercises.

    Thanks for the videos they are very informative. One question regarding the single leg squat test at 2:08 you mentioned that ankle pronation causes 'external' rotation of the tibia but everywhere else I've checked says it is an internal rotation. Can you list a source for the Mulligan external rotation fault you alluded to? Just curious to learn more thanks.

    It's more due to a lack of dorsiflexion causes pronation and eversion. It's the abnormal pattern that causes tibial ER.

    Does this last small meniscus tear? I felt the pinch was gone, but I wonder will it just go back on movement?

    You can do this several times a day to see if it will help, I would say 10 repetitions 6-8 times daily to see if it sticks. If you can get relief for hours, you can keep the improvements going.
