Thyroid Clinical Exam - Clinical Skills - Medical School OSCE Revision - Dr Gill

How to perform a Thyroid Gland Examination - Clinical Skills Revision
The thyroid examination is one of the first sessions of the clinical skills block for medical students at Warwick Medical School - largely as it touches lightly on to other clinical areas, such as the cardiac examination, and the peripheral neurological examination making it an excellent starting point for building further knowledge
This is a clinical examination of the thyroid gland is performed by Dr James Gill following the approach in Macleod’s Clinical examination.
------------------
Please note that there is no ABSOLUTE way to perform a clinical examination. Different institutions and even clinicians will have differing degrees of variations - the aim is the effectively identify medically relevant signs.
However, during OSCE assessments. Different medical schools, nursing colleges and other health professional courses will have their own preferred approach to a clinical evaluation - you should concentrate on THEIR marks schemes for your assessments.
The examination demonstrated here is derived from Macleods Clinical Examination - a recognised standard textbook for clinical skills.
Some people may experience an ASMR effect from watching this medical clinical examination

    Because the thyroid acts as the “accelerator pedal” for the whole body. If you have and under or over active thyroid EVERYTHING is affected It’s an old video but this does explain it kzread.info/dash/bejne/p4ucrNSGqrebiqQ.html

    Dr James Gill Indeed! I have Graves’ disease, diagnosed as a teenager I was considered one of the worst affected in the country as my readings were off the charts, several doctors viewed me in a group as a case study I was so “toxic” - all under control now but wow those symptoms, I felt like my body was losing its mind if that makes sense 😅

Especially when the doctor would use the stethoscope, because it would go all quiet except for the rustling of their white coat as they moved it around, and softly asked me to take deep breaths. I have Hashimoto’s disease and I’ve never had a thyroid exam like this. It’s typically just a bunch of feeling my neck, which is extremely painful, and blood work. I’ve never had a doctor listen to my throat with a stethoscope or look that closely at my eyes, especially not from behind. I’ve also never had a doctor kindly give me a glass of water when asked to swallow. I want to be seen by Dr Gill! ❤️

    I'm not a doctor, nor am I a medical student, and yet I always enjoy watching Dr. Gill's videos. I understand almost nothing of Dr. Gill's medical terminology, but there are some things that make a strong impression on me. The first thing is the great respect that Dr. Gill gives to the patient. In Dr. Gill's hands, the patient feels like a king who is given a lot of attention. The second thing is the soothing casual voice of Dr. Gill, which creates a uniquely relaxing atmosphere. This voice hypnotizes and relaxes the person, and this is already half of the cure. I especially like to watch when Dr. Gill's hands measure the patient's heart rate or tap lightly on his chest. And not forget the check of the reflexes of the hands and feet with a special hammer. I have never liked the name ASMR, which is given to this kind of sensation. There is something crude and too scientific in that name. And the sensations of ASMR are a kind of opening of consciousness, something that Science has yet to understand and appreciate with dignity. So it is not known when Dr. Gill helps more - when he teaches medical techniques or when he unknowingly treats a huge number of people with his soothing voice. Think about it, Dr. Gill, please !

  • @DrJamesGill

    @DrJamesGill

    4 жыл бұрын

    Astrea Studies part of it is just flow. You are looking for different things at each point. I will do a video on it. But a quick list of general eye signs which covered both hypo (so Hashimoto’s is a hypothyroidism for anyone else reading this) and hyper - lid lag - kid retraction - proptosis / exophthalmos - double vision - signs of anaemia - pain moving eyes - odema around the eyes
