    I know this is a frank and honest page, but all the people saying she looks terrible now need to remember this woman is human and probably reading the comments. How would you feel? We can express in slightly kinder way.

    people should learn to face realty. Especially when - like in this case - they created their own problems. If I do something to look better, I need to be ready to hear other people saying I look like shit. I can't pretend people do not express their own opinions just because it makes me sad.

    Amusegirl1980 this page is for comments and opinions. If we can’t speak out mind what good is it? I’m Sure she knows precisely what she looks like. If she is happy, doesn’t matter what anyone else thinks. If we think it’s unsightly, maybe others can be aware of the General consensus and steer clear of it. I would never set out to hurt anyone’s feelings but she clearly gave her consent to have her photo used.

    I had threads a while ago, at a reputable clinic in London. Everything I read about the procedure said it was "minimally invasive." Minimally invasive my arse. It was incredibly painful, took 10 days to recover fully, 4 weeks to get to a "finished" result, and was very expensive. I was happy with the result, and it has lasted a lot longer than fillers, but unless I had unlimited funds and was signing up for a whole programme of treatments over a period of a year or so to set me up for the next 5 years of my life, of which threads were a part, I'm really not sure I'd have it done again.

    Hmmm...she looked better before. I mean, she does maybe look a tiny bit YOUNGER, but her face shape was more flattering before. Could you please do a review or video on Profound for jowl tightening? I think that's really my only option that might accomplish something short of a facelift on my lower face but it is SO expensive.

    Agree but It's a slightly misleading image. Her face is slightly tilted down in the after pic and she is wearing her headband lower which makes her face look shorter. It probably looked better in real life

    I agree on jowl tightening.. there has to better methods to do for it, that are not so expensive! I don’t want to do my whole face just the dang gowls 😃

    I’ve had threads done twice (5 & 10 years ago) and the results were subtle but improved over the months and lasted 2-3 years (I am 63). If you are older with jowls and looser skin on the neck the results are much better, this model I don’t think needed them (no downturns at corner of mouth to pull up) and looked quite young. Mine were done in one session and I had little bruising or swelling - getting silhouette threads done in 2 days. Yes it is expensive but not ready for a face lift ($3,000 and $5,000 in Australia) and I go to a cosmetic full surgery doctor.

    Also, I'm curious what Aqualix (sp?) is? It was mentioned for dissolving fat around the lower face. I've never heard of it before! Any chance of a video on this procedure down the road? Thank you!

    Funny that this is my biggest fear. I had an inflammation (not cosmetic related) and wouldn’t take steroids bc I was afraid of getting my face looking wider 😀 Thank you for saying that.

    46 year old here, and I think a problem a lot of people have when it comes to peels, plastic surgery, etc, is that they want to look *younger* and not simply “better.” The model in this procedure did look younger after. Having a shorter, more elfin face makes one look younger. But, as you point out, she might have looked “better” with different procedures. Perhaps some people want to do one big thing rather than several small things? I recently found myself feeling that I looked too old and drawn, so I renewed my motivation to get microdermabrasion, and also decided to start wearing my hair long and down. With long hair, I no longer notice my long face, LOL! Sometimes it makes more sense to revamp your look or personal style, rather than trying to change the shape of your face.

    thank you so much for being honest Dr Sarah. I have just had thread lifts put in by my cosmetic doctor; it is nearly two weeks now and one side is still painful should I apply pressure ,say,when I am cleansing my face. On the plus side, I love the results.

    I think she looked great before the procedure. It also seems that the interventions are never ending. Even after the lift, you mentioned a long list of other procedures you would perform on her. When does it end? When is it enough? When are we happy with our face? If you dissect the face piece by piece, like you did here, a person will never be content and happy with their appearance no matter how many procedures they do.

    Yeah... no. Though she did get a decrease in the crepe neck, the rest of her face now looks to wide. Not a fan of thread lifts. With the amount of time it takes to settle down and the real possibilities of fistula formation. A mini lift and more conservative treatments and fillers seem the wiser choice to me.

    annette fournier that’s interesting. I was married to a plastic surgeon for 19 yrs and he had work done. Pretty much all the wives have work done. But Texas has great surgeons so the work looks pretty good. But to your point, I remember sitting and speaking to an ophthalmologist about Lasix surgery and noticing halfway through the consultation that he was wearing glasses. 🤔🤓

    Love your dress, you make it look amazing! I can’t agree with you enough, the only place where the threads have good results is her neck. I think fillers and toxin would’ve given her much more of the results she wanted. I now have a much better understanding of why you don’t like doing these! Thank you for keeping us so well educated, and for all of the information at the beginning of the video and not just diving into the procedure. It’s really interesting to know the why and how behind the procedure, the different types available, potential side effects, etc.

    Hi :) do you think, that using only the small 2-3cm ones in the neck could be enough for the exact same result - without the barbed ones - by using maybe if needed just more pieces (or adding screw ones)? I would think just because they seem less invasive and secondly in the neck you work only vertically, so might not be necessary always to use the larger barbed ones. 🤷🏻‍♀️ What is your experience?

    I got 35 done. I got one bruise. Swelling was almost non existent. I took arnica before hand. I haven't really seen too much of a result and I'm pretty young with good skin. Hopefully, I'll see some good results. Doc charged me 3k. I got PDO threads. Doc said it would last for about 1 to 1.5 yrs. Hopefully, it will last longer.

    Plastic surgeons Rn assistant here. I hate thread lifts. Hate them! I’ve seen patients two-three years later whose threads have literally sawn through their cheeks and they are left with a near impossible to correct sinus tract. Yes! A tract that drains saliva outside the wound and down the cheek possibly forever! One woman, who was also a friend, had an open wound for 5 year until a plastic surgeon finally agree to do radical surgery to repair it. She is permanently disfigured. Don’t have a thread lift. It’s not worth it. Even when they “work” they look like shit.

    My Mum has a beautiful thread lift. Honestly if you applied that attitude, based on all the clearly TERRIBLE filler work out there, then nobody would get that done either. Same for botched Botox - tonnes of bad experiences of all treatment on www.realself.com

    Amusegirl1980 I agree there is no shortage of bad work being done. But at least fillers don’t last forever. The things I’ve seen with thread lifts years after is chilling. I am happy your mom is happy and sincerely hope she stays that way.

    You are honest!!! That really good of you =) I'm almost 60 years old and have never had anything done to my face but since last year I've lost a lot of wights and have two lines on each side of my mouth which I've been told can be sorted by having fillers. The thing is I'm very scared of things going wrong. What would you suggest? Thank you so much

    Faranak Peart I’m not a doctor, but I would say make sure you go to a good doctor and don’t feel rushed into it. Try going to a doctor just for a consultation and if you’re not comfortable with them or are still not sure if you actually want to do it yet just tell them you want to think about it a bit more.

    I have seen many thread lifts & this is absolutely the 1st one where the face is wider🤔 I believe the after picture is after the 3 mls of filler you added. Threads lift up, after my cousin's her chubby cheeks appeared smaller from the tightening & her face appeared thinner.

    Firstly, I LOVE your total honesty .... and your great SOH!! Thanks for managing to make me smile in virtually every video! I'd also have total confidence in your recommendation but as I live in Australia, I can't see myself coming to London for treatment, especially as I expect that you are booked up years ahead just with your 'regular' patients. Mind you, I watch your videos with a couple of my friends and we would all love to have you living here! :) I have a Bells Palsy thanks to CIDP and am fed up with constantly being asked if I've had a stroke. The nasolabial fold appears to be quite dropped because the other side has remained quite good .. I'm 67 and am regularly told that I look 10 to 15 years younger (not that I can see it) apart from the 'stroke' effect. My doctor suggested that I'd be better off with thread lifting than Botox, fillers, etc., what type of thread would you suggest for the fold? I'm also aware that there are solvable threads , however with a limited budget, I wouldn't envisage having it repeated down the track. Your suggestions would be greatly appreciated.

The very best beauty comes from kick ass confidence, self belief, kindness and empathy towards others and self. I'm more interested in understanding the facts.     I could see a very natural rejuvenated look and she looked very good. That being said, your much appreciated candid comments on positives and negatives of products or procedure you use or have researched and after watching the procedure, I could not bring myself to placing a permanent foreign substance in my body. Kudos to all those who have done it and love it but it just isn't for me...

    OMG, I didn't do this because I was afraid it would drive me crazy to have something under my skin. Now I'm so glad didn't! All I need is a rounder face -- then with fillers? I would have balloon head.

    I do love how you explain the procedures :) So it's a real thread? A chemical thread? Is it a permanent implant? Does it dissolve? How does it interact with the surrounding tissue? Are they easily removed?

    soraninja it’s a PDO thread which is the same stuff dissolvable stitches are made from. It dissolves and it causes a little fibrosis in surrounding tissues which holds the face up

    Does filling the chin crease work for everyone or does it look bad on some? My chin crease was filled with 0.3 ml HA. I hated it and it took away the look of my pouty lower lip and made my chin appear longer. It's been dissolved now. The filler was injected gradually and with 0.2 ml I couldn't see any change yet, but with a little more, it gave a look I didn't like. Is it possible to have the crease filled without it being at the expense of the surrounding features? Is it down to technique?

    Adding filler to the chin crease will elongate the chin slightly yes. It depends what your chin looks like to begin with because if its already long then adding filler might not be favourable, but if it is short then you can get away with adding more. You can find this out by measuring the facial thirds.

    She looks amazing. People also do not realize that this type of procedure takes more than one session. Typically around three sessions. I’ve had this done years ago. Also I must say that she does have a more rounded softer shape. And the same thing happened to me. Having a more rounder shape is less photogenic. Sharper features Photgrapher better. But amazing work doctor. I watch your channel all the time. 😘

    Hi! I am considering threadlits to the midface but am not sure! I feel like fillers will make me look, well, filled and I’ve gotten voluma in my cheeks. I want more cheek contour and my smile lines are very obvious. I can send u a picture. Let me know! I’m 31 and my weight fluctuates after my baby. I went from 125 to 170 to 115 back to 125

    Rou8nder, smoother jawline but neck is very much improved...only 3 weeks out where maximum effect will take several weeks longer.

    Very informative. I guess I never knew what a "thread lift' was actually. I certainly see some lift apparent, a bit similar to that old trick of smooshing your face up to mimic a facelift but also see that filler is probably needed to address some areas too,

    Results are really apparent immediately after this treatment but you need to overcorrect in practice so that once everything settles down you have a really nice result ☺️ 100% a. mix of threads and filler would be best for most patients

    Sarah, interesting, I had threads done when they first came out, the barbed type. I only had 2 on each side of my face, they are from chin to temple. I must have been lucky because I had no bruising and very little swelling. I did have a barb that caused a bump on my jaw and my derm removed it, I have a tiny scar there. I still have injections and botox but feel they have kept me from having major sagging. How do you feel about cheek implants. I'm considering them in the future and there is a lip lift that looks interesting. I'm on hold with my injections, I want some around my mouth and lips but I'm having dental inplants placed in March and I'm afraid of displacing the filler. I like the Voluma products and I already purchased them just on hold till later. I'm " under construction".

    I'm about your age and just had a lip lift after much research and just love the result- plus no more fillers that didn't look good anyway.

    @@drdorann Thanks, I haven't heard anything negative about the lip lift, now if I can find someone who's had cheek implants I'll be happy. Filler is okay but it's a bother to have it done over and over. I've said before my aim is to have a soft look, not so much younger, just a pleasant face. Looking angry, or downright unfriendly worries me more than age.

    I don't know anyones whose had cheek implants nor did my doctor recommend them for me. I worry too much about having asymmetry and then needing to get them revised. I know the filler is not a great answer though as I get it every so often..expensive and doesn't last. Found the best doc. for the lip lift from recommendations I found on Realself.com.. @@donnabissell5369

    It’s interesting isn’t it for her I would say that her face is shorter looking because the jowl has been pulled up and this in turn makes her look like the face is wider

    I almost got a thread lift years ago and went to a extremely well-known plastic surgeon in Charleston South Carolina. He said please do not consider doing a thread lift he said that he did more surgeries to remove them then a actual face lift. As the years go by they start to look worse and worse and you can feel them inside and women want them out. So I would recommend never going that route

    It really does not compare to a mini facelift. I do no see much of a difference. But i am glad i watched this so now i know i would not have that done . This is just my own opinion.

    I had mono threads under my eyes and a few years before I had fillers in the same area. Honestly I think fillers gave better results with almost no downtime. I never bruise after fillers or Botox, but the monothreads left me with bad bruising that took 2 weeks to fade away. Results were very subtile, so I’m not sure it was worth it. Especially because I have bad under eye circles that mono threads are not able to tackle.

    Dr Sarah Tonks I yet to see something that works under the eyes 100%. The first doctor used too much product eyes looked a bit puffy first. It looked great after about 8 months or so. Couple of years later, the second time, the fillers left the under eye area uneven and the darkness faded only slightly. I tried two different doctors. Now I have been going to a third one for a year or so for Botox but she is reluctant to do fillers under the eye. So was another one whom I went to consultation for. I reckon fillers under the eyes are tricky to do so doctors don’t particularly like to carry out this procedure. I am also not super pushy because after all the area is not THAT bad (it will be once I hit 40 - runs in the family) and I’m worried about the potential risks of loosing eyesight due to error in procedure.

    I would do both threads and fillers, depending on the area. For the neck the threads were awesome and she has to insist on them to keep the jowl in good shape. But no need for the face unless some time passes and we see an updated pic with the progress. I see a nice smooth forehead and very good skin, the problematic area is mostly the eyes where the skin is loose. Maybe she is afraid of doing anything too close to the eyes? I would tell her to go for a small tweak at least.
