    The concept of total preservation of rectum in W& W approach is highly desirable. Unless we have an evaluation modality ( ie mri or T dna or pet scan) that can categorize the possibility of pcr on a standard scale such as IRAD as LIRAD for hcc, the wait & watch regimen can not be made the standard of care.

    Yes and will insurance companies pay for these expensive scans every 6 months etc...Highly unlikely

    In view of the morbidity related to anterior resection syndrome the clinical trials of W& W regimen the quality of life scores should also be considered.
