You should read the description again very carefully. The multiple endings refer to the visual novel, not the movie. It's just the title of the game. The next point, as the description says here, is ''It is every The Quintessential Quintuplets fan's dream situation: "Futaro Uesugi confesses his feelings to each of the five sisters." and ''The game features an original story set during a graduation trip to Okinawa. The player, Futaro Uesugi, goes sightseeing in Okinawa with the five sisters.''. So it's just a fan's dream situation and it has an original story. It only happens in our heads with its own original story. Moving on to the next point: ''During the trip, Futaro strives to kiss the one he confesses to and secure a future for his long-distance relationship. Will he get his kiss? That depends on the player.'' As it says here, Fuutarou is not going out with any of the quints after their graduation in this story and it's about confessing your love in the graduation trip and making the right decisions to get a kiss. So the story of the game is about how Fuutarou only confesses his love for the quint in the graduation trip. So the game is not based on the movie because in the movie Fuutarou confesses his love and goes out with the quint before their graduation. Which means none of the routes in the game are based on the movie because it has nothing to do with the movie's story.

The name of the game has nothing to do with the movie. The first game also had the name of the second season of the anime and was not based on the second season of the anime. And now MAGES has made a new game where they use the name of the movie. That's all.
