The Mt Sinai Recovery Plan for Long Covid | With Dr David Putrino

Ғылым және технология

One of the world’s leading clinics for the treatment of Long Covid is the Mt Sinai Hospital in New York.
In this film Dr Asad Khan and I talk to Dr David Putrino - director of rehabilitation innovation at Mt Sinai - about the groundbreaking work they’re doing to treat long haulers.
Long Covid is a complex puzzle but one part of it - dysautonomia - is responsible for many of the myriad symptoms patients face. The Mt Sinai team have put together a comprehensive plan for recovery and rehabilitation - all whilst working within long haulers' energy limits.

I've been dealing with my own long covid for the last 4 months. Basically feels like my body is buzzing all the time, particularly in my ribs/flank/neck. If I exert myself too much, it gets worse to the point it feels like I am tremoring, with some tremors being observable. Prior to covid, worked out 6 days a week. I did jiujitsu, weight lifting, rock climbing, swimming, running, cycling, etc. Now if I go on a two mile walk, 24-48 hours later I will crash for 3-4 days. Hoping it gets better, going to try this stuff.

Oh my gosh is that what is happening to me. Im 4 months out from covid, and I did have horrible fatigue. I had weak legs, shaky and could not do much of anything but lay around. I was never out if breath, but I did have a racing heart for the whole time of covid. Which now its alot better and only happens here and there with little intensity. But then I saw this vlog and watched a few of them. So I figure hey ok im long covid. Well I've been doing great this last month, and I realize I'm still recovering, so I've been taking it easy. Well now after hearing this man talk about PEM!!!! That what I have this week. I like a fool thought I felt good and strong, so I did my full workout Monday and felt a little nauseous after I did it. But the next day I was so weak and felt like I had covid all over again, which for me was fatigue and racing heart. My heart has raced 2 times this week, and I've had on and off fatigue. Wednesday felt better went to town did a bunch of shopping lugged around all my bags and flats of water, ate dinner went to bed. Woke up next day Thursday sick as a dog again. I thought oh my God, do I have covid again? So I went to the ER last night and was tested, had a electro cardiograhm test, blood work! All was good, no covid tested negative. The doctor came in and said I have long covid and to do my best to rest and listen to my self as far as energy. He said I was doing well with all the vitamins and natural food I was eating, but he said I was doing too much activity physically, and he said I was crashing, or called it a flare. So lest to say I've been sick and down all week like I have covid again. I cant eat, my stomach is bloated I am weak and feel like crap! I need to find some type of recovery facility like this in California close to Sacramento. Does any of you know about any hospitals treating long covid patients? I think someone said on her UC Davis. But is it treatments like medicines or is it Physical Therapy for the autonomic nerve issue which he is calling, I cant remember the name. Anyhow I need help!

Amazing work! The understanding of "how you feel the next day" was an eye-opener. No wonder it is so easy to overdo exertion. We are accustomed to monitoring our body response and sensations to tell us when we are at the edge, but long covid requires seeing how we are doing the next day. Sore muscles give way to being kicked down a long hillside. Until a patient understands that he or she is not going to like the process.

For me its not the next day or even the 2nd day, but 72 hours latter I have a crash, so its easy to forget during those 3 days after too much exertion, to not exert further, making the Post Exertional Malaise crash PEM, far worse. I have learnt to rest those 3 days, and it does help reduce the severity of PEM.

🌞🙏🌞🙏Thank you very much for these videos. For me, one month 3x per week of very light physical therapy got me ready for beginners pilates with a trainer who's very much aware of my symptoms. I tested it for 2 weeks to ensure I was ready for that. I don't feel that I can add cardio of any level yet because my resting heart rate is still high (90-122bpm). I did ask my physiotherapist for water exercise but now i'm going to ask for the tests mentioned here. Thank you!!! Your channel has done more from my healing than my eight month of many many doctors, at times more than nine at a time. My cognitive function isn't as sharp after pilates and may be in bed the day or next day but I feel stronger and overall less pain and fatigue. Keep up the great work!

You just have to accept that you are where you are, and it can't be overcome or hurried or beaten. Your autonomic nervous system will recalibrate, and you will recover stamina eventually. You need to treat inflammation. I had terrible fatigue, brain fog, memory cognitive issues after covid. I'm Still recuperating. New doctor treated me with I meg solumedrol IV steroid. Within hours of treatment the pressure I felt on top of my head lifted. Several days later I could think clearer my short term memory lapses improving. My fatigue also is much better. I nap once or twice a week once a day. Not every day twice or three times. Why do doctors ignore treating brain inflammation. It is documented on MRI scans. They know it's there but don't treat it. I thank God I finally found a doctor who treated it. Look for doctors who will give corticosteroids to treat long term covid inflammation. This is what causes long covid!

It just gotten so bad again, especially with the brain fog. And the bizarre reaction after eating sometimes, after only.2 or 3 bites! I've even mentioned this to doctors and no one has batted an eye with it. The stress of it all has gotten so bad I think myPrimary doctor and rheumatologist thinks I'm nuts. My sister even thought I was having a breakdown about 2 weeks ago and quite honestly that's what I felt like. And I'm literally in the process of filling out short term disability papers for my job, and I'm terrified that this may never straighten up!

I contracted covid in early April 2020, and at that time I encountered Dr Putrino on the long covid facebook site. Eating gave me galloping tachycardia for a year and a half, and it still does occasionally. But it settles eventually. I would get a heart rate of 200 sitting on the sofa, and it took me a year to be able to walk for half an hour without being very tachycardic. Breathwork helps, and I recommend the Pulmonary Wellness Foundation with Dr Noah Greenspan, long covid lectures. There are good breathwork episodes for long covid. It's nearly two years since I got sick, and I still get symptoms, but nothing like a year ago. What I can say is there's no way you're going to 'get over' this in a hurry. It takes as long as it takes, and it's different for every individual.

I really don’t know what happened to me when I went to the Mount Sinai hospital to get help because I developed long COVID symptoms and I spent almost 2 years going there and I just spoke to the doctor never had any therapy or see and specialist maybe I was very unlucky. I stopped going there because I felt I was wasting my time I was getting sicker and I am more sicker now without any help from no one yet. I feel like dying little by little every day my blood pressure the high is higher than normal and my low is normal now I am having attacks of hypoglycemia my stress is to the roof also I am suffering from chronic anxiety, insomnia, fatigue and worse dehydration and really scared and afraid I am going to die because nobody believes me what I am feeling complaining for 3 years in muted ears or any one I have gone to see. I can’t believe it that I am more up today about what’s going on with the new researchers and I go to see a doctor or ER and when I said that I have long COVID symptoms is like another thing no even trying to understand why I went there I don’t feel the know anything so that’s why I feel alone struggling to be alive

Interestingly my recent reasonably successful n=1 experiment started about 5 days ago, as I was forced to do much more exertion than than my safe baseline normal amount, as was shifting rooms. Even though I had a person helping me, I still had to move about beyond what normally triggers ‘Post Exertional Malaise’ PEM for me. So I knew I was in for one hell of a crash. Other times I did anything near similar to that amount of exertion I was bedridden for up to three months with what feels like endless influenza. However this time I remembered there was a left over supply of dexamethasone (4mg) that my mother didn’t use. So immediately after the exertion I took one tablet a day, for 5 days. The key was not to do much during this period. i.e. not do extra during a steroid high like many people think they can. as a worse crash is inevitable I normally have quite late onset of PEM, usually between 48 and 72 hours after exertion but the PEM didn’t hit me! I noticed my baseline level of exertional ability has still dropped, but I’m not godawful sick, and no flu like symptoms,. My brain fog has increased slightly and a slight hangover like sensation came, but this is definitely much better than what normally happens. So the exertion still has affected me but not to the degree it would have if I hadn’t prophylactically taken dexamethasone. I’ve tried dexamethasone a couple times in the past but I’ve taken it after PEM itself has begun which I feel is too late, as has no for me benefit then. I’ve often wondered whether or not we could take some sort of a immune suppressor or modulator immediately after (or during) exertion if it would have an affect on preventing PEM. I have also used Low Dose Naltrexone and NSAIDS immediately after too much exertion and that has had some slight but still noticeable effect in reducing PEM severity. I am aware that some people with Long covid and MS/CFS cannot tolerate glucocorticoid steroids like dexamethasone, so wont help them, but maybe some other immune suppressant might?. Although we have to be aware that these drugs can cause problems when taken long term. My suggestion is more for emergency PEM prevention. i.e. a short course once in a while.

Good information, I'm blown away that no matter how high the level of physical conditioning was before covid, that if highly conditioned individuals and very poorly conditioned are equally reduced to the same state of functioning and fatigue after similar amounts of exertion then i would assume that would point to something along the lines of MCAS, dysautonomia? ( and a pile of everything else out of whack ). I've got my Zwift setup on hold for 4 months, but walking 3-5kms every other day plus recumbant stationary bike works well ( 20mins at max 110bpm). I may just start using zwift again but dial the effort level to 40% or 50%. Also, i was doing fairly well before i got the first vax (moderna) ... huge setback, but feel a difference in the dysautonomia and mental clarity. ... I was shocked to hear about the peleton person 5 months in... i wonder if their recovery from a massive flare up will be much quicker than the original? Possibly it could even be worse? You know, in Toronto there are so few doctors that are dialed in, or seem to care to be dialed in. Many doctors it seems are burned out and overwhelmed just trying to get through the regular stack of patients (lots of zoom appointments), and not interested in auxillary info. I'm also interested in knowing if I'm even close to the standard physio protocol, Levine is it? Hey and tell us what kind of exercise you are up to these days? walk / jogging or just straight slow jogging yet?

Would be kept waiting weeks for the most elemental details , like 'how much will the recovery sessions cost me'? , to no avail A total mess before we even began, huge lack of organisation, . Incredible lack of integrated information, lack of support, and unbelievable 'attitude' from several people I spoke to. A rabbit hole. and that is Before the treatment. Months digging what could be sinply explained on a brief call or leaflet. I kept feeling it was not Mount Sinai but the health service in a less fortunate developing country.

Thankfully I started taking Pepcid and almost immediately my symptoms started improving. I have since raised my bed head and taking antacid medication together with a GERD diet. Not that it is important for the long haul problems the question remains. But is long haul a result of "gain of function" in the lab or would the normal untouched covid also result in so many long haul patients?????

all post viral conditions are caused by a weak immune system that cant expel dead viral debris from the system, low atp levels coming from low glutathione levels is responsable for that ,everyone that drinks the miracle mineral solution is over covid and any other post viral condition,it does this by clearing the dead viral debris out of the system the symptoms all disappera the immune system calms down and the energy levels rise, and you will say itsa miracle, all the symptoms are caused by dead viral debris moving the immune sysem responds with cytokine storms, all that stops in hours the cost of the miracle 7.59 uk pounds free p and p for anyone that is vaccine injured if they drink the miracle mineral solution it sees the toxic spiked protein as toxic and remove it from the system ,end of vaccine injury

    a) breathwork calms the sympathetic nervous system, activates the parasympathetic - this is a biological process with a beneficial physical outcome b) we’ve just recorded a film specifically on the psychologicalisation of chronic illness with a top class panel - it’ll be going up in the next week.

@@ShanKatOD we are very much in the biomedical school of thought when it comes to disease. But LC has turned conventional thinking on its head and we firmly believe that the treatment for it has to be holistic.

Sorry I am not so optimistic with twenty years of #MEcfs, progressive. In not so many words, you are pushing GRADED EXERCISE for #LongCovid even though CDC 2018 un-recommended it? So tell me: of 100 patients you treated how many you brought to PEM and how many REALLY RECOVERED? HOPE is easy to sell. But medical doctors should not be the sellers.

Dr Putrino here is quite clear that the regime works only within the energy envelope of the patient. The goal is to NOT ever induce PEM.
