The letter was addressed to Dr Joo the son of the murdered man and also the woman who opened the letter. The murderer wrote to her son from prison.

Yes, when they met on the train, and were talking about getting love letters while in the army, they flashed to a scene where he received one of those prison letters when working in the hospital. The killer still addresses them to him at the hospital, which is why his mother saw it while sorting his mail.
