No, the lava was not real, they used to swim out of the lava during the replay. If the lava was real you could not even go near it so when they were on the obstacles they were near it soooo nobody died. According to the BTS

At 1:40-1:52... when the guy with the headband bandana falls in, you can see him at the bottom of the screen floating around atop and swimming around, they just stay under for like 3 seconds and the camera edits them out
