The BEST Dental Implant Results (& how we achieve them): Upper Full Arch Implant Bridge

Board Certified Prosthodontist and Board-Certified implant Dentist, Dr. David McFadden, reviews a case of a Full Arch Implant Bridge, from beginning to end, to demonstrate the steps that must be taken to assure excellent outcomes. This patient had all of his teeth removed, and replaced all of his upper teeth with dental implants and an implant bridge. He never had to wear a temporary denture, NOR did he have to risk the success of any implants by having to use them for immediate support of a fixed (cemented-in) temporary bridge, while waiting for them to fully heal, after implants were placed. By temporarily retaining a few select teeth solely for the purpose of supporting the temporary bridge while the implants healed, he was able to have a cemented-in (fixed) temporary bridge from the very first day until the day he wore his final permanent bridge home.

    Beautiful restoration!! I have been in a battle with periodontal disease for nearly 20 years which began after having a total hysterectomy for the treatment of early stage endometrial cancer. Since this was considered an estrogen feeding cancer my doctor would not prescribe hormone replacement post op to reduce my risk for recurrence. Although this surgery saved my life, it did have a negative impact on my bones most notably my dental bones. Within two years of my surgery I was referred by my dentist to a periodontist for consultation. The periodontist said due to my deep pocketing a deep cleaning would be no help, but he felt I was a good candidate for LANAP surgery which was relatively new at that time. I chose to go forward with the LANAP surgery which took a year to complete as I opted to do a quadrant at a time to allow 3 months healing time which was my periodontist's protocol to reduce the risk of infection. All my surgeries went well, and during the procedures and after I was religious with my home maintenance and recommended follow up which was cleanings 4 x a year which I alternated having that done with my periodontist and general dentist. Around 3 years post op my periodontist abruptly retired due to a serious health issue, and the periodontist he sold his practice to I did not care for, and as a result I did not continue care with him. I discussed this issue with my general dentist and his hygenist and they agreed to take over my periodontal maintenance with 4 x a year cleanings and exams. As the years progressed my dentist extracted my 3 remaining wisdom teeth due to mobility that I should have had these teeth extracted years ago. And I had a left upper molar on a heavily filled tooth extracted that continually got infected. I opted to not have an implant or bridge replacement of that tooth because it was not in my smile line and it did not create an issue with my chewing. Although LANAP is more gum sparing than osseous surgery, I did have some additional recession following the LANAP of primarily my 4 top front teeth that I had porcelain veneers placed on them in my early 30s as a cosmetic fix for a slight overbite that developed from me being a thumb sucking difficult child that refused braces. I also developed mobility of my two front lower teeth following LANAP which a splint was placed for stability and I never had any issues with those teeth after the splint. Last year I retired from a 44 year R.N. career and my husband and I decided to relocate from R.I. to central Florida for retirement. With relocating to a new state I had to get all new healthcare providers including a new dentist. My new dentist who I very much like is concerned over a few tiny cavities that I have gotten in some exposed roots area of my teeth with veneers, and also she is concerned about the significant bone loss and chronic infection above one of my upper molar crowned teeth so she referred me to a prosthodontist for a consultation that has yet to take place. I know I am putting the cart before the horse and have no clue at this point what type of restoration I would be a candidate for, but watching enough videos including yours just the thought of All on 4s gives me nightmares. I like the bridge implant shown in this video and the rationale behind it, but what concerns me since it sits on your own gums, how would I be able prevent periodontal disease from recurring in my gums and bone again when I am already having a ongoing loosing battle with it despite having done the best personally and professionally to prevent it ??? And to make matters worse I have pretty good Metlife high option dental insurance from my former employer being the Federal Government's group plan that would cover some of the cost, and I have the means to pay the remaining out of pocket the costs of at least for now my upper arch. But to spend +/- $60k for a bridge or any other restoration that for me may doomed for failure because of my predisposition for periodontal disease is indeed a concern for me. And the last thing I want is to end up worse off down the line than where I started from. Thanks in advance for reading my lengthy comment and any feeback to it.

    Hello DR McFadden . Thank you for the videos that show. The absolutely beautiful natural look & I imagine the feel too. The comparison to the atrocious so called all on X ON. I cannot express enough how you have saved me from travelling from the Gold Coast QLD Australia to either Asia or India. I’m a female 66 years of age & I have 9 teeth on top & 8 teeth on bottom. The thought of travelling alone & having the extensive & expensive dental work done did actually scare me some. After watching your videos & listening to good advice, detailed information & outcome you attain is wonderful. Now I only hope you can help me by suggesting a Doctor in Australia whom you know is capable and can help achieve implant bridge with the same outcome you achieve. There are many dental implant centre opened up around Australia. I need help in finding the right one though. A dentist that can perform the same outcome you do. I only hope you have colleagues which have the same understanding and want dental bridge work that they would want if they needed to have same.PLEASE PLEASE PLEASE 😁😩😁

    Yes. Bone width and depth largely determines the size we use.

    Zygomatic implants are used when there is a severe lack of bone anywhere else. An implant bridge could NOT be fabricated on zygomatic implants.

    Not better, but they are less expensive. Overdentures are just dentures, that snap onto implants and are removable. You have to take them out to clean them. They are a better option than conventional dentures, though, and for people who can’t afford implant bridges (or All-On-4), they are a reasonable alternative.

    I am missing tooth numbers 3 & 4. I am wanting a dental implant in tooth # 3 with a bridge to tooth # 4. Is that possible, bc I've noticed most are 3 teeth/2 implants per dental implant bridge? Tooth # 5 & 2 are healthy. Thank you 😊.

    It's all dependent upon your bone volume. A CT scan will tell us yes or no. Most likely, yes.

    @@davidmcfadden7927 awesome! I appreciate your response 😊. I live a few hours away from your office and was thinking about driving in for a consult. We have no one. I had a facial & maxillary oral surgeon place 4 bottom implants several years ago and they're doing great, but I'm not sure about them doing the uppers bc these teeth are directly below a huge maxillary cyst. Hasn't drained in years but I do get constant sinus infections on that side from the cyst. Wasn't sure if that's something you deal with or not or if I need another specialist? I think she said I had pretty good bone structure back then but I'm planning on getting a cat scan for my ear, nose and throat Dr. 😊

    Nice case presentation. However looking at his oral hygiene maintenance, it is poor especially around the lower teeth, with visible plaque and inflamed papillae , I hope he doesn’t end up with peri-implantitis in future. He needs more motivation and education to maintain good oral hygiene.
