That's probably because you spammed rotate or have rotated beforehand. If you rotated twice, even when the Z piece looks the same, it's facing down and the spin will not work

it's because of the kick table, you probably rotated already without knowing and it won't work

I know you've probably solved your problem already but you need the seemingly symmetrical piece to actually be rotated the right way. CW and CCW are not interchangeable.
