TEG Explained - Understanding the Thromboelastography
Let's talk TEG, also known as the thromboelastography! The popularity of the TEG is growing in the critical care environment. The TEG takes a complex look at our patients entire clotting process and gives us useful values as well as a visual representation of this process helping us to better identify and treat various clotting disorders that could be going on with them. In this lesson I break down the values and appearance of our normal TEG tracing, as well as talk about various tracings that we can see depending what is happening with our patients, as well as the potential treatments in those scenarios.

Omg! Thank u for going in depth with this TEG. I always encounter this when CVT surgeons ask for this prior to heart surgery. We don't do this test in our hospital and replace it with another test. I am learning so much in your videos!

Wow! Thank you for this video! A lot of nurses don’t know what this really is (including myself) and it’s great to get some education on it! Looks pretty complex but broken down like this is very helpful in the basic understanding. Now I can look at my patients TEG value and know what I’m looking at 😁

You have never heard about TEG because it was recognized as a valid method by FDA, although it was discovered in the 1940's. Option 1: it's a brilliant technique that nobody recognized as brilliant for, hm, 70 years? Option 2: this is overengineered, absurdly complex method that cannot work.

In DIC, all coagulation factors are decreased. I was wondering how come R is shortened in the DIC example you showed. Shouldn't it be prolonged?
Wonderful video! Do you think using TEG exclusively in cirrhotic patients or patients on DOACs who are bleeding (where INR is not useful), may be useful to guide therapy besides just giving PCC and Vit K?
👉R 5 to 10min 👉K initiation to 20mm strength point1 to 3 min (Cryo ppt for fibrinogen) 👉Alpha angle 50 to 70° Cryo of reduced 👉MA 55 to 73mm platelet and DDAVP

Oh Dear, Today I was night duty and all what I was thinking of is you and TEG and really how important it is. We had that DIC patient ,in which Doctor took the decision to administer tranixemic acid regularily. I tried to discuss and explain but he shut me off. I hope she will make it without perminent disability :( .

Is TEG something done at bedside/in surgical suite or does lab typically handle it?

Both! At least where I am, in the OR they have a machine to be able to run right there. In our units, we send the specimen off to lab to be run.

This is so complex, so dependent from viscosity, temperature... There is no way in the Universe that this thing can be reliable.
Please cover the individual channels - CK, CKH, CFF etc. Also it's "aminocaproic", not "aminocaporic" acid All good otherwise !
TXA should not be routinely given in DIC!
